THE FLIGHT CREW REPORTED SMOKE IN THE COCKPIT DURING DESCENT INTO KMFR. THE AIRCRAFT LANDED IN KMFR WITHOUT INCIDENT. EMERGENCY VEHICLES WERE DISPATCHED. PDX MAINTENANCE DROVE OUT TO AIRCRAFT AND DEFERRED LH PACK. MAINTENANCE DROVE TO KMFR AND DEFERRED THE LH PACK IAW MEL 21-50-2. A/C WAS PLACARDED AND DEFERRED PER MEL AND RETURNED TO SERVICE. AIRCRAFT IS CURRENTLY DOWN, AND THIS MEL SQUAWK IS BEING WORKED ALONG WITH OTHER MAINTENANCE TASKS. SUPPLEMENTAL SDR WILL BE SUBMITTED ONCE THE AIRCRAFT HAS BEEN REPAIRED AND MEL HAS BEEN CLEARED.
NR 2 OIL PRESSURE WAS FLUCTUATING BETWEEN 55-40 PSI. NR 2 OIL PRESSURE AURAL WARNING ENGINE "OIL" SHUTDOWN. REPLACED NR 2 PCU IAW MM 61-21-00. GROUND OPS AND LEAK CHECK GOOD. PCU WAS FOUND TO BE LEAKING EXTERNALLY AT REAR OF UNIT.
DURING INSPECTION, FOUND RT VERTICAL STABILIZER TO FUSELAGE ATTACH ANGLE CRACKED AT THE AFT PRESSURE BULKHEAD.
DURING "A" CHECK, DISCOVERED A CORRODED SUPPORT OF THE MAIN CABIN DOOR WHICH REQUIRED THE SUPPORT REPLACEMENT.
DURING "A" CHECK DISCOVERED CRACKS ON LOWER WING SKIN AROUND ACCESS PANELS, 143FB AT WING STATION Y-657, 143BB LT WING STATION Y-657, AND 521AB LT WING STATION YA1359.
AILERON JAMMED AT TOP OF CLIMB. PULLED AILERON DISCONNECT. RIGHT AILERON THEN APPEARED TO MOVE FREELY. INSPECTED AIRCRAFT AND DETERMINED AILERON TRIM TAB ACTUATOR TO BE THE CAUSE.
FOUND LOOSE AND MISSING RIVET ON LOWER WING SKIN STA Y4534 AFT OF STRINGER 10. REPAIRED IAW EMB-120 SRM 51-40-03B.
EMERGENCY BUSS OFF LIGHT ILLUMINATED. REPLACED NR (2) ENGINE STARTER GENERATOR AND RESET CB0471, EMER 1 BUS CIRCUIT BREAKER. OPS CHECKED ALL BUSSES AND TRANSFER FUNCTION IAW MM 24-52-00. NO DEFECTS NOTED.
DISCOVERED UPPER RT LATCHING MECHANISM ON FWD DOOR BROKEN, REPLACED BACKSTOP/STOPPER (ASSY PN 120-25452-006) IAW AMM 52-11-00.
A DEEP NICK WAS FOUND ON THE MAIN DOOR 1.5" AFT OF FRAME 15, APPROX 12.5" ABOVE DOOR CUTOUT. REPAIRED DAMAGE IAW EO 1051 AND SRM 52-10-01.
ON GEAR RETRACTION RED GEAR LIGHTS A AND B REMAINED ON AND LT MAIN GEAR DOOR WOULD NOT SHUT. MAINTENANCE INVESTIGATION REVELED THAT THE PN 19659 BUSHINGS IN BOTH THE LT AND RT UP-LOCK HOOK SUPPORT ASSY. PN 19552 WERE AT MINIMUM OVERHAUL TOLERANCES. IN COLD TEMPERATURES (-35F TO -40F) THESE CLEARANCES WERE TOO TIGHT. BUSHINGS WERE REAMED TO THE UPPER O/H CLEARANCE LIMITS. ASSY REASSEMBLED AND OPERATIONALY CHECKED. AC RETURNED TO SERVICE WITH NO ADDITIONAL PROBLEMS. (K)
RIGHT OUTBOARD FLAP CAME DOWN 5 DEGREES SITTING ON RAMP WITH ENGINES OFF AND APU RUNNING. RAN QRH AND FLAP FAILED TO RETRACT. REPLACED RIGHT OUTBOARD FLAP ACTUATOR. OPERATED FLAPS SEVERAL TIMES. OPS CHECK GOOD. NO LEAKS FOUND.
