CHAFING DAMAGE WAS DISCOVERED ON THE TRANSMISSION DECK. THE DAMAGE IS APPROXIMATELY 0.028â DEEP AND 0.35â WIDE BY 0.52â
WHILE WORKING AAIP A 1660.25.1 A CRACK AROUND 68MM LONG WAS FOUND I THE RT PICKUP WEB OF CANOPY FRAME UNDER THE FLOOR. REMOVED PAINT AND PRIMER TO CONFIRM THE CRACK. FOUND THAT IT WAS INDEED CRACKED. DOCUMENTED THE CRACK AND REPORTED IT TO AMC SERVICE ENGINEERING FOR A CORRECTIVE ACTION.
PILOT REPORTED LOSS OF FLI INDICATION ON APPROACH TO THE BASE. AFTER LANDING AND ROLLING THE THROTTLE TO IDLE, A FLASHING AMBER GOV LIGHT WAS ILLUMINATED. UPON INSPECTION OF THE T4 HARNESS , I FOUND THE T4 HARNESS WAS DAMAGED. AFTER GETTING A VIDEO SCOPE TO INSPECT FURTHER, ANOMALIES WERE FOUND INSIDE THE MODULE 3 OF THE ENGINE. VIDEOS, AND PICTURES FROM THE ENGINE WERE SENT TO TECH REP RICH FULMER AD IT WAS DETERMINED THAT FOR SURE A MODULE 3 WOULD BE NEEDED AND THE POWER TURBINE BLADES WOULD ALSO NEED TO BE INSPECTED.
WHILE TAKING OFF, THE RIGHT PEDAL BECAME INCREASINGLY HARDER TO MOVE AS POWER WAS APPLIED. AFTER LIFTING, ABORTED TAKEOFF. EXECUTED A NORMAL LEFT AND RIGHT PEDAL TURN, MEDIC STATED HE SMELLED SMOKE. RIGHT PEDAL FELT LIKE IT WAS BINDING WHEN APPLIED DURING PEDAL TURNS. LANDED AIRCRAFT ON RAMP, ABORTED FLIGHT REQUEST AND SHUTDOWN.
WHILE PERFORMING AN AIRWORTHINESS CHECK, A CRACK WAS FOUND ON THE UPPER SURFACE OF THE HORIZONTAL STABILIZER ON THE LT SIDE. THE CRACK BEGAN IN THE RIVET LINE OF THE METAL LAP ATTACHMENT POINT ALONG THE TAIL BOOM AT APPROX 3 INCHES FROM THE TE & CURVED FORWARD & OUTWARD TO APPROX 5 INCHES FROM THE TE. CRACK IS MEASURED TO BE APPROX 2.5 INCHES PROTRUDING FROM THE RIVETED SEAM, WHICH IS LONGER THAN THE 2.362 INCHES MAX PERMISSIBLE FOR DRILL STOP.
DEPARTED BASE FOR A SCENE FLIGHT AT 16:06. LANDED ON SCENE AT 16:35. AFTER AN EXTENDED RUN TIME ON THE GND SHUT DOWN THE ENG TO SAVE MY FUEL FOR THE MISSION. AFTER LOADING THE PATIENT IN THE ACFT THE ENG FAILED TO RESTART. IT REACHED 210 RPM & ACTED AS IF IT WAS A HUNG START. START ABORTED & ACFT CALLED & NOTIFIED MECHANICS. MX FOUND THE FUEL VLV TO BE FAULTY. THE PART WAS REPLACED & THIS ACFT RETURNED TO SERVICE WITH NO FURTHER DEFECTS NOTED.
THE PILOT & MEDICAL CREW WERE ON THEIR WAY TO A SCENE LOCATION TO PICK UP A PATIENT WHEN APPROXIMATELY 4 MILES FROM THEIR DESTINATION, THE ENG CHIP LIGHT ILLUMINATED. THE PILOT NOTIFIED THE CREW THAT THEY WOULD NOT BE ABLE TO CONTINUE & PROCEEDED TO FIND A SUITABLE LANDING LOCATION. THE LANDING WAS UNEVENTFUL & MX WAS NOTIFIED OF THEIR LOCATION. ONCE THE MECHANIC ARRIVED, HE CHECKED THE ENG CHIP PLUG ONLY TO FIND A LARGE AMOUNT OF METAL PARTICLES. AFTER REVIEW OF THE MM & A CONVERSATION WITH TECHNICAL SUPPORT, IT WAS DECIDED THAT SEVERAL MODULES OF THE ENG WOULD NEED TO BE REPLACED. THIS ACFT IS CURRENTLY OUT OF SERVICE AWAITING PARTS.
AT CRUISE ALTITUDE 2500 FT MSL, COCKPIT BEGAN TO FILL WITH SMOKE. HYD AND SERVO LIGHTS ON CAUTION PANEL ILLUMINATED. REDUCED AIRSPEED AND MADE EMERGENCY LANDING. AFTER LANDING AND ENGINE SHUTDOWN THE PRIMARY HYD BELT WAS FOUND BROKE AND MISSING. NO OTHER ACFT DAMAGE WAS NOTED. MECHANIC INSPECTED THE ACFT AND FOUND THE HYD PUMP DRIVE ASSEMBLY HAD FAILED CAUSING THE BELT TO BREAK. REPLACED THE PUMP DRIVE ASSY TO INCLUDE A NEW BELT WITH NO ADDITIONAL DEFECTS. THE ACFT WAS RETURNED TO SERVICE AND FLOWN BACK TO ITS BASE OF OPERATION.
