NR 2 ENGINE FIRE LIGHT ILLUMINATED INTERMITTENTLY.
WHILE INSPECTING THE TAIL BOOM, FOUND THE CASTING FOR THE TAIL ROTOR GEARBOX CRACKED. AWAITNG ACFT MFG TO COME AND REPAIR.
ON POST FLIGHT, FOUND CRACK ON L/E OF RT VERTICAL STABILIZER THAT EXTENDED AROUND TO BOTH INBD AND OTBD SIDES.
DURING RUN-UP, BOTH ENGINES STARTED & GENERATORS PLACED IN "ON" POSITION FOLLOWING NORMAL START. WHILE PILOT WAS PLACING SYS & SWITCHES TO THE CHECKLIST, PARAMEDIC IN LT SEAT ALERTED HIM TO SMOKE. LOOKED FOR SIGNS OF SMOKE, VERIFIED NO FIRE WARNING LIGHTS, & LACK OF SYS INDICATIONS OUT OF LIMITS WHILE SIMULTANEOUSLY EXECUTING EMERGENCY SHUTDOWN PROCEDURES. DURING SHUTDOWN, PILOT NOTED A 30 PERCENT NG & MGT OF 0. SHUTDOWN OF REMAINING ENGINE. CONDUCTED VISUAL INSPECTION OF THE COWLING AREA, EXHAUST AREA, AND PROCEEDED BOTH ENGINE BAYS. NOTED NOTHING ABNORMAL ON INSPECTION. FOLLOWING INSPECTION AND GROUND RUNS BY MX, CAUSE REMAINED IN QUESTION. DUE TO LACK OF CAUSE, LACK OF ANY INDICATIONS OF EXCEEDENCES, AND NORMAL GROUND RUNS, AIRCRAFT RETURNED TO SERVICE.
ACFT WAS ENROUTE FOR AN INTERFACILITY MISSION. SHORTLY AFTER LIFTING THE SHADIN UNIT BEGAN FLASHING L2 EXCEEDED. A CHECK OF ALL GAUGES INDICATED NORMAL. THEN THE COCKPIT LIGHTING AND INSTRUMENTS STARTED TO FLICKER AS IF A LOSS OF PWR. A THICK SMELL OF SMOKE WAS NOTED BY ENTIRE CREW OF AN ELECTRICAL NATURE. THE SHADIN UNIT THEN BEGAN SHOWING ROM FAILURE. ALL CREW AGREED THAT AN IMMEDIATE PRECAUTIONARY LANDING WAS PRUDENT. TWR WAS NOTIFIED ALONG WITH HCA OPERATIONS. THE ACFT WAS SAFELY PLACED IN A FIELD. PIP WAS INITIATED BY HCA OPERATIONS. CREW DEBRIEFING WAS MADE. ARRIVED AT LOCATION AND TALKED TO PILOT WHO INFORMED ME OF THE CODES RECEIVED ON THE SHADIN ETM REMOTE PROCESSOR. REMOVED THE ETM REMOTE PROCESSOR FROM THE CTR CONSOLE AND IMMEDIATELY SMELLED THE STRONG ODOR OF AN ELECTRICAL SHORT COMING FROM THE BACK OF THE ETM REMOTE PROCESSOR. HAD THE PILOT SMELL THE BACK OF THE ETM REMOTE PROCESSOR AS WELL AND HE ALSO VERIFIED THAT WAS THE SAME SMELL THAT CAUSED HIM TO MAKE A PRECAUTIONARY LANDING. FURTHER CHECK BEHIND THE CTR CONSOLE FOR POSSIBLE ELECTRICAL PROBLEMS AND COULD NOT FIND ANY. ONCE TALKING WITH THE FMS THE ETM REMOTE PROCESSOR WAS PLACARD INOP AND SIGNED OFF FOR CONTINOUS OPERATIONS IAW MEL CH. 31, ITEM 4, CATEGORY C. AS OF TODAYâS DATE 04-20-09 THE ITEM HAS BEEN MEL'D.
DURING SCHEDULED INSPECTION A 18MM LONG CRACK WAS DISCOVERED ON THE LT 'A' FUSELAGE FRAME 2L. THE CRACK IS ON THE LOWER SKIN ONLY OF A DOUBLE SKIN ASSEMBLY. THE CRACK ORIGINATES FROM A BOLT HOLE, EXTENDS 9MM IN EACH DIRECTION FROM THE BOLT HOLE. THE BOLT IS USED FOR THE LOWER FORWARD ATTACHING SUPPORT FOR THE ECU (ENVIRONMENTAL CONTROL UNIT)
SIGNAL GENERATOR, INTERMITTENTLY CAUSES EFIS TO GO TO (TEST IN PROGRESS)-THEN ERROR OF SIGNAL ON EFIS T, REMOVED AND REPLACED.
ATTITUDE GYRO, TUMBLING, REMOVED AND REPLACED.
ATTITUDE GYRO, INTERNAL LIGHTING, INOPERATIVE. REMOVED AND REPLACED.
ENCO ALTIMETER, CODE OFF, FLAG WILL NOT PULL. REMOVED AND REPLACED.
ALTIMETER OUT OF CALIBRATION. DOES NOT AGREE WITH COPILOT''S ALTIMETER OR TEST BOX. OFF AT 3,000 FEET. REMOVED AND REPLACED. (X)
