THE CREW REPORTED SMOKE IN THE CABIN WITH A PACK 2 OVERHEAT. THE ACFT LANDED WITHOUT INCIDENT WHERE MAINTENANCE INSPECTED THE ACFT, R & R THE NR 1 ACM, AND BOTH Y-DUCTS ON THE LT AND RT PACKS. OPS CHECKS WERE PERFORMED WITHOUT DEFECT, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
PILOT REPORTED A PACK 2 OVERHEAT AND PACK 2 VALVE FAIL MESSAGE ON EICAS. PACK 2 OVERHEAT QRH SAID TO SHUTOFF PACK FOR 3 MINUTES. DURING 3 MINUTE PACK SHUTDOWN, SMOKE ENTERED COCKPIT FROM DV WINDOW VENTS. CABIN SMOKE IAC & QRH COMPLIED WITH. WHEN RECIRCULATING FAN TURNED OFF, SMOKE CLEARED. RESET PACK 2. ACFT LANDED WITHOUT INCIDENT. AFTER LANDING, PACK 1 OVERHEAT ON EICAS ONLY HOT AIR WAS DISTRIBUTED. MAINTENANCE PERFORMED OPS CHECK OF PACK 2, NO FAULTS FOUND AND NR 1 ACM PLACED ON MEL. MAINTENANCE R & R NR 1 ACM WITH OPS CHECK GOOD. ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING ROUTINE MAINTENANCE THE FORWARD EXIT ROW EMERGENCY SIGN WOULD NOT TEST. MAINTENANCE REPLACED THE NR 6 EMERGENCY LIGHT INVERTER. OPS CHECKS WERE GOOD.
THE CREW REPORTED UPON ACTIVATION OF NR 1 PACK, OBSERVED SMOKE AND A BURNING ODOR IN THE FLIGHT DECK. CREW REPORTED HEARING ABNORMALLY LOUD AND UNUSUAL NOISES AND PASSENGER IN SEAT 1A REPORTED SMOKE. MAINTENANCE INSPECTED AND REPLACED THE NR 1 AIR CYCLE MACHINE. OPS CHECKS WERE GOOD.
DURING C-CHECK, RIVETS AT CIRCUMFERENTIAL SPLICE SHOW SIGNS OF CORROSION FRAME 13. INSTALLED FILLER AND 2 DOUBLERS AT FRAME 13 SPLICE IAW SRM 53-00-02. ACFT APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK, BAGGAGE DOOR CUTOUT LOWER BEAM HAS CORROSION, FR 65-68. R & R BAGGAGE DOOR CUTOUT LOWER BEAM IAW SRM 51-40-02. ACFT APPROVED FOR RETURN TO SERVICE.
DURING MAINTENANCE INSPECTION, THE APU FIREWALL WAS FOUND TO BE CRACKED AT THE RT BOTTOM FRAME ATTACHMENT POINT. MAINTENANCE REMOVED THE DAMAGED SECTION OF FIREWALL AND PERFORMED A FILLER/DOUBLER REPAIR IAW SRM 53-32-12, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING MAINTENANCE INSPECTION, THE APU FIREWALL WAS FOUND TO BE CRACKED AT THE LT BOTTOM FRAME ATTACHMENT POINT. MAINTENANCE REMOVED THE DAMAGED SECTION OF FIREWALL AND PERFORMED A FILLER/DOUBLER REPAIR IAW SRM 53-32-12, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED A SMOKE INDICATION FROM THE CARGO BAY, THE QRH WAS FOLLOWED AND THE CARGO FIRE BOTTLES WERE DISCHARGED. THE ACFT LANDED WITHOUT INCIDENT WHERE MAINTENANCE INSPECTED THE ACFT, FOUND NO EVIDENCE OF SMOKE OR FIRE, OPERATIONALLY TESTED THE CARGO SMOKE DETECTION SYSTEM, REPLACED THE CARGO FIRE BOTTLES, PERFORMED ENGINE RUNS ALL WITH NO DEFECTS NOTED AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
PILOT REPORTED THE F/O PFD CIRCUIT BREAKER TRIPPED IN FLIGHT WITH AN ELECTRICAL BURNING ODOR NOTICED. MAINTENANCE R & R THE NR 2 PFD WITH OPS CHECK GOOD. ACFT WAS APPROVED FOR RETURN TO SERVICE.
