DURING INSPECTION A .500" CRACK WAS FOUND IN THE STRINGER.
AFTER TAKEOFF, NOTICED MAIN GEARBOX OIL PRESSURE DROPPING BUT INITIALLY STAYING IN ACCEPTABLE RANGE WITH NO SIGNIFICANT INCREASE IN TEMPERATURE. FLIGHT CONTINUED FOR A FEW MINUTES AND THEN PRESSURE DROPPED TO WARNING INDICATION LEVEL. EXECUTED AN UNSHEDULED LANDING WITHOUT INCIDENT. FOUND A LOSE OIL LINE AT THE TRNSMISSION OIL COOLER ASSY, INSPECTED LINE FOR DAMAGE, NONE FOUND. SECURED, TORQUED THE OIL LINE, SERVICED THE TRANSMISSION, PERFORMED A SERVICABILITY CHECK, GROUND RUN FOR 1 HOUR IAW MM WITH NO ABNORMAL INDICATIONS AND RETURNED THE AIRCRAFT TO SERVICE.
ON REMOVAL OF LOWER FUEL CELL INSPECTION PANELS, DISCOVERED THE LOWER RT FUEL CELL WAS LEAKING FUEL AT THE FWD AND AFT FUEL PROBE PANELS. FUEL PROBE PANELS ARE DELAMINATING FROM THE MAIN FUEL CELL.
AFTER TAKE OFF DURING CLIMB OUT, THE NR 2 ENGINE OIL TEMP. ROSE TO 120 DEGREES. AND A HIGH OIL TEMP CAUTION LIGHT ILLUMINATED. PILOT STARTED DESCENT AND AFTER LANDING SHUT ENGINE DOWN AND TAXIED TO PARKING LOCATION WITH NO FURTHER DIFFICULTY.
AFTER DEPARTURE, ON A TRAINING FLIGHT, RAISED THE GEAR AND GOT A RED GEAR UNSAFE INDICATION, ESTABLISHED A STEADY CRUISE AT 80KIAS AND CHECKED INDICATIONS AND HYDRAULIC SYSTEM PRESSURE. NORM UTILITY PRESSURE INDICATED RED AT 474 PSI NOT CHARGING. ALL OTHER HYDRAULIC PRESSURES WERE IN THE GREEN. AFTER CYCLING THE GEAR A FEW TIMES WITH NO CHANGE IN PRESSURES OR GEAR INDICATION, CONSULTED THE EMERGENCY CHECKLIST AND EXECUTED THE EMERGENCY LOWER GEAR PROCEDURE. UPON ACTIVATION OF THE EMERGENCY DOWN SYSTEM I GOT A 3 GREEN INDICATIONS. HEADED BACK TO MHV AND HAD AN UNEVENTFUL LANDING. NOTICED THAT THE AFTER BLOWING DOWN THE GEAR, THE UTILITY SYSTEM STARTED CHARGING AGAIN AND THE NORM UTILITY PRESSURE WAS IN THE GREEN AT 1350 PSI AT LANDING.
PILOT INDICATED HIGH OIL TEMP RED LIGHT CAUTION IN CRUISE FLIGHT, OTHER INDICATIONS IN THE GREEN. PERFORMED PRECAUTIONARY LANDING. PILOT AND CREW INDICATED 3 GREEN SAFE LANDING GEAR INDICATION. NOSE LANDING GEAR COLLAPSED WHILE TOUCHING DOWN. RT MAIN GEAR PARTIALLY EXTENDED. NO ATTEMPT MADE TO UTILIZE EMERGENCY DOWN CONTROL FOR MAIN LANDING GEAR.
IN FLIGHT CREW REPORTED AN ODOR OF FUEL THROUGHOUT THE CABIN. AIRCRAFT WAS INSPECTED AND FOUND THAT THE LT FORWARD FUEL CELL HAD A SMALL PIN HOLE SIZE LEAK AROUND THE FORWARD FUEL QUANTITY PROBE MOUNT FLANGE RUBBER SEAM. THE RT FORWARD FUEL CELL APPEARS DRY HOWEVER IT HAS A LINE ACROSS THE AFT PORTION THAT COULD HAVE BEEN CAUSED BY SEAPAGE THROUGH THE RUBBERIZED FABRIC AT SOME POINT AND IS SUSPECT TO FUTURE FAILURE. THE LT FUEL CELL HAD BEEN RECENTLY REPLACED.
APPROXIMATELY 15 MINUTES INTO FLIGHT CREW REPORTED AN ODOR OF FUEL THROUGHOUT THE CABIN. AIRCRAFT WAS RETURNED TO BASE.
DISCOVERED DURING A SCHEDULED MAINTENANCE EVENT THE FOLLOWING DISCREPANCY. WHILE PERFORMING BULLETIN 109EP-1222 & AMC EO-06-6322-EO-452, 200 HOUR INSPECTION OF RIGHT SIDE OIL COOLER BLOWER FAN IMPELLER PLAY MEASUREMENT, HE FOUND THE PULLEY TO BE SLIPPING ON IMPELLER SHAFT. UPON FURTHER DISASSEMBLY, THERE WAS REVEALED A SHEARED SHAFT WHERE THE PULLEY CONNECTS TO THE IMPELLER SHAFT. THIS AIRCRAFT REMAINS OUT OF SERVICE AT THE TIME OF THIS SUBMISSION AWAITING PARTS.
DURING C-CHECK INSPECTION, NR 1 FRAME ON LEFT AND RIGHT ENGINE NACELLES WERE FOUND TO HAVE CRACKS INBOARD AND OUTBOARD OF MOUNT HOLES FOR LEFT AND RIGHT SIDE OF ENGINE FORWARD TORQUE MOUNT. REMOVED AND REPLACED NR 1 FRAME ON LEFT AND RIGHT ENGINE NACELLES. (M)

