Probable cause
FUEL STARVATION CAUSED BY A LOOSE FUEL LINE 'B' NUT, DUE TO IMPROPER MAINTENANCE; AND THE FAILURE OF THE PILOT TO CORRECTLY JUDGE THE LANDING FLARE DURING THE AUTOROTATION.
