NR 2 ENG FADEC FAIL IN FLIGHT. SLIDE ON LANDING NO DAMAGE. TROUBLESHOT DOWN TO NR 2 ENGINE FADEC ( EECU ) INOPERATIVE. READ POWER TO CONNECTORS, SWAPPED EECU WITH NR 1 SIDE AND PROBLEM FOLLOWED. INSPECTED AIRCRAFT FOR OTHER DAMAGE OF SKIDS AND SKID PLATES. ALL CHECK GOOD. REMOVED AND REPLACED EECU. PERFORMED MAINTENANCE TEST FLIGHT AND ALL CHECKS ARE COMPLETED. THIS AIRCRAFT HAS BEEN RETURNED TO REVENUE SERVICE, REF MEC-013895. REPLACED BOTH DECU UNITS, COMPLETED SKID TUBE INSPECTION, RII CHECKLIST, GROUND RUN AND CHECK FLIGHT.
#1 ENG FADEC FAILED IN FLT.
DURING SHUT DOWN AT JACKSON GENERAL HOSPITAL AFTER DROPPING OFF PATIENT, SMELLED AN ELECTRICAL BURNING SMELL COMING FROM COCKPIT. REPLACED BLOWER MOTOR.
23-5300-EO-3820 PART 1 EC 135- FRAME 5 INSPECTION AND REPAIR. DURING THE INSPECTION, NO CRACKS WERE FOUND. HOWEVER, WE HAD ALL THE PARTS IN HAND FOR THE REPAIR AND WHEN TALKING WITH SERVICE ENGINEERING THEY INSTRUCTED US TO CONTINUE WITH THE REPAIR. THIS ACFT WAS OOS FOR AN EXTENDED PERIOD FOR 1000HR INSPECTION .THIS REPAIR TOOK PLACE DURING THE OOS EVENT.
ON THE RETURN LEG HOME I TURNED ON THE FAN TO MOVE SOME AIR AROUND IN THE COCKPIT. AFTER APPROXIMATELY 10 MINUTES I SMELLED A "HOT" ELECTRICAL SMELL COMING FROM THE AIR VENTS. I TURNED THE FAN OFF AND THE SMELL DISSIPATED. I LANDED THE AIRCRAFT BACK AT BASE WITH NO FURTHER INCIDENT.
PILOT REPORTED ALL THREE CREW MEMBERS SMELLED AN ELECTRICAL BURNING SMELL. PILOT TURNED OFF DEFOG, AND ALL VENTILATION, OPENED WINDOWS, MADE A PL AT NEAREST AVAILABLE, SUITABLE LZ. EVENT COULD NOT BE DUPLICATED. ACFT RETURNED TO SERVICE.
PERFORMED D 0560-100 HOUR/12 MONTH INSPECTION OF LANDING GEAR AND SKID EXTENSION. FOUND CRACK ON FWD LT CROSS TUBE EXTENSION. CRACK IS LOCATED ON THE BACKSIDE OF THE EXTENSION AND RUNS VERTICAL FROM THE UPPER MOUNTING BOLT TO THE TOP OF THE EXTENSION. REMOVED THE PAINT IN THE AREA WITH SAND PAPER AND VERIFIED THE CRACK WITH A LIGHT SOURCE.
DURING FLIGHT, THE MAIN TRANSMISSION CHIP LIGHT ILLUMINATED. THE PILOT ADVISED CREW THAT THE MISSION WOULD BE ABORTED AND THEY WOULD LAND. THE PILOT REDUCED POWER AND MONITORED THE TRANSMISSION OIL PRESSURE AND LANDED LANDED SAFELY WITHOUT ANY ADDITIONAL ISSUES (EMERGENCY PROCEDURES WERE USED). AFTER LANDING, MECHANIC INSPECTED THE MGB CHIP DETECTORS. STAGE A CHIP WAS FOUND. MAINTENANCE ACCOMPLISHED UTILIZING ASB EC-135-63A-01 AND RETURNED TO SERVICE WITH NO ADDITIONAL DEFECTS NOTED.
POWER TURBINE NOZZLE GUIDE VANE SEAL HAS BECOME FREE TO ROTATE BETWEEN THE REAR BEARING SUPPORT AND THE POWER TURBINE NOZZLE GUIDE VANE ASSEMBLY. CUTTING THRU THE POWER TURBINE NOZZLE GUIDE VANE AND INTO THE REAR BEARING OIL RETURN DUCT AND CAUSING AN OIL LEAK.
WHILE PERFORMING AN AIRWORTHINESS INSPECTION, MECHANIC DISCOVERED A CRACK ON THE LEFT AFT LOWER FIREWALL. THIS FIREWALL WAS REMOVED AND REPLACED. THE AIRCRAFT HAS SINCE BEEN RETURNED TO SERVICE.
PERFORMED FUZZ BURNER INSPECTION AT 0500, EVERYTHING CHECKED OUT FINE. PILOT GOT A CALL FOR A FLIGHT AND SAW A CHIP LIGHT SO HE ABORTED THE FLIGHT, UPON ARRIVAL INSPECTED AND CLEANED THE CHIP DETECTOR AND HAD THE PILOT RUN UP THE ENGINE TO ASSURE WE WERE GOOD. MOC WAS GOOD AND RETURNED AIRCRAFT TO SERVICE.
NR 2 ENGINE CHIP LIGHT ILLUMINATED IN FLIGHT. ATTEMPTED TO BURN IAW RFM WITH NO SUCCESS. LANDED IAW RFM. MX INSPECTED CHIP DETECTORS AND FOUND CLASS B CARBON LIKE PARTICLES AND WIRE LIKE FILAMENTS ON REDUCTION GEARBOX CHIP PLUG. REMOVED AND INSPECTED STARTER DRIVE MAGNETIC SEAL AND BREATHER GEAR MAGNETIC SEAL. STARTER SEAL ROTATING RING WAS FOUND WITH .015 OF FACE WORN OFF WITH METAL TO METAL CONTACT AND SAME TYPE OF PARTICLES FOUND ON PLUG. BREATHER SEAL WAS FOUND WITH ROTATING RING STARTING TO DETERIORATE. PERFORMED DRAIN AND FLUSH OF OIL, REMOVED, INSPECTED, AND CLEANED PLUGS AND STRAINERS. REMOVED, INSPECTED, AND CLEANED OIL FILTER. ENGINE IS SUBJECT TO CONTROLLED SETTING INTO SERVICE ONCE SEALS ARE REPLACED AND WILL REQUIRE 5 HR INSP OF FILTER, PLUGS AND STRAINERS FOR THE NEXT 25 HOURS TIS.
THE PILOT NOTICED A STRONG BURNING SMELL ON APPROACH. AFTER UNEVENTFUL LANDING, MECHANIC WAS ABLE TO ISOLATE THE PROBLEM TO THE SLEWABLE SEARCH LIGHT MOTOR. THE SYS WAS DEFERRED AND THE ACFT RETURNED TO SERVICE.
PIC AND MED CREW SMELLED SMOKE AND RETURNED TO, AND LANDED AT THE HOSPITAL. A MECHANIC HAS TROUBLESHOT ANY SYSTEM THAT COULD PRODUCE THE ODOR. PERFORMED A MAINTENANCE OPERATIONAL CHECK FLIGHT AND FOUND NO REOCCURRENCE OF THE ODOR. DECIDED THAT THE ECU HAD BEEN USED FOR THE FIRST TIME THIS SEASON AND THE BURNING ODOR POSSIBLY WAS FROM THE DUST THAT HAD ACCUMULATED DURING THE HEATERS NON-USE.
