NTSB accidents & incidents

Incident · 2015-08-17 Operator named in NTSB report
QUEST KODIAC 100 (N771RT) No injuries Part 135

Operated by Arrow West Aviation (per NTSB report)

The pilot's inadvertent failure to engage the seat in the locked position before takeoff, which resulted in the seat sliding back during the takeoff roll and a subsequent loss of directional control. Contributing to the incident was the seat locking mechanism's design of limited feedback, which made it difficult to verify if the seat stops were positively engaged.

Maintenance disclosure history

Service Difficulty Reports are self-reported maintenance findings. The NUMBER of reports is not a measure of reliability — diligent operators file more, not fewer. These are shown for transparency, not as a negative signal. To verify a report, search its control number at the FAA SDRS site.

Reports span 2009–2018. Most-reported systems: ATA 85 — 4; Equipment & furnishings (25) — 1; Landing gear (32) — 1. Most-common natures of condition, as filed: OTHER — 5; FLUID LOSS — 1.

6 reports on file.
