NTSB accidents & incidents
Operated by D&D Aviation (per NTSB report)
The fatigue fracture of the fan blade due to the blade vendor's improper setup of the forging dies, which resulted in fan blades with airfoils that were below the minimum thickness in the highest stressed area of the airfoil and resulted in operating stresses that exceeded the material's capability. Contributing to the incident was the engine manufacturer's lack of a dimensional inspection of the fan blade's airfoil.
Operated by D & D AVIATION (per NTSB report)
an uncontained failure of the second stage turbine wheel due to fatigue. Factors were: obstructions in the forced landing area and the inability of the airplane to climb after the turbine wheel failure.

Maintenance disclosure history
Service Difficulty Reports are self-reported maintenance findings. The NUMBER of reports is not a measure of reliability — diligent operators file more, not fewer. These are shown for transparency, not as a negative signal. To verify a report, search its control number at the FAA SDRS site.
Reports span 2014–2014. Most-reported systems: Hydraulic power (29) — 1. Most-common natures of condition, as filed: FLUID LOSS; WARNING INDICATION — 1.

Part: LINE — CHAFED (HYD SYSTEM) · airframe 2,054 hrs · 1,846 cycles
AFTER TAKEOFF AND GEAR RETRACTION, PILOT REPORTED RECEIVING A "LOW HYDRAULIC FLUID LEVEL" INDICATION. EXTENDED THE MLG WITH NORMAL GEAR DOWN INDICATION AND RETURNED TO AIRPORT. ON EXAMINATION, THE NLG WAS FOUND TO BE COATED WITH HYDRAULIC FLUID. THE HYDRAULIC SYSTEM WAS SERVICED AND THE GEAR WAS RETRACTED. NOTED THAT THERE WAS HYDRAULIC FLUID SPRAYING FROM ONE OF THE NLG UPLOCK ACTUATOR HYDRAULIC LINES. THE LINE WAS REMOVED AND FOUND TO BE CHAFED THROUGH WHERE THE LINE PASSED OVER THE NLG SHIMMY DAMPER ATTACH POINT. A NEW LINE WAS INSTALLED.