SUBMITTER STATED SEEMED TO BE A REPETITIVE DISCREPANCY AS PREVIOUSLY DETAILED AND REPORTED AND POSSIBLE CAUSE IS STILL UNDETERMINED. WOULD SUSPECT ENGINE TORQUE AND VIBRATION AS POSSIBLE CONTRIBUTING FACTORS. WOULD SUGGEST TIMED INTERVALS FOR REPETITIVE INSPECTION OF AREA.
LEFT VERTICAL STABILIZER CHINE ANGLE IS CRACKED .250 INCH BY .010 AND HAS WORKING RIVETS. SUBMITTER SUGGESTED CAUSE AS FATIGUE/VIBRATION AND RECOMMENDED A DETAILED INSPECTION BE CALLED OUT DURING C-CHECK INSPECTION.

RIGHT VERTICAL STABILIZER CHINE ANGLE IS CRACKED .250 INCH BY .010 INCH AND HAS WORKING RIVETS. SUBMITTER SUGGESTED POSSIBLE CAUSE AS FATIGUE/VIBRATION AND RECOMMENDED A DETAILED INSPECTION BE CALLED OUT DURING C-CHECK INSPECTION.
DURING C-CHECK INSPECTION, CORROSION WAS DISCOVERED BETWEEN FUSELAGE FRAMES 13A AND 15A, FS 4214.0 TO FS 4992.0 ON LOWER PASSENGER/CREW ENTRANCE DOOR SURROUNDING STRUCTURE ANGLE. SUBMITTER SUGGESTED POSSIBLE CAUSE AS MOISTURE PENETRATION.
FLT 7268 - ROA-ATL - DURING CRUISE FLIGHT AT 20,000 FEET, CABIN PRESSURIZATION STARTED TO CLIMB. CREW ELECTED TO MAKE AN EMERGENCY DESCENT TO 10,000 FEET AND CONTINUE TO ATL, WHERE LANDING WAS MADE SAFELY AND WITHOUT INCIDENT. REMOVED AND REPLACED CARGO DOOR SEAL. OPERATIONAL AND LEAK CHECKS WERE GOOD AND AIRCRAFT WAS RELEASED FOR FURTHER SERVICE. (M)
LEVELING OFF AT 2000 FEET DEPARTING DFW, LEFT ENGINE SURGED SLIGHTLY AND SHUTDOWN COMPLETELY WITH AUTOFEATHER. NO LIMITATIONS WERE EXCEEDED. AN EMERGENCY WAS DECLARED AND THE AIRCRAFT RETURNED TO DFW WHERE A LANDING WAS MADE WITHOUT INCIDENT. UPON INSPECTION FOUND P3 LINE BROKEN AT LEFT ENGINE OVERSPEED GOVERNOR. REPLACED P3 LINE AT HMU TO OVERSPEED GOVERNOR. OPS CHECKS GOOD ON GROUND RUN-UP. AIRCRAFT RELEASED FOR SERVICE. (M)

STL - FLT 7452 - TRIM WARNING SOUNDED ON TAKEOFF. MAINTENANCE RESET AURAL WARNING CIRCUIT BREAKER. SYSTEM CHECK GOOD. (X)
MSN - FLT 7171 - RT MAIN INTRANSIT LIGHTS ON BOTH CHANNELS WOULD NOT EXTINGUISH WHEN GEAR WAS SELECTED UP. MAINTENANCE CLEANED UPLOCK SENSOR BOX AND ADJUSTED RT MAIN GEAR UPLOCK SENSOR STRIKER ARM. GEAR SWINGS REVEALED NORMAL. (X)
