IN ROUTE TO SCENE#2 ENGINE CHIP ILLUMINATED. PILOT ATTEMPTED TO BURN OFF CHIP BUT LIGHT CAME RIGHT BACK ON. PILOT ATTEMPTED A SECOND BURN AND AGAIN THE LIGHT CAME BACK ON. PILOT NOTIFIED ECC THEY WERE RETURNING TO BASE. PILOT VERIFIED STEPS WITH EMERGENCY CHECKLIST AND SHUT DOWN ENGINE #2.PILOT NOTIFIED TOWER AND DECLARED AN EMERGENCY FOR SINGLE ENGINE OPERATION. PILOT PERFORMED APPROACH TO RUNWAY. PRIOR TO TOUCHDOWN AIRCRAFT HAD SUFFICIENT POWER TO CONTINUE, PILOT CONTINUED TO TAXIWAY AND LANDED WITHOUT INCIDENT. MAINTENANCE MOVED AIRCRAFT TO HANGER. INSPECTED ENGINE MAGNETIC AND ELECTRICAL CHIP PLUGS I/A/W SAFRAN MM. ALSO CONFERRED WITH SAFRAN TECHNICAL REPRESENTATIVE. DETERMINED SIZE AND AMOUNT OF CHIPS TO BE EXCESSIVE AND THE AIRCRAFT WAS REMOVED FROM SERVICE. CHIP SAMPLES WERE SENT IN TO FOR ANALYSIS. RENTAL ENGINE WAS PLACED ON ORDER AND ENGINE CHANGE PERFORMED.
OVER THE COURSE OF 24 HOURS LIQUID OXYGEN CONVERTOR EXPELLED ALL LIQUID OXYGEN DUE TO A FAULTY VENT RELIEF VALVE.
WHILE DOING A PATIENT FLIGHT AT APPROXIMATELY 16:45 CENTRAL TIME, AIRCRAFT TOTAL TIME OF 2872.2 ONE OF OUR FLIGHT NURSES STARTED MENTIONING HOW OUR MEDICAL BED FELT RATHER WARM. AS I LOOKED BACK INTO THE CABIN I NOTICED A FAINT HAZE. THE FLIGHT NURSE STARTED TO LOOK FOR THE SOURCE AND NOTICED THE TEMPERATURE ON THE IV WARMER WAS READING AROUND 300 DEGREES. AS HE OPENED THE DRAWER TO THE WARMER A SIGNIFICANT AMOUNT OF SMOKE CAME OUT OF THE DRAWER ALONG WITH THE SMELL OF MELTED PLASTIC. WE MADE A DECISION TO DIVERT. WE INFORMED ATC OF OUR INTENTION AND DECLARED AN EMERGENCY. ABOUT HALFWAY THROUGH OUR DECENT THE FLIGHT NURSE WAS ABLE TO ISOLATE AND UNPLUG THE IV WARMER. WE CONTINUED ON AND A LANDING WAS MADE WITHOUT FURTHER INCIDENT. UPON SHUTDOWN WE VENTILATED THE AIRCRAFT AND HAD LOCAL MAINTENANCE DEFERRED THE IV WARMER ACCORDING TO THE MAYO CLINIC NEF PROGRAM. WE CONTINUED ON TO THE DESINATION WITH NO FURTHER INCIDENTS. IV WARMER HEAT PAD FAILED AND OVERHEATED, SUBSEQUENTLY MELTING MEDICATION FLUID BAG CAUSING SMOKE.
IN FLIGHT, AFT CABIN CREW DETAILED A BURNING SMELL FOLLOWED BY SMOKE (DESCRIBED AS A HAZE) DEVELOPING IN THE AFT CABIN. CREW MEMBER STATES LOOKED LIKE THE MAJORITY OF THE WHISPING SMOKE MAY HAVE COME FROM THE RT SIDE OXYGEN PANEL. A PRECAUTIONARY LANDING WAS PERFORMED WITHOUT INCIDENT. MAINTENANCE WAS NOTIFIED AND PERFORMED A THOUROUGH INSPECTION OF THE RT MED WALL WITH NO INDIATIONS FOUND OF A BURNING AREA. THE ENTIRE AFT CABIN INTERIOR WAS REMOVED AND ALL WIRING WAS INSPECTED FOR BURNING. THERE WERE NO DISCREPANCIES FOUND AND AFTER 1.5 HOURS OF GROUND RUNS THE AIRCRAFT WAS RETURNED TO SERVICE.
WHILE FLYING, CREW IN THE BACK ANNOUNCED THAT THEY SMELLED SMOKE. TWO SECONDS LATER THEY ANNOUNCED THAT THEY SAW SMOKE IN THE AFT CABIN. SHUT OFF THE AIR CONDITIONER. INFORMED THE CREW THAT WE WOULD BE MAKING A PRECAUTIONARY LANDING. ALREADY ON FREQUENCY WITH TOWER, TOLD THEM MAKING A PRECAUTIONARY LANDING TO THE RAMP. SHUTDOWN & COULD NOT SMELL SMOKE ANYMORE. NEVER SAW SMOKE, ONLY SMELLED IT.
ENROUTE, APPROXIMATELY 2 MILES NORTHWEST OF AIRPORT, A STRONG ODOR OF SOMETHING HOT WAS NOTICED IN THE COCKPIT. THE FLIGHT WAS ABORTED AND AN UNEVENTFUL LANDING WAS MADE. MAINTENANCE FOUND THE ODOR COMING FROM THE INSIDE OF THE VEMD.
UPON TAKEOFF, FIRE INDICATION ON NR 1 ENGINE FLICKERED ON AND OFF FOR 5-10 SECONDS, PILOT TURNED AND LANDED, FIRE INDICATION GONE UPON LANDING, LANDED WITHOUT INCIDENT, TROUBLESHOOTING INDICATED LOWER AFT FIRE DETECTION PROBE CREATED WARNING PREMATURELY, REPLACED LOWER AFT FIRE DETECTION PROBE ON NR 1 ENGINE.
