THE DENTED AND DEBONDED AREAS ARE REPORTED TO BE FROM 0.50â X 0.50â IN SIZE TO 4.0â X 4.0â IN SIZE. A DETAILED DESCRIPTION OF THE DAMAGE AND AN APPROVED REPAIR PROCEDURE IS PROVIDED IN AIRBUS REPAIR DESIGN APPROVAL SHEET RDAS-EC135-53-2025-3327A, REV A DATED 17 OCT 2025.

LEE, DER-Y # 905240204, DATED 23 OCT 2025 WITH DER SPECIFIC SEQUENCE # 25RL023AM.
DURING INSPECTION, HEAVY CORROSION WAS DISCOVERED ON T-CAP P/N L533M1028203 ON TOP OF T-CAP UNDER MAIN CABIN FLOOR AT FS3150MM, LBL500MM - RBL500MM. THE T-CAP IS NOT AVAILABLE FOR PURCHASE.; THEREFORE, A REPLACEMENT T-CAP WAS FABRICATED AS PER THE INSTRUCTION ON AIR METHODS ENGINEERING AUTHORIZATION 23-5300-EA-5302, INITIAL REVISION DATED 20 OCT 2025 AND WAS APPROVED BY FAA FORM 8110-3 BY RYAN LEE, DER-Y # 905240204, DATED 22 OCT 2025 WITH DER SPECIFIC SEQUENCE # 25RL025AM.
SHORTLY AFTER TAKE OFF PILOT AND MEDICAL CREW NOTICED A SMELL OF. HOT ELECTRICAL WIRING/COMPONENTS IN BOTH THE COCKPIT AND CABIN AREAS, WITH NO EVIDENCE OF SMOKE OR FIRE. FOUND HOTDOG SPLICE RESTING AGAINST PILOT PITOT HEATER BREAKER CONTACT. HOTDOG SPLICE AND CONTACT SINGED. REPLACED HOTDOG SPLICE AND PILOT PITOT HEATER BREAKER IAW EC135 AMM 24-62-01 , 8-1. OPS CHECK GOOD.
#1 ENG CHIP LIGHT ILLUMINATED ENROUTE BACK TO HOME BASE. EMERGENCY PROCEDURES WERE PERFORMED IAW WITH THE RFM. LANDING AT HOME BASE WAS ACCOMPLISHED WITHOUT INCIDENT. APPROPRIATE LOGBOOK ENTRIES AND NOTIFICATIONS WERE MADE.
BIRD STRIKE IN CRUISE FLIGHT CAUSE DAMAGE TO PILOT WINDSCREEN, CO-PILOT CHIN BUBBLE. POST FLIGHT INSPECTION BY MECHANIC FOUND DAMAGE TO TWO MAIN ROTOR BLADES AND CO-PILOTS KICK/FLOOR PANEL.
A STRONG ELECTRICAL ODOR WAS NOTICED BY THE PILOT AND THE MEDICAL CREW MEMBERS ON A RETURN FLIGHT TO THE BASE. IT WAS BELIEVED TO BE COMING FROM THE AIR CONDITIONING SYS. ONCE ON THE GROUND AND SHUTDOWN, THE ODOR WAS STRONGEST IN THE RT CABIN OVERHEAD LOCATION. AFTER INSP, THE MECHANIC HAD FOUND THAT THE AFT AIR CONDITIONING BLOWER ASSY HAD A BADLY BURNED "MOLEX STYLE" DISCONNECT. THIS PARTICULAR DISCONNECT AND ASSOCIATED HARNESS ASSY WAS JUST REPLACED, SB EC135-8010, REV N/C, DATED 11/15/09 IN FEBRUARY 2010. THIS ACFT IS JUST 60 HOURS OUT OF AN 800 HR INSP WHERE THERE WAS NO NOTICEABLE SIGNS OF OVERHEATING IN THAT AREA. THE NEW HARNESS HAS BEEN ORDERED IAW THE PREVIOUSLY COMPLIED WITH SB.
