PILOT CLAIMED THAT IN LEVEL FLIGHT THE CYCLIC FLIGHT CONTROL WOULD NOT MOVE MORE THAN AN INCH IN ANY DIRECTION. TRIED SYSTEM 2 AND HAD THE SAME BLOCKAGE AND WENT BACK TO SYSTEM 1. NOTED TO ME THAT THE COLLECTIVE AND YAW WAS ACTING NORMAL ONLY THE CYCLIC HAD BLOCKAGE. DID NOT WANT TO BREAK PASS THE STOPPAGE IN CASE HE COULDN’T BRING IT BACK. ALL OTHER FLIGHT OPERATION ACTIONS WERE NORMAL. AFTER HARD LANDING THE SKIDS WERE SPREAD AND WHEN THE AIRCRAFT CAME TO STOP MAIN ROTOR SYSTEM CONTACTED THE UPPER WIRE STRIKE. AIRCRAFT SHUT DOWN WAS NORMAL.

MADE THE FIRST LEG FROM BASE TO HOSPITAL, LANDED FINE, KEPT BOTH ENGINE RUNNING AT IDLE, PATIENT LOADED, READY TO GO, HOVER CHECK WITH BOTH ENG NR 1 & NR 2 TORQUE NEEDLE MATCH AT 60 PERCENT, MAX PERFORMANCE TAKEOFF TO ABOUT 150FT OGE, ENG NR2 TORQUE GAUGE DROPPED TO 0 AND ENG NR1 TOT 800C, GOT IN CRUISE, ENGINE ANALYSED, TORQUE NEEDLE NR2 FLUCTUATING, NO OTHER INDICATIONS, NP ENG 1 & 2 AT 101 PERCENT, NO WARNING LIGHTS, NO TONES. ALL FINE. CONTINUE IN CRUISE 2500FT WITH 40 PERCENT TORQUE BUT TOT ENG NR1 UP AT 700-750C. TORQUE NEEDLE ENG NR2 CONTINUE FLUCTUATING THE ENTIRE FLIGHT FROM 0 TO 40-60 PERCENT TORQUE, CONTINUED TO THE AIRPORT/PROGRAM BASE LOCATION AND MADE A SHALLOW APPROACH WITH NORMAL LANDING IN A HOVER. MED CREWS SWAPPED THE PATIENT OVER TO OTHER HELICOPTER AND OFF THEY WENT TO THE HOSPITAL. ALL WORKED OUT WELL. MECHANIC ON SITE AND DECISION MADE TO REMOVE ACFT FROM SERVICE FOR FURTHER INSPECTION AND TROUBLESHOOTING.

PILOT REPORTED A TAIL ROTOR CHIP LIGHT FLICKER AND THE LIGHT WENT ON SOLID AT LANDING. PILOT CALLED THE MECHANIC WHO INSPECTED THE CHIP DETECTOR AND FOUND A CHIP. MECHANIC THEN DRAINED THE GEAR BOX, SERVICED AND DID A 30 MINUTE GROUND RUN. INSPECTED THE CHIP DETECTOR AND FOUND METAL PARTICLES. THE GEARBOX WAS THEN DRAINED AGAIN AND METAL PARTICLES WERE FOUND IN THE OIL. THE ACFT IS OUT OF SERVICE AND AN O/H GEARBOX AND TOOLS ARE BEING SENT TO REPLACE THIS TAIL ROTOR GEARBOX.

FOUND HARDWARE INSTALLED INCORRECTLY CAUSING: WEAR IN THE HYD PACK AND MOUNT HOLES. CRACKS IN THE CABIN FLOOR AND CROSS STRUCTURE SUPPORT IN THE CABIN CEILING UNDER THE HYD PACK. AFT HYD MOUNT ON DECK HAS WEAR IN THE BUSHING HOLES 0.100" THICK, FITTING DAMAGE IN 0.025" DEEP.

PILOT FELT BINDING IN TAIL ROTOR CONTROLS IN HOVER, MX IS TROUBLESHOOTING PROBLEM. REPLACED CSAS COMPUTER AND CSAS ACTUATOR PROBLEM WAS GONE FOR ABOUT 12 FLIGHT HOURS. PROBLEM RETURNED. THIS TIME WE FOUND THE SLIDING SLEEVE TO BE BINDING ON THE TAIL ROTOR SHAFT WHEN RADIAL FORCE WAS APPLIED TO THE SLIDING SLEEVE.

THIS WAS FILLED OUT AND FAXED 9/9/2003.