CREW REPORTED A BURNING ODOR DURING DECENT AND AGAIN DURING LANDING. MX PERFORMED AN OPS CHECK OF THE AIR CONDITIONING PACKS AND COULD NOT DUPILCATE ODOR. ACFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED FAINT OIL BURNING ODOR FROM NR 2 PACK DURING CLIMB AND DESCENT. BLEED SOURCE WAS NR 2 ENGINE. OPEN
THE CREW REPORTED A MUSTY ODOR WHEN EITHER BLEED ENGINE NR 1 OR NR 2 SELECTED BUT GOES AWAY WHEN APU IS SELECTED. MAINTENANCE R & R THE NR 1 ENGINE, OPS TESTED WITH NO DEFECTS NOTED, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK, FRAME 10 WAS FOUND CRACKED UNDER F/O WINDOW. MAINTENANCE R & R THE CRACKED FRAME IAW SRM 51-40-02. ALL WORK ACCOMPLISHED AND WILL BE RELEASED AT THE END OF THE CHECK.
DURING C-CHECK. APU GROUNDING SHUNT BAR WAS FOUND CORRODED AT FRAME 80. MAINTENANCE FABRICATED AND INSTALLED A REPAIR DOUBLER AT FRAME 80 FOR THE GROUNDING BAR MOUNT IAW EC 5310-01544, REV C. ALL WORK ACCOMPLISHED AND WILL BE RELEASED AT THE END OF THE CHECK.
DURING C-CHECK, A CRACK WAS FOUND AT FRAME 20, S3L IN THE COASTAL REINFORCEMENT. MAINTENANCE R & R COASTAL REINFORCEMENT IAW SRM 51-40-02.
DURING C CHECK, FOUND RT OMEGA BEAM GUSSET AT FRAME 61 WAS CORRODED OUT OF LIMITS. MAINTENANCE REMOVED AND INSTALLED A NEW OMEGA BEAM IAW SRM 51-40-02.
DURING C CHECK, CORROSION WAS FOUND AROUND NUTPLATE HOLES IN SERVICE DOOR THRESHOLD BETWEEN FRAMES 20 - 22. MAINTENANCE R & R THE CORRODED THRESHOLD IAW SRM 51-40-02.
DURING C-CHECK, MULTIPLE AREAS OF CORROSION FOUND ON THE INTERIOR BELLY SKIN AT FRAMES 17-19 AND STRINGERS 21L-22R. MAINTENANCE CUTOUT THE DAMAGE AREA, FABRICATED AND INSTALL A FILLER,SHIM AND A REPAIR DOUBLER IAW EC 5320-01890. ALL WORK ACCOMPLISHED AND WILL BE RELEASED AT THE END OF THE CHECK.
DURING SCHEDULED MX INSPECTION THE LT OVERWING EMERGENCY EXIT LIGHT DID NOT ILLUMINATE. MX R & R INVERTER, OPS CHECKED SUCCESSFUL. ACFT WAS APPROVED FOR RETURN TO SERVICE.
EMERGENCY LIGHTS OVER THE EXIT ROWS WOULD NOT LIGHT, DURING TEST. MX INSPECTED AND PERFORMED OPS CHECKS OF THE EMERGENCY LIGHTS. NO DEFECTS WERE FOUND. OPS CHECKED GOOD.
PILOT REPORTED THE EMERGENCY EXIT LIGHT IN THE CEILING FACING THE FLIGHT DECK WAS DISCOVERED INOPERATIVE. MX PERFORMED AN OPS CHECK OF THE CABIN EMERGENCY LIGHTS SUCCESSFUL. ACFT WAS APPROVED FOR RETURN TO SERVICE.

CLE FLT 2611 AIR RETURN CREW REPORTED AIRCRAFT WOULD NOT PRESSURIZE AFTER TAKEOFF. AIRCRAFT RETURNED AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND REPLACED PRESSURIZATION DIGITAL CONTROLLER. OPERATIONAL CHECKS WERE GOOD.
