Part: CYLINDER — CRACKED (ENGINE)
CYLINDER DEVELOPED CRACK FROM INTAKE SEAT AROUND THE OUTER PORTION OF CYLINDER TO SPARK PLUG HOLE. CRACK FOLLOWED THE PATH OF 1 OR 2 FINS AND WENT JUST AFT (CASE SIDE) OF FUEL INJECTOR PORT. THE CRACK WAS ALLOWING NOTICABLE FUEL LEAK/STAIN NEAR INJECTOR PORT AND AT THE 3 O`CLOCK POSITION FOLLOWING THE FINS TO THE RT. THE ENGINE HAD BEEN ON AIRCRAFT AS A REBUILT EXCHANGE SINCE JULY 2004. Part: ROTOR — FAILED (MAGNETO)
RT MAGNETO HAD EXCESSIVE DROP AFTER STARTUP. TRIED CLEARING ENG AND FOUND ENGINE DIED ON THAT MAG. FOUND MAGNETO DISTRIBUTOR GEAR HAD 13 TEETH MISSING (ALL PIECES RECOVERED FROM THE MAG). REASSEMBLED WITH NEW PART. PILOT FLEW ABOUT AN HOUR NO PROBLEMS. NEXT MORNING LT MAG FAILED. FOUND 5 TEETH MISSING (ALL PIECES RECOVERED FROM THE MAG.) BOTH STARTS WERE IN AM WITH AN OVERNIGHT TEMP DOWN TO ABOUT 20 DEG F. POSSIBLE ROTORCRAFT HARMONICS MAY CAUSE THE NYLON GEAR TO HAVE THE CORRECT AMOUNT OF GREASE FROM THE 500 HR INSP. CONFIRMED MAG GEAR HOLDING TOOL (SOMETIMES USED DURING STABBING MAG TO ENGINE) WAS NOT USED OR NOT A CAUSE SINCE THE DAMAGED TEETH CLOCK ANGLE WAS NOT NEAR THE (WINDOW) TIMING TEETH.
Part: WHEEL — FAILED (MLG)
WHEEL FAILED WHILE TAXIING. APPARENTLY, ONE OF THE 6 BOLTS ON OUTSIDE FLANGE LOOSENED TORQUE OVER TIME. A FEW BOLTS BROKE AND CENTER HUB FRACTURED AT REMAINING BOLT LOCATION. SUBMITTER RECOMMENDED EITHER A LIFE-LIMIT ON PART, OR THE OPTION TO INSTALL BOLT THROUGH ASSEMBLY TO BRAKE SIDE OR INCREASE INSPECTION TIMES.