TEN MINUTES AFTER TAKEOFF, THE RIGHT SIDE HYDRAULIC RESERVOIR LOW LEVEL LIGHT ILLUMINATED, FOLLOWED SHORTLY BY THE RIGHT SIDE MAIN PUMP LOW PRESSURE LIGHT. RETURNED TO LAX AFTER COMPLETING QRH PROCEDURES. RIGHT HYDRAULIC PUMP OUT OF FLUID APPROX 15 MINUTES. INSTALLED SERVICEABLE LEFT HYDRAULIC FLAP ACTUATOR IAW EMB MM 27-50-00. INSTALLED NEW HYDRAULIC PUMP. RAN ENGINE, NO LEAKS NOTED. CYCLED FLAPS, OPS CHECK GOOD. CHECKED ACTUATOR, NO LEAKS NOTED. ALL OPS CHECKS GOOD.
ON DOOR OPENING 3RD AND 4TH STAIRS FROM THE TOP DID NOT EXTEND. REPOSITIONED BOTTOM STEP SPRING AND REPLACED STEP SPRING ON 3RD STEP DOWN FROM TOP. OPS CHECK GOOD.
VERTICAL FIN LEADING EDGE DEICE BOOT FAILED INFLIGHT WITH BOTH TIMERS 1 AND 2. FOUND GREEN CORROSION AROUND PIN-B IN CONNECTOR AT S0213 (VERTICAL FIN PRESSURE SWITCH). REMOVED AND REPLACED CONNECTOR P1080 AND SOCKET-B IN CONNECTOR. GROUND OPS CHECK OK WITH LEFT AND/OR RIGHT ENGINE AIR.
FLAP CONTROL FAULT DURING FLAP RETRACTION AND AGAIN WHEN SETTING FLAPS TO 25 DEGREES FOR LANDING. ALL LIGHT BARS FLASHING. QRH RESET PROCEDURE WORKED BOTH TIMES.REMOVED AND REPLACED FLAP SELECTOR IAW MM 27-51-00. OPS CHECK GOOD ON GROUND.
RT NACELLE FLAP POT IS ERRATIC.REMOVED AND REPLACED RIGHT NACELLE FLAP ACTUATOR. OPS CHECKS GOOD IAW MM 27-50-08.
''HIGH SPEED'' AURAL WARNING/TRIPLE CHIME SOUNDED ON TAKEOFF ROLL - IAS APPROXIMATELY 40 KNOTS. TAKEOFF ABORTED. REMOVED AND REPLACED PILOT'S AIRSPEED INDICATOR IAW MM 34-14-00 TO START HISTORY. COULD NOT DUPLICATE BEFORE REPLACEMENT. OPS AND LEAK CHECK GOOD. P/N: 320AS1 S/N ON: AH789 S/N OFF: AH752

LEFT INBOARD FLAP ACTUATOR FEEDBACK POT FAILED INSULATION RESISTANCE CHECK PER PARKER TROUBLESHOOTING GUIDE. REMOVED AND REPLACED LEFT INBOARD FLAP ACTUATOR IAW MM 27-50-07. OPS CHECK GOOD. NO LEAKS FOUND. P/N: 320300-1005 S/N ON: 1029 S/N OFF: 0059
ON TAKEOFF ROLL CAPTAINS VSI/TCAS DISPLAY WENT BLANK AND REMAINED BLANK THROUGHOUT ABORTED TAKEOFF. STARTED WORKING DURING TAXI BACK. CHECKED SECURITY OF DISPLAY ELECTRICAL CONECTOR. CYCLED DISPLAY POWER. NO DISCREPANCIES FOUND DURING INSPECTION.
AFTER TAKEOFF, THE LANDING GEAR WAS SELECTED UP, THE A AND B DOWN PROXIMITY LIGHTS REMAINED LIT, AS WELL AS ALL A AND B DISAGREEMENT LIGHT. AFTER TWO MINUTES, GEAR CAME UP. CYCLED TWO TIMES WITH SAME RESULT. RETURNED TO SFO. REPLACED LANDING GEAR CONTROL BOX. CYCLED THE GEARS SEVERAL TIMES. OPS CHECKS GOOD. P/N ON: 120-38594-501, S/N: 120009R, S/N OFF: 120081R.
EMERGENCY LIGHTS FRONT LEFT FLOOR AND MIDDLE CEILING AND SIDE INOP. REPLACED FORWARD EMERGENCY LIGHT BATTERY, OPS CHECKS GOOD. S/N: 263, S/N OFF: 12741896.
EMERGENCY LIGHTING INOP FORWARD FLOOR AND LEFT WALL EXIT SIGN. AFTER PROPER CHARGING, OPS CHECK OF EMERGENCY LIGHTING SYSTEM PER MM 335000 CHECKS GOOD.
ON DESCENT, FLAP AND ADVANCED STALL WARNING LIGHTS ILLUMINATED ALONG WITH CONTROL FAULT LIGHT AND SECOND ROW OUTBOARD FLAP LIGHTS ON FLAP PANEL. FOLLOWED QRH AND RESET, WARNINGS CORRECTED. AFTER LANDING ALL WARNINGS RETURNED AND WOULD NOT RESET. CLEANED FLAP ACTUATOR, CANNON PLUG AND RERACKED FLAP CONTROL UNIT, OPS CHECK GOOD.

