Probable cause
THE PILOT'S FAILURE TO MAINTAIN AIRSPEED (VSO) RESULTING IN A STALL/MUSH. CONTRIBUTING TO THE ACCIDENT WAS THE NEAR TOTAL LOSS OF ENGINE POWER DUE TO SEPARATION OF ONE OF THE TWO COUNTERWEIGHTS FOR UNDETERMINED REASONS.
