Probable cause
THE PILOT'S IMPROPER IN-FLIGHT PLANNING/DECISION BY FAILING TO ALLOW ENOUGH TIME FOR THE ROTOR WASH TO DISSIPATE BEFORE ATTEMPTING THE APPROACH. THE WAKE TURBULENCE (ROTOR WASH) WAS A FACTOR IN THIS ACCIDENT.
