REMOVED AND REPLACED ANTICIPATOR. THIS AIRCRAFT HAS BEEN RETURNED TO REVENUE SERVICE. REF# 7481.091524.2054.TT
DURING FLIGHT ON AN AIR AMBULANCE OPERATION. AS THE INCREASE OF COLLECTIVE TO APPROXIMATLEY 9.0 FLI, HEARD THE ENG SURGE WITH A LEFT YAW TO THE ACFT. IMMEDIATELY LOWERED COLLECTIVE & VERIFIED ALL SYS NORMAL. SMOOTHLY AND SLOWLY INCREASED THE COLLECTIVE & AT AROUND 9.0 FLI THE ENG SURGED AGAIN WITH A LT YAW & FLI RAPIDLY INCREASED INTO THE TRANSIENT ON FLI, APPROX 10.2-10.4. DECREASED POWER ONCE AGAIN & CONTINUED AT A LOW POWER SETTING. MADE DECISION( DECLARED EMERGENCY W/ KHTS) TO CONDUCT A LOW POWERED APPROACH TO RUN-ON LANDING TO PREVENT ANY CHANCES OF A HIGHER POWER REQUIREMENT SUCH AS NORMAL APPROACH TO HOVER OR IF HAD TO DO A GO-AROUND FOR ANY REASON WITH NOT ACTUALLY KNOWING WHAT THE AIRCRAFT WOULD DO IF NEEDED TO USE MTOP OR CLOSE TO THAT. MADE THE LOW POWERED APPROACH TO KHTS TAXIWAY AND CONDUCTED A NORMAL ENG SHUTDOWN. DID NOT OBSERVED ANY AIRCRAFT DAMAGE NOR WAS ANYONE INJURED DURING THIS EVENT. NO FURTHER INFORMATION AT WHAT MAY HAVE CAUSED THIS ISSUE.
AIRSPEED INDICATOR FAILURE DUE TO PITOT TUBE COVER REMAINING ON PITOT TUBE. COVER REMAINS ON THE TUBE DURING THE DAY WHEN HELICOPTER NOT IN SERVICE TO PREVENT BUGS FROM ENTERING THE PITOT TUBE WHILE ON GROUND, WHICH HAS OCCURRED IN IMMEDIATE HISTORY.
TAIL ROTOR BALL END AT TGB LEVER HAD FRICTION POINTS WHICH CAUSED STIFFNESS IN TAIL ROTOR PEDALS. REPLACED BALL END WITH NEW.
SMELLED AN ELECTRICAL BURNING TYPE SMELL. LANDED AT THE AIRPORT, SHUT DOWN THE AIRCRAFT AND CALLED MECHANIC. THE MECHANIC ARRIVED AND IDENTIFIED THE SOURCE OF THE SMELL AS THE NR 1 GNS-430W. THIS RADIO WAS REPLACED. NO ADDITIONAL DEFECTS, ODORS, OR SMELLS NOTED.
SKIN CRACKED, FOUND DURING A 100 HOUR INSPECTION. CRACK APPROX 60MM IN LENGTH WAS DETECTED UNDERNEATH THE CABIN FLOOR IN THE RT FWD CANOPY WEB FRAME.
THE PILOT REPORTED THAT WHILE DURING A TRAINING FLIGHT DURING CRUISE APPROXIMATELY 10 MINUTES INTO THE FLIGHT, THE PILOT NOTICED A SMELL OF SMOKE FILL THE CABIN. THIS WAS IMMEDIATELY FOLLOWED BY THE HYD & SERVO AMBER CAUTION LIGHTS INDICATING A RT HYD SYS FAILURE. THE TRAINING CREW ON BOARD IMMEDIATELY OPENED THE CABIN WINDOWS & RETURNED TO THE LOCAL AIRPORT WITH FURTHER DAMAGE OR INCIDENT. AFTER SHUTDOWN, IT WAS NOTED BY THE TRAINING CREW THAT THE CAUSE OF THE SMOKE SMELL WAS A FAULTY HYD PUMP DRIVE. THIS ACFT IS CURRENTLY OUT OF SERVICE FOR THIS EVENT, AWAITING PARTS.
DURING CRUISE FLIGHT, CREW NOTICED THE ODOR OF SMOKE IN COCKPIT, FOLLOWED SHORTLY BY THE HYD & SERVO AMBER CAUTION LIGHT, RT HYD SYS FAILURE. THERE WAS NO LOSS OF ACFT CONTROLLABILITY, RETURNED TO DEPARTURE WITHOUT FURTHER DAMAGE OR INCIDENT. AFTER SHUTDOWN, DISCOVERED THE BELT FOR THE RT HYD PUMP UNDERNEATH THE MAIN ROTOR TRANSMISSION & PUMP ITSELF WAS COMPLETELY SEIZED. REPLACED PUMP DRIVE ASSEMBLY. FUNCTIONAL CHECKED GOOD WITH NO DEFECTS NOTED. THE ACFT WAS THEN RETURNED TO SERVICE.
