The pilot's inadequate compensation for the crosswind condition and failure to maintain directional control during the landing roll. Contributing to the accident was the crosswind condition.

the failure of both the flight instructor and student pilot to regain control of the airplane in a timely manner during an intentional spin maneuver, resulting in a collision with terrain. A factor in the accident was the instructor's inadequate supervision of the flight.

The incorrect use of an abbreviated callsign by the TRACON sector radar controller to issue a descent clearance to N434PA and the sector controllers failure to detect that the clearance had been read back with the full callsign by the pilot of N304PA. Also causal was the pilots failure to question a clearance that descended them below the published Minimum En route Altitude (MEA). Contributing to the accident was the failure of both the Center and TRACON controllers to properly respond to the aural and visual Minimum Safe Altitude Warning System (MSAW) alert.

The flight instructor's failure to comply with the instrument approach procedure in that he descended prematurely below decision height resulting in an in-flight collision with trees, power lines and the ground. Contributing factors were the dark night light condition and the low ceiling.

The disengagement of the left control wheel from the control column, which was due to the attaching screw backing out of the nut plate. Also causal was the manufacturer's inadequate design of the installation and the improper length of the screw.
