THE PILOT IN COMMAND'S FAILURE TO MAINTAIN AN ADEQUATE VISUAL LOOK-OUT DUE TO HIS SELF IMPOSED PRESSURE OF TAXIING, COMPLETING COCKPIT CHECKLISTS AND OBTAINING A CLEARANCE AT THE SAME TIME.
