DAMAGE TO THE LANDING GEAR LH OTBD AFT BEARING IN THE FORM OF GOUGE. THE GOUGE DAMAGE IS AT LOWER FLANGE OF THE BEARING AND WAS MEASURED TO BE APPROXIMATELY 0.3895â X 0.1105â X DEPTH MORE THAN HALF THE THICKNESS OF THE BEARING.
LOSS OF NR INDICATION ONE CLIMB OUT. REMOVED NR SPEED SENSOR AND REPLACED WITH NEW AS PER AIRBUS AS350B3 AMM 62-31-00, 4-3 .CLEANED AND APPLIED STABILANT 22 TO MGB HARNESS CANON PLUG. GROUND RAN AIRCRAFT. OPS CHECK GOOD.
DAMAGE TO THE TAIL BOOM AFT SKIN PANEL (PN: 350A23-0020.AL) IN THE FORM OF A CHAFE NEAR THE LOWER VERTICAL FIN MOUNTING BRACKET.
DURING FLIGHT, THE CREW REPORTED SMELLING JET FUEL COMING FROM AIR CONDITIONING VENTS. PILOT COULD NOT SMELL THE ODOR. RETURNED TO BASE. THE MECHANIC REMOVED THE LEFT SIDE INNER FUEL TANK PANELS AND AFT BELLY PANEL WAS LOWERED TO INSPECT FOR FUEL LEAKAGE, AND AN OPS CHECK WAS PERFORMED BY PRESSURIZING FUEL SYSTEM TO INSPECT FOR LEAKS. THE OPS CHECK WAS GOOD SO A GROUND RUN WAS PERFORMED, NO LEAKS OR DEFECTS NOTED. A MAINTENACE FLIGHT WAS PERFORMED TO CHECK FOR ODOR. NO LEAKS WERE FOUND AND DURING FLIGHT NO ODOR OF JET FUEL WAS DETECTED BY PILOT OR MECHANIC. PILOT REPLICATED EXACT FLIGHT CONDITIONS THAT OCCURRED WHEN MED CREW SAID THEY COULD SMELL THE ODOR.
ENROUTE, YELLOW GOV LIGHT, FOLLOWED BY GEN LIGHT ILLUMINATED. INTERCOM INOP. PERFORMED GEN RESET W/ NO RESULTS. STARTED SHEDDING POWER CONSUMERS WHEN TOTAL ELECTRICAL FAILURE OCCURRED. INFORMED MED CREW WE HAD A TOTAL ELECTRICAL FAILURE AND WE WERE GOING TO LAND AT THE AIRPORT, AND TO NOTIFY DISPATCH VIA THE HAND HELD RADIO. MADE AN UNEVENTFUL LANDING AT OUR LOCAL AIRPORT. UNABLE TO SHUT THE ACFT DOWN USING NORMAL PROCEDURES. SHUT THE ACFT DOWN BY PULLING THE MASTER FUEL SHUT OFF. GROUND AMBULANCE ARRIVED APPROX ONE MINUTE AFTER WE SHUTDOWN AND TRANSPORTED THE PATIENT TO THE RECEIVING TRAUMA CENTER LOCATED 3 MILES AWAY. MECHANIC ARRIVED AND FOUND STARTER GENERATOR PARTIALLY SEIZED.
AC WAS REQUESTED TO RESPOND TO THE AIRPORT FOR A SCENE RENDEZVOUS WITH THE AMBULANCE. AFTER DEPARTURE FROM HOSPITAL, THE MEDICAL CREW NOTICED A FUEL ODOR. THE FLIGHT WAS ABORTED AND AC LANDED AT MEDICAL CENTER WITH ALL AIRCRAFT INDICATIONS NORMAL. ON FINAL APPROACH AND LANDING, THE FUEL ODOR BECAME STRONGER AND AFTER LANDING, A LARGE QUANTITY OF FUEL WAS NOTED COMING FROM THE AC DRAINS AS WELL AS FUEL SPATTER ON THE SIDE CABIN WINDOWS. PARAMEDIC CREWMEMBER STOOD FIREGUARD AS AC WAS SHUTDOWN. AFTER SHUTDOWN THE ENGINE COWLING WAS OPENED AND LARGE QUANTITY OF FUEL WAS FOUND POOLING ON THE ENGINE DECK. MAINT HAD COMPLETELY REPLACED THE SEAL ON THE HMU EARLIER IN THE DAY AND MAINT RUNS HAD BEEN COMPLETED PRIOR TO THE AIRCRAFT BEING RETURNED TO SERVICE. THE SAME SEAL WAS AGAIN LEAKING. SEAL WAS REPLACED AGAIN.
