Maintenance disclosures
THE LH AND RH HYDRAULIC TEMP SENSORS GIVE FALSE INDICATIONS. WE HAVE REPLACED 5 SENSORS IN THE LAST FEW YEARS AND THE LATEST ONE THAT IS BRAND NEW FAILED AFTER TWO WEEKS IN SERVICE.
ON TAKEOFF THE CREW SAW THE MAIN CABIN DOOR CAS MESSAGE ILLUMINATE AS WELL AS THE CABIN VENT DOOR CAS MESSAGE. BOTH CAME ON AT 11,000 FEET WHILE DEPARTING ASPEN. THEY RETURNED TO ASPEN AND LANDED WITHOUT INCIDENT. MAINTENANCE FOUND THE INTERIOR CABIN DOOR HANDLE LOOSE. IT WAS TIGHTENED AND OPS CHECK GOOD.
DURING FLIGHT CONTROL CHECKS, PRIOR TO FLIGHT, IT WAS DISCOVERED THAT THE UPPER RUDDER CHANNEL B FAILED THE PRE-FLIGHT CHECKS. THE UPPER RUDDER POSITION WOULD STAY FULLY DEFLECTED UNTIL RESET. THE MAINTENANCE FOUND THE CHANNEL B RUDDER POSITION SENSOR DEFECTIVE. THE SENSOR WAS REPLACED AND OPS AND TRAVEL CHECKS WERE NORMAL.
DURING POST FLIGHT INSPECTIONS, THE RT THRUST REVERSER PRESSURE HYDRAULIC LINE WAS FOUND TO BE CHAFED BY NEARBY B-NUT CAUSING A HYDRAULIC LEAK. . THE LOCATION OF THE CHAFED HYDRAULIC LINE WAS WITHIN THE AFT MAINTENANCE BAY ON THE RT SIDE OF THE COMPARTMENT JUST ABOVE AND AFT OF THE WATER SEPARATOR. THE B-NUT WAS LOCATED AFT OF A CHECK-VALVE THE CHAFED HYDRAULIC LINE ROUTES VERTICALLY AFT OF THE WATER SEPARATOR. THE AREA CHAFED IS NOT READILY SEEN AND COULD BE MISSED DURING VISUAL INSPECTION. THE HYD LINE WAS REPLACED AND ADJUSTMENTS WERE MADE TO ACHIEVE CLEARANCE.
WHILE WAITING FOR PASSENGERS WITH THE APU RUNNING AND THE AIR CONDITIONING OPERATING, THE CREW REPORTED THAT VISIBLE SMOKE WAS SEEN IN THE COCKPIT WITH THE ODORS OF SOMETHING GETTING HOT, WERE UNABLE TO CONTROL THE COCKPIT TEMP. THE CREW SHUTOFF THE COCKPIT AIR CONDITIONING PACK AND THE ODOR AND SMOKE DISSIPATED. TROUBLESHOOTING DETERMINED THAT THE COCKPIT AIR CONDITIONING PACK OIL SEALS FAILED ALLOWING THE AC PACK OIL TO BYPASS. THE PACK WAS REPLACED WITH A NEW UNIT. OPS CHECKED SATISFACTORY. THE AIRCRAFT WAS RETURNED TO SERVICE.
