FUEL SMELL IN CABIN DURING FLIGHT, AWAITING RESOLUTION DETAILS FROM AMT
MED CREW DETECTED A FUEL SMELL COMING FROM THE AIR CONDITIONING VENTS IN THE CABIN WHILE FLYING. AIRCRAFT IS CURRENTLY OUT OF SERVICE FOR TROUBLESHOOTING THIS FUEL SMELL. SUP INFO: INSPECTED AIRCRAFT AND PERFORMED A CHECK FLIGHT FOR ONE HOUR. FOUND NO EVIDENCE OF FUEL OR FOREIGN SMELLS. UNABLE TO DUPLICATE
CREW REPORTED SMELL OF EXHAUST IN THE BACK OF THE HELICOPTER DURING FLIGHT. PILOT DIDN'T SMELL IT IN THE FRONT. AMT NOTES: INSPECTED ALL FUEL LINES AND FUEL VENT PORTS WITH ENGINE ILLUMINATION 100%, NO LEAKS OR FUEL SMELL DETECTED, AIRCRAFT RETURNED TO SERVICE.

R/H SPLICE TO ROOF BOX BEAM FITTING HAS MULTIPLE CRACKS AND R/H SIDE WEBBING ON THE VERTICAL BOX ASSY. UPDATE TO ORIGINAL SUBMISSION UNIQUE ID ENTERED INCORRECTLY AS QMLA202120069, SHOULD BE AS SHOWN AS QMLA 2021123020069
FOLLOWING A RECENT SIMILAR EVENT, THE SAME CREW MEMBER SMELLED FUEL IN FLIGHT ONCE THE AIRCRAFT REACHED APPROXIMATELY 100 KIAS STRAIGHT AND LEVEL. THIS TIME THE SMELL APPEARED TO BE MUCH STRONGER. THE PILOT IMMEDIATELY RETURNED TO BASE AND CONTACTED THE MECHANIC. HIS SUBSEQUENT INSPECTION REVEALED A BAD SEAL ON THE FUEL CAP. ANOTHER ONE WAS ORDERED AND REPLACED. THE DISCREPANCY WAS THEN SIGNED OFF, AND THE AIRCRAFT RETURNED TO SERVICE.
MED CREW REPORTED SMELLING FUEL IN CABIN DURING THE SECOND LEG OF A PATIENT TRANSPORT FLIGHT. THE AIRCRAFT WAS LANDED, AND MECHANIC WAS CALLED TO INSPECT THE AIRCRAFT. THE BAGGAGE COMPARTMENT MEDICAL SERVICES PANEL WAS REMOVED TO EXPOSE A FUEL VALVE THAT WAS THOUGHT TO BE THE SOURCE OF THE SMELL; HOWEVER NO LEAKS OR FUMES WERE DETECTED. THE MECHANIC INSPECTED THE AIRCRAFT AND RELEASED IT BACK TO SERVICE FOLLOWING A THOROUGH INSPECTION WITHOUT A SOURCE FOR THE SMELL IDENTIFIED.
ENGINE WAS MAKING UNUSUAL NOISE IN ALL PARAMETERS. WHEN ENGINE WAS SHUT DOWN, LOWER AND UPPER CHIP PLUGS WERE REMOVED AND BOTH PLUGS WERE FOUND TO CONTAIN METAL CHIPS.
TRANSMISSION CHIP LIGHT ILLUMINATED IN FLIGHT, PILOT MADE AA PRECAUTIONARY LANDING. INSPECTION REVEALED METAL IN OIL. TRANSMISSION REMOVED FOR OVERHAUL/REPAIR.
