The pilot's failure to maintain directional control after the airplane encountered a wind gust during takeoff roll. Contributing to the accident was the pilot inadvertently using a runway that resulted in a gusty tailwind condition.

AFTER TAKEOFF, GEAR WAS SELECTD UP AND THE GEAR UNLOCK LIGHT ILLYMINATED. CYCLED THE GEAR ONCE WITH THE SAME EFFECT. AT LANDING, GEAR WAS SELECTED DOWN WITH 3 GREEN. MAINTENANCE FOUND MAIN GEAR ACTUATOR BROKE FROM WING ATTACH POINT. COMPONENT HAD 6016 LANDINGS AND LIFE LIMIT FROM CESSNA IS 22,300 LANDINGS. PILOT NOTED LANDING SURFACE AT KASX TO BE IN ROUGH SHAPE.

WHILE LANDING NOSE GEAR FAILED TO EXTEND PILOT LANDED AIRCRAFT WITH NOSE GEAR UP, BOTH MAINS DOWN AND LOCKED. FOUND NOSE GEAR PUSH PULL TUBE JUST FWD OF THE GEAR BOX TO BE BROKEN BY THE BEARING ATTACH POINT AT THE IDLER BELLCRANK. PROBABLE CAUSE: AGE, HOURS, CYCLES. TO PREVENT THIS RECURRENCE, SUGGEST REPLACING THIS PART AT 15,000 HRS. (K)

NOTICED NUMEROUS OF OUR MFG BRAKE DISC, PN 164-00806 BEING CRACKED. THE CRACKS BEGIN WHERE MFG NAME IS STAMPED ON THE BRAKE DISC. FOUND 3 CRACKS WHERE THE LETTER (P) IS STAMPED AND ONE CRACK WHERE THE LETTER (R) IS STAMPED. PROBABLE CAUSE IS THE STAMPING LOCATION SETS UP A STRESS POINT. RECOMMEND DIFFERENT STAMPING LOCATION TO PREVENT RECURRENCE. (K)