LEADING EDGE VERTICAL FIN LIGHT ILLUMINATED ON ICE PROTECTION PANEL. RETURNED TO PDX. INSTALLED SERVICEABLE VERTICAL FIN DEICE BOOT IAW 301000, OPS CHECK GOOD. (P/N 29S70513539) ON, (S/N ME27100) ON, (S/N ME314140) OFF.
AFTER TAKEOFF, SELECTED LANDING GEAR UP, ALL LIGHTS ON GEAR CONTROL PANEL ILLUMINATED. NO GEAR DOOR NOISE OR GEAR RETRACTION HEARD CYCLED LANDING GEAR HANDLE WITH THE SAME RESULTS. RETURNED TO LAX. REPLACED RIGHT GEAR UPLOCK A HARNESS. OPS CHECK GOOD.
CAPTAINS ALTIMETER FAILED CLIMBING THROUGH 13,100. OFF FLAG DISPLAYED. RETURNED TO SEA. REMOVED AND REPLACED CAPTAIN'S ALTIMETER IAW MM 34-12-03. CHECKS GOOD.
FLAP DISAGREEMENT WARNING WHEN FLAP SELECTED FOR LANDING APPROACH. COULD NOT DUPLICATE DISCREPANCY. OPERATIONAL CHECK OF FLAP SYSTEM IAW EMB M/M WAS SATISFACTORY.
GREEN HYDRAULIC SYSTEM LOW LEVEL WARNING INDICATION INFLIGHT. REPLACED THE RESERVOIR QUANTITY TRANSDUCER.
WHEN FLAPS SELECTED TO 15 DEGREES FLAP DISAGREEMENT WARNING ON. COULD NOT DUPLICATE DISCREPANCY. CLEANED FLAP TRACKS, OPERATION OF FLAPS NORMAL.
EMERGENCY FLOOR PATH LIGHTING, ROWS 1 THRU 5, IS INOP. REPLACED EMERGENCY LIGHT BATTERY.
AILERONS BINDING AND NOISY WHEN MOVED LT OR RT. REPLACED THE AUTO PILOT SERVO. PART TOTAL CYCLES, 20,917.
EMERGENCY EXIT LIGHT AT ROW 5C IS INOP. REPLACED THE EMERGENCY BATTERY PACK.
DURING AUTO PILOT TEST THE ELEVATOR TRIM RUNS WILD. AUTO PILOT SYSTEM PLACED ON DEFERRED MAINTENANCE LIST.
NR 2 FLIGHT IDLE STOP CB POPPED. REPLACED NR 2 FLIGHT IDLE STOP CIRCUIT BREAKER.
BOTH LT AND RT COOLING PACKS FAIL TO COOL PROPERLY. REPLACED THE LT AND RT PRESSURE REGULATING VALVES.
ANTI-SKID LIGHT ILLUMINATED WHEN LANDING GEAR RETRACTED. REPLACED THE ANTI-SKID CONTROL BOX.
COPILOTS STICK SHAKER ACTIVATED DURING TAKEOFF ROLL. REPLACED THE STALL WARNING COMPUTER.
FLIGHT IDLE STOP SOLENOID CIRCUIT BREAKER TRIPPED, WOULD NOT RESET. REPLACED THE FLIGHT IDLE SOLENOID.
CABIN WILL NOT HOLD PRESSURIZATION. ADJUSTED AND RESEALED PILOT'S DIRECT VISION WINDOW.

RT WING ROOT FAIRING UPPER ATTACH ANGLE HAS NUMEROUS BROKEN RIVET HEADS AND POSSIBLE CRACKS. REPAIRED FAIRING SUPPORT ANGLE.
THE RIGHT POWER LEVER FLIGHT IDLE STOP SOLENOID IS INOPERATIVE. REPLACED THE FLIGHT IDLE STOP SOLENOID.
HYDRAULIC FLUID LEAK AT VERTICAL STABILIZER. REPLACED THE RUDDER POWER CONTROL UNIT.
FLAP ASSYMETRY WARNING. RT OUTBD FLAP INDICATES 25 DEGREES WHEN SELECTED 15 DEGREES. REPLACED RT OUTBD FLAP ACTUATOR.
DURING INSPECTION OF UNDER FLOOR LAVATORY AREA, THE DOUBLER FOR THE OAT SENSOR WAS FOUND TO HAVE CORROSION. THE DOUBLER WAS REPLACED BY A FABRICATED DOUBLER AND INSTALLED IN ACCORDANCE WITH AERODESIGN AIRCRAFT ENGINEERING REPORT 1452-1.
INBOARD FLAPS WOULD NOT RETRACT AFTER TAKEOFF. REPLACED ROLLERS ON LT INBOARD FLAP.
