The pilot's inadvertent failure to engage the seat in the locked position before takeoff, which resulted in the seat sliding back during the takeoff roll and a subsequent loss of directional control. Contributing to the incident was the seat locking mechanism's design of limited feedback, which made it difficult to verify if the seat stops were positively engaged.

The pilot's decision to attempt a takeoff at a density altitude outside of the takeoff performance envelope of the aircraft along with using an aircraft flap setting higher than the manufacture's recommended setting for takeoff.

Part: BRAKE CALIPER — LEAKING (LEFT WHEEL)
LEFT BRAKE FAILED, IT HAD WORKED FINE DURING RUN-UP. PILOT TOOK OFF AND THEN FOUND IT INOPERATIVE ON LANDING. LANDING WAS UNEVENTFUL.

DURING AN ANNUAL INSPECTION, THE TECHNICIAN DROPPED THE OIL ON THE AIRCRAFT AND CLOSED THE SUMP DRAIN. LEFT FOR THE EVENING. UPON RETURN, NOTICED OIL LEAKING FROM THE SUMP DRAIN. REMOVED THE QUICK DRAIN AND FOUND A PIECE OF BROKEN SAFETY FROM A BOLT HEAD STUCK IN THE DRAIN. UPON FURTHER INSPECTION WITH A BORE SCOPE, FOUND THE DIFFUSER PLATE INSIDE OF THE OIL SUMP HAD SHED A SAFETY FROM THE RETAINING BOLTS THAT HOLD THE PLATE. ALONG WITH THE DIFFUSER BEING CRACKED AND BROKEN IN MULTIPLE PLACES. LYCOMING SAID THAT THEY HAD CHANGED THE METALLURGY IN THIS PART.

NR 4 CYLINDER HEAD SEPARATED FROM BARREL DURING NORMAL OPERATION. AD2009-26-12 WAS COMPLIED WITH 24.5 HRS PRIOR WITH NO INDICATIONS STATED IN AD THAT WOULD INDICATE CYLINDER WOULD FAIL.

FUEL STAIN ON OUTER ALUMINUM CASTING, CRACK FOUND BETWEEN COOLING FINS NEAR UPPER SPARKPLUG HOLE AND FUEL INJECTOR PORT. CRACK CONFIRMED BY PRESSURIZING INDUCTION SYSTEM AND SOAP BUBBLES. CRACK ORIGINATED IN THE INTAKE PORT AREA. THIS IS THE 3RD CYLINDER REPLACED ON THIS SET OF CYLINDERS. THE LAST CYLINDER SEPARATED DURING FLIGHT, (NR 5 POSITION), CAUSING LOSS OF POWER AND FORCING AN EMERGENCY LANDING. THE FIRST CYLINDER FAILED LEAK DOWN TEST, (NR 4 POSITION), (LEAKAGE WAS DUE TO EXHAUST VALVE).
