Maintenance disclosures
CREW ON FLT 145, DISCOVERED ON APPROACH THAT THE AIRPLANE LANDING GEAR WOULD NOT EXTEND. TOWER WAS NOTIFIED. INITIATED MANUAL GEAR EXTENSION PROCEDURES, WHICH WERE SUCCESSFUL. LANDED WITHOUT INCIDENT. MX FOUND THE 60 AMP LANDING GEAR CIRCUIT BREAKER IN LT NACELLE TRIPPED. THE GEAR WAS RETRACTED AND EXTENDED SEVERAL TIMES BUT THE DEFECT COULD NOT BE DUPLICATED. AN OPS CHECK FLIGHT WAS CONDUCTED SUCCESSFULY WITH NO REOCCURRANCE OF THE LANDING GEAR ISSUE. RELEASED BACK TO SERVICE.
ON DEPARTURE, PILOT EXPERIENCED A FUEL IMBALANCE THAT REQUIRED FULL AILERON TRIM TO CORRECT. ACFT RETURNED TO BASE, MX DETERMINED 1 FUEL QUANTITY INDICATOR WAS INDICATING INCORRECT FUEL QUANTITY. THE FUEL TANKS WERE SUMPED AND THE FAULTY GUAGE THEN INDICATED CORRECTLY. THE FUEL TANKS WERE TOPPED OFF AND THE GUAGES AGAIN CHECKED FOR PROPER INDICATION. THE ACFT WAS THEN RETURNED TO SERVICE.
ON CLIMBOUT, PASSING THROUGH 500 FT, THE OUTER PLY OF THE PILOT'S WINDSHIELD CRACKED FROM TOP TO BOTTOM. LANDED WITHOUT INCIDENT. THE WINDSHIELD WAS REPLACED BY MX.
ON TAKEOFF ROLL, THE LANDING GEAR "IN TRANSIT" LIGHT ILLUMINATED. THE CREW ABORTED THE TAKEOFF AND TAXIED BACK TO THE HANGAR. THE LIGHT REMAINED ILLUMINATED UNTIL THE LT ENGINE WAS SHUTDOWN. MX DISCOVERED THAT A WIRE HAD COME LOOSE FROM A LANDING GEAR WARNING CIRCUIT BREAKER. THOUGH THE CIRCUIT BREAKER APPEARED TO BE FUNCTIONAL IT WAS REPLACED AS A PRECAUTIONARY MEASURE. THE LANDING GEAR WARNING SYS CHECKED GOOD THROUGH SUBSEQUENT GEAR SWINGS AND AN OPS CHECK FLIGHT.
PILOT REPORTED HIS ENCODING ALTIMETER WAS INOPERATIVE ON TAKEOFF. THIS SITUATION ALSO RENDERED THE ALTITUDE ALERTER AND TRANSPONDER MODE C INOPERATIVE. EXCESS FUEL WAS BURNED OFF AND THE ACFT RETURNED TO BASE. MX REPLACED THE PILOT'S ENCODING ALTIMETER, PERFORMED A SYS LEAK CHECK, AND RECERTIFIED THE TRANSPONDER SYS. ACFT WAS THEN RELEASED FOR SERVICE.
2 MINUTES OUT OF LANDING,THE PILOT REPORTED (ON LANDING) THAT HE HAD TAKEN A LIGHTNING STRIKE TO THE NOSE OF THE ACFT. INSP OF THE ACFT REVEALED THAT THE TIP OF THE RT ELEVATOR HAD "BLOWN UP". A MECHANIC HAS BEEN DISPATCHED TO THE SCENE WITH AN ELEVATOR AND WILL FURTHER INVESTIGATE FOR MORE DAMAGE.
ON LANDING, LT BRAKES LOCKED UP. PILOT STATED THAT AFTER LANDING THE LT BRAKES ACTIVATED WITHOUT INPUT FROM EITHER CREW MEMBER. THE DEGREE OF BRAKING ACTION WAS SLIGHT INITIALLY BUT BUILT AS TAXI PROGRESSED TO THE POINT THAT THE ACFT COULD NOT BE TAXIED AFTER DEPARTING THE RUNWAY. MX WAS DISPATCHED TO THE SCENE AND FOUND THE BRAKES TO BE FREE AND OPERATING NORMALLY. ACFT WAS THEN TOWED TO THE HANGAR WITHOUT FURTHER INCIDENT. MX WAS UNABLE TO FIND ANY ISSUES WITH THE ACFT WHEELS/BRAKE SYS AND WAS UNABLE TO DUPLICATE THE PROBLEM. A CHECK INTO THE ACFT HISTORY REVEALED THAT 5 DAYS PRIOR THERE WAS AN OCCURANCE ON THE RAMP IN WHICH A PILOT REPORTED "LITTLE" BRAKING ACTION ON THE LT SIDE. AT THAT TIME THE BRAKE SYSTEM WAS BLED AND OPERATED NORMALLY. FURTHER OPS CHECKS PRODUCED INCONSISTENT BRAKING ACTION FROM THE PILOTS LT PEDAL. THE LT BRAKE MASTER CYLINDER WAS CHANGED AND THE SYSTEM OPERATIONALLY CHECKED NORMAL.
