ON THE AFTERNOON OF APRIL 30TH 2025 AT 1500 EST, AIRCRAFT N799WW DEPARTED KHXD HEADED TO KCLE. AT APPROXIMATELY 19,000 FEET THE CREW HEARD A LOUD BAND AND SAW THAT THE LH WINDSHIELD WAS SHATTERED. THERE WERE NO ABNORMAL INDICATIONS OR PRIOR WARNINGS. THE CREW IMMEDIATELY DESCENDED TO 10,000 FEET AND DECLARED AN EMERGENCY. THEY THEN PROCEEDED TO THE PLANNED DEPARTURE ALTERNATE KSAV. AIRCRAFT N740KD WAS REPOSITIONED TO RECOVER THE FLIGHT. MAINTENANCE CALLED AN AOG COMPANY TO EVALUATE THE WINDSHIELD. THEY COULD NOT DETERMINE WHAT CAUSED THE WINDSHIELD TO SHATTER. THEY REMOVED AND REPLACED THE LH WINDSHIELD WITH A NEW WINDSHIELD PER THE AMM. SEE ATTACHED LBE FOR WORK PERFORMED. THE AIRCRAFT IS SCHEDULED TO RETURN TO KCLE 5/5/2025 AT 1000 EST. WINDSHIELD IN SERVICE SINCE 2016.
DURING FLIGHT FROM BCT TO CLE AIRCRAFT EXPERIENCED UNEXPECTED LOSS OF PRESSURIZATION. AIRCRAFT DIVERTED TO KSAV FOR EVALUATION. AIRCRAFT WAS INSPECTED AND A FERRY FLIGHT TO CLE WAS COMPLETED. IT WAS DISCOVERED THAT THE ECS MUFFLER HAD CRACKED ALONG A BEND LINE OPENING UP AND ALLOWING CONDITIONED AIR TO BE UNRESTRICTED. MUFFLER WAS REPLACED AND AIRCRAFT RETURNED TO SERVICE.
THE LOWER ENGINE MOUNT WAS FOUND TO BE CRACKED DURING MAJOR ENGINE INSPECTION NDT TASK 530017.
STARTER/GENERATOR ORDERED AND RECEIVED. UPON INSPECTION OF STARTER/ GENERATOR IT WAS FOUND THAT ONE OF THE FOUR BRUSH TERMINALS WAS LOOSE AND NOT MAKING PROPER CONTACT. FURTHER TROUBLE SHOOTING NOTED THAT THE MOUNTING SCREW WAS TIGHT BUT THE BRUSH HOLDER ATTACH LUG WAS LOOSE.
THE INTEGRATES STANDBY INSTRUMENT ISI FAILED AND WAS REPLACED WITH AN INSPECTED PART. THE INSPECTED ISI FAILED DURING NEXT FLIGHT. UPON INQUIRING ABOUT THE INSPECTED ISI WE WERE NOTIFIED THAT IT WAS ORIGINALLY REMOVED FOR THE SAME FAILURE AND WAS NOT DUPLICATED DURING A BENCH TEST SO IT WAS RELEASED AS INSPECTED. WE REMOVED THE INSPECTED ISI WITH A NEW ISI. ORIGINAL DISCREPANCY HAS BEEN CORRECTED.
REAR BAY OVERHEAT ILLUMINATED DURING DEPARTURE CLIMB TO CRUISE ALTITUDE.
LOSS OF PRESSURIZATION
LOSS OF MAIN HYDRAULIC PRESSURE WHILE IN CRUISE FLIGHT.
REASON FOR FAILURE: PULLED DC MOTOR FROM PUMP ASSEMBLY AND FOUND 1 OF 4 BRUSHES TO BE STUCK IN HOUSING WHICH MADE CONTACT WITH ARMATURE NOT POSSIBLE. THEREFORE WITHOUT PROPER POWER BEING APPLIED TO THE ARMATURE IT EITHER DID NOT SPIN AT ALL OR AT A VERY REDUCED SPEED CAUSING LOW FUEL FLOW TO THE ENGINE DRIVEN FUEL PUMP.
ONLY TWO GREEN LIGHTS AFTER SELECTING GEAR DOWN. RIGHT MAIN GEAR NO INDICATION.
CABIN PRESSURE LOW LIGHT ILLUMINATED.