FLIGHT 2217 - EWR - FLIGHT CREW REPORTED ELECTRICAL SMELL AT ROWS 1-4. EWR MX RAN ELECTRICAL SYSTEMS BUT COULD NOT DUPLICATE ELECTRICAL SMELL. ACFT WAS APPROVED TO RETURN TO SERVICE.
IAH - FLIGHT 2433, ON TAKEOFF THE CREW REPORTED THE STANDBY ALTIMETER TO BE STICKING. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE STANDBY ALTIMETER, OPERATIONALLY TESTED WITH NO DEFECTS. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE INSPECTED THE AIRCRAFT, PERFORMED GROUND RUNS, PRESSURIZED THE AIRCRAFT, ACTIVATED MULTIPLE SYSTEMS ALL WITH NO DEFECTS NOTED, AND UNABLE TO DUPLICATE THE DISCREPANCY. THE AIRCRAFT WAS SUBSEQUENTLY APPROVED FOR RETURN TO SERVICE.
CLE - FLIGHT 2775 - THE CREW REPORTED A LANDING GEAR LEVER DISAGREE MESSAGE ON THE EICAS AFTER SELECTING GEAR DOWN. THE MESSAGE CLEARED IMMEDIATELY AND NO OTHER INDICATIONS WERE NOTICED. MAINTENANCE INSPECTED AND REPLACED THE RIGHT NOSE GEAR DOOR ACTUATOR. OPERATIONAL CHECKS WERE GOOD.
ICT - FLIGHT 2725, THE CREW REPORTED AN ELEC EMERGENCY ABNORMAL MESSAGE ON EICAS DURING CLIMB. THE CREW PERFORMED AN EMERGENCY DESCENT, AND LANDED AT ICT WITHOUT INCIDENT. THE CREW RESET THE AIRCRAFT POWER AND VERIFIED NORMAL OPERATIONS, MX VERIFIED THAT LANDING WAS LESS THAN 6 FOS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
FLT 2359 - EWR - CREW REPORTED ELECTRICAL EMERGENCY ABNORMAL AND EMERGENCY LIGHT NOT ARMED INDICATIONS CAME ON TAXI. EWR MX REMOVED AND REPLACED RELAY K0097 AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
MX DURING AC ROUTINE MAINTENANCE, MX FOUND LT FWD EMERGENCY LIGHT INOP. MX REMOVED AND REPLACED FWD EMERGENCY LIGHT. OPERATIONAL CHECK WAS PERFORMED SUCCESSFULLY. AC WAS APPROVED TO RETURN TO SERVICE.
IAH - FLIGHT 2511, DURING CRUISE, THE CREW REPORTED THE MAIN PITCH TRIM TO BE INOPERATIVE. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE HORIZONTAL STABILIZER ACTUATOR, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

DURING C-CHECK ,THE LAV WASTE DRAIN BASE WAS FOUND TO BE CORRODED. MAINTENANCE REMOVED AND REPLACED THE WASTE DRAIN BASE IAW EMB 145 SRM 51-40-02. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV- DURING C-CHECK THE RIGHT HAND ENGINE INTAKE WAS FOUND TO BE GOUGED AT THE UPPER COWLING ALIGNMENT HOLE. MAINTENANCE REMOVED THE DAMAGED AREA AND PERFORMED A DOUBLER REPAIR IAW EMBRAER IC 2004-145/1466. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

ACFT N12922, FLT NR 2962 (BTV - CLE) DURING CLIMB-OUT THROUGH 18,000 FT, THE NR 2 ENGINE SHUTDOWN UNCOMMANDED. THE CREW WAS UNABLE TO RESTART. ACFT DIVERTED TO SYR AND LANDED SAFETY. MECHANICS WERE DISPATCHED TO ACFT. INITIAL FINDINGS INDICATE METAL ON THE OIL TANK MAG PLUG, WITH NO EVIDENCE OF EXTERNAL DAMAGE OR LEAKAGE. UPDATE 8/27/03 - THE ENGINE WAS INTERNALLY INSPECTED AND DAMAGE WAS FOUND ON THE TOWER SHAFT GEAR. THE ENGINE WAS REMOVED AND REPLACED AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
