CRACK FOUND ON TAILBOOM UPPER LEFT-HAND INTERCOSTAL
PARTIAL ENGINE POWER LOSS AT 5FT HOVER, POWER RECOVERED AT 2FT HOVER. COMPLETED TOUCHDOWN WITH ENGINE POWER. NO HARM TO CREW OR AIRCRAFT.
FLIGHT CREW REPORTED A FUEL ODOR IN CABIN. DURING A MAINTENANCE INVESTIGATION, FOUND FUEL SEEPING AROUND THE LOWER MAIN CELL FUEL PROBE GASKET AND ODOR PERMEATING THROUGH PROBE COVER GASKET.
EXCESSIVE SMOKE FROM EXHAUST ON SHUTDOWN.
TAIL ROTOR HEAD HAS EXCESSIVE BEARING MOVEMENT. REPAIRED.
BELLCRANK ROUGH BEARINGS. REPLACED BEARING (2 EACH) LN9089-20E1 DUE TO WORN. REPAIRED.
ENGINE REMOVED DUE TO RECURRING CHIP LIGHTS. HEAVY STEP WEAR ON GEAR TEETH CAUSING PROBLEM IN EXCESS FOR NORMAL WEAR. REQUEST CREDIT REIMBURSEMENT ON AFA#63953A.

ENGINE REMOVED DUE TO HIGH OIL CONSUMPTION, 5 QUARTS PER FLIGHT HOUR. INSPECTION OF TURBINE PARTS REVEALED EXCESSIVELY GROOVED AND FLAKING INTERNAL SUMP SEALS.
NOT WORKING WHEN IN GUARD.
ENGINE REMOVED DUE TO TURBINE RUBBING. UPON INSPECTION OF PARTS NOTED: SCORING BEYOND O/H MANUAL LIMITS ON NR 6 AND NR 7 BEARING ROLLERS. SERIAL NUMBERS REMOVED ARE MP42920 AND MP42909. NEW PARTS INSTALLED.

LIGHT INOPERATIVE. UNIT WAS TAGGED 412 USE ONLY, CAUTION 5V LIGHTS AND WERE INSTALLED IN A BO105 WHICH IS A 8V LIGHTS, NOW LIGHTS ARE INOPERATIVE - REPLACED 2 EACH 5-VOLT LAMPS P/N 7152, RECALIBRATED, BENCH CHECK GOOD (10-27-98)
TRIPLE RPM INDICATION - NR 2 NEEDLE FLUCTUATING AND READS LOW.
RPM INDICATOR ERRATIC UP TO 30 PERCENT, AND HAS WATER CONTAMINATION.
AUDIO CONTROL HAS BROKEN SWITCH.
SEARCH LIGHT INTERMITTENT AND STAYS ON WHEN SWITCH IS OFF.
FUEL PRESSURE TRANSMITTER HAS FLUCTUATION.
AUDIO CONTROL HAS LOUD STATIC WHEN TALKING ON FM.
TEFLON UNBONDING FROM BEARING. REPLACED WITH NEW BEARING.
ENGINE REMOVED DUE TO 3500 HOUR TURBINE OVERHAUL. UPON INSPECTION OF TURBINE PARTS NOTED: CRACKED BEYOND SERVICEABLE LIMITS ON NR1 NOZZLE SHIELD DOME AREA ABOUT 45% AROUND.
T/R BORE NOZZLE RING SEAT AND SUPPORT PIN SEAT DUE TO WEAR.
KNOBS DON'T CHANGE ANYTHING. PERFORMED PRELIMINARY INSPECTION. ALSO FOUND ENT BUTTON UNREADABLE.
ENCODER OUTPUT REPORTING 21,000 FEET WHILE A/C AT 4,000 FEET.
MAGNETIC COMPASS HAS BROKEN TERMINAL.
AFT FUEL TANK LEAKING.
SEAL LEAKING. REPLACED SEAL WITH NEW PART.
CRACKED GLASS. REPLACED GLASS, BEZEL AND LOCK NUT. REPAIRED. PERFORMED TOTAL CALIBRATION AND ALIGNMENT. INSTALLED 28 VOLT DECAL. BENCH CHECK GOOD.
