A loss of engine power due to the failure of the manufacturer to reinstall the oil filter after it was removed for inspection, which led to coke pollution that obstructed the oil jet and resulted in the subsequent oil starvation of the axial compressor rear bearing oil and its subsequent failure.

A loss of tail rotor effectiveness, which resulted in an uncontrolled rotation and descent into terrain. Contributing to the accident was a sudden shift in wind conditions and the helicopter's high power demand condition due to its load weight and operating altitude.

A loss of throttle control movement during a practice autorotation that was planned to terminate with a power recovery for reasons that could not be determined because postaccident examination did not reveal any anomalies that would have precluded normal operation. Contributing to the accident was the soft terrain, which resulted in the skid getting caught, the nose-down movement of the helicopter, and subsequent main rotor contact with the tailboom.
