The flight crew’s decision to descend below the published MDA, cancel their IFR clearance to conduct an unauthorized circle-to-land approach to another runway while the airport was in nighttime IFR conditions, and the exceedance of the airplane’s critical angle of attack, and subsequently entering an aerodynamic stall at a low altitude. Contributing to the accident was the tower crew’s failure to monitor and augment the airport weather conditions as required, due in part to, the placement of the AWOS display in the tower cab and the lack of audible AWOS alerting.

Part: DOOR — OUT OF RIG (LT MLG)

WHILE CONDUCTING A TRAINING EXERCISE, CREW SIMULATED AN ENGINE OUT WITH THE LANDING GEAR DOWN. DURING COURSE OF THE SCENARIO, THE GEAR WAS RETRACTED. THE CREW NOTED THE HYDRAULIC PUMP STAYED ON UNUSUALLY LONG. THE GEAR WAS PUT BACK DOWN AND AIRCRAFT SOON AFTER LANDED. UPON INSP, LT INBOARD GEAR DOOR EDGE WAS FOUND BENT UP. SUBSEQUENT INSP FOUND THE OVERCENTER CONDITION OF THE GEAR DOOR RIGGING WAS NOT RIGGED PROPERLY AND THE YAW CONDITION DURING GEAR UP OPS CAUSED THE WHEEL TO CATCH THE DOOR. ACFT GEAR DOOR RIGGING HAD JUST BEEN DONE AT RECENT INSPECTION. (X)
