CRACK EXCEEDING ALLOWABLE LIMITS.
LOSS OF PILOT SIDE BRAKES. PILOT NOTICED IN FLIGHT THAT BRAKES HAD LOSS PRESSURE. MAINTENANCE IN DURANGO CO, INSPECTED SYSTEM AND DETERMINED THERE HAD BEEN AN AIR BUBBLE IN THE SYTEM THAT WORKED ITS WAY TO THE PILOTS MASTER CYLINDERS CAUSING LOSS OF BRAKES. THIS IS A RECURRING ISSUE WITH PIPER MERIDIANS.
TOTAL LOSS OF PILOT SIDE BRAKES. DISCOVERED DURING FLIGHT.
DURING ANNUAL INSPECTION, A LONGERON WAS FOUND CRACKED AT THE AFT MLG ATTACHMENT HOLE ON BOTH SIDES.
FLIGHT DEPARTED, WHILE AT CRUISE, THE PILOTS (LT) WINDSHIELD INNER PANE SHATTERED. THE PILOT FOLLOWED THE EMERGENCY PROCEDURES IN THE FLIGHT MANUAL AND DIVERETED. THE LANDING WAS UNEVENTFUL AND THE ACFT WAS SECURED FOR REPAIRS.
APPROX 8 INCHES OUT FROM BLADE CUFF IN CTR OF THE BACK OF THE PROP FOUND TO BE AN AREA OF DELAMINATION 1 INCH DIAMETER.
CREW REPORTED STRONG SMELL OF FUEL IN CABIN. MX INSPECTED FOUND FUEL LEAKING ONTO CARPET ON THE FWD LT SIDE OF CABIN. TRACED LEAK TO THE VENT LINE HEADER TANK ATTACH FITTING. FITTING FOUND TO BE TIGHT BUT LINE CRACKED AT COMPRESSION SLEEVE CONNECTOR. LINE AND CONNECTOR REPLACED NO LEAKS NOTED.
