F/A REPORTED A BURNING ODOR COMING FROM CEILING LIGHT IN GALLEY ONCE TURNED OFF, THE ODOR DISSAPATED. MX R & R THE GALLEY CEILING LIGHT LAMP WITH OPS CHECK SUCCESSFUL. ACFT WAS APPROVED FOR RETURN TO SERVICE.
CREW REPORTED ICE COND- ANTI ICE SYS INOPERATIVE WITH WING ANTI/ICE FAIL. AN ODOR THAT SMELLED LIKE BURNED CRAYONS ENTERED THE CABIN. ANTI-ICE WAS OBSERVED TO WORK INTERMITTENTLY. MX R & R DUAL TEMP CONTROL VALVE SLEEVES THAT WERE FOUND LEAKING. OPS CHECKS WERE SUCCESSFUL AND NO ODOR'S NOTED. ACFT WAS APPROVED FOR RETURN TO SERVICE.

CLE FLIGHT 2423 - THE CREW REPORTED AN APU FIRE WARNING DURING TAXI WITH APU AUTOSHUT DOWN. THE AIRCRAFT RETURNED TO THE GATE. MAINTENANCE INSPECTED AND REMOVED EXCESSIVE BUILD UP OF ICE AND SNOW FROM THE APU INLET. OPERATIONAL CHECKS OF APU WAS GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.

CXO - FLIGHT 4945, THE CREW REPORTED THAT THE BAGGAGE EXT LIGHT ILLUMINATES IN FLIGHT. THE AIRCRAFT LANDED IN CXO WITHOUT INCIDENT WHERE MAINTENANCE INSPECTED THE AIRCRAFT, OPERATIONALLY CHECKED THE SMOKE DETECTORS WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

DURING ROUTINE MAINTENANCE, THE AFT CABIN AISLE PATH EMERGENCY LIGHT WAS INOPERATIVE. MAINTENANCE INSPECTED AND REPLACE THE NR 4 EMERGENCY LIGHT BATTERY. OPERATIONAL CHECKS WERE GOOD.
DURING 1A INSPECTION MAINTENANCE FOUND EMERGENCY LIGHT ON RIGHT SIDEWALL AFT OF THE EMERGENCY EXIT HATCH NOT WORKING. REMOVED AND REPLACED EMERGENCY EXIT SIGN IAW EMB145 AMM NR 33-50-00. OPS CHECKED GOOD.
DURING FLIGHT, PILOT REPORT LOW HYDRAULIC QUANTITY INDICATION ON NR 1 ENGINE AND DECLARED EMERGENCY LANDING. MX FOUND NR 1 ENGINE PUMP LEAKING. REMOVED AND REPLACED NR 1 ENGINE HYD PUMP IAW AMM 29-10-03 LEAK AND OPS CHECK GOOD. SERVICED NR 1 HYD SYSTEM IAW EMB 145 AMM 12-13-01. AIRCRAFT WAS APPROVED TO RETURN FOR SERVICE.

DURING A SCHEDULED INSPECTION THE CENTER OMEGA BEAM IN THE GALLEY WAS FOUND CORRODED. MAINTENANCE REMOVED AND REPLACED THE CENTER OMEGA BEAM. IAW SRM-51-40-02.

IAH - FLIGHT 2720, THE CREW REPORTED THAT DURING FLIGHT THEY SMELLED A BURNED PACK SMELL WITH NO INDICATION OF SMOKE. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT WHERE MAINTENANCE REMOVED AND REPLACED THE NR 1 ENGINE, OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING MAINTENANCE INSPECTION, ONE AISLE PATH EMERGENCY LIGHT STRIP WAS FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED LIGHT STRIP, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. THIS PART HAS NO S/N AND TIMES ARE NOT TRACKED.
EWR - CREW REPORTED THAT THE NR 1 PACK WAS BLOWING ONLY WARM AIR IN AUTO & MANUAL MODES WITH MINIMAL AIRFLOW, AND A BURNING SMELL AT LOW POWER SETTINGS. THE AIRCRAFT WAS RELEASED ON MEL 21-51-00-1 CONTROL NR 048227. FLIGHT 3138 IAH MAINTENANCE REMOVED AND REPLACED THE NR 1 PACK DUAL VALVE, OPERATIONALLY CHECKED WITH NO DEFECTS. REMOVED THE MEL, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 2964, THE CREW REPORTED THAT THE LANDING GEAR WOULD NOT RETRACT. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE OPERATIONALLY TESTED THE LANDING GEAR RETRACTION AND EXTENSION SYSTEM SEVERAL TIMES WITH NO DEFECTS NOTED AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CREW REPORTED, SMOKE CAME FROM FIRST OFFICERS YOKE READING LIGHT AFTER LANDING. MAINTENANCE IN MSN DEACTIVATED FIRST OFFICER'S CHARTHOLDER LIGHT AND RELEASED THE AIRCRAFT IAW MEL 33-10-00 CAT-C CONTROL 046503. UPON ARRIVAL INTO EWR MAINTENANCE SECURED CHARTHOLDER WIRES AND OPS CHECKED LIGHTS, OPS CHECKED GOOD IAW EMB145 MM 33-11-01. NO SMOKE NOTED. MEL CLEARED AND AIRCRAFT RETURNED TO SERVICE.
IAH - FLIGHT 2197, THE CREW REPORTED A STRONG FUEL ODOR WHEN THE APU WAS STARTED AFTER LANDING. MAINTENANCE OPERATIONALLY CHECKED THE APU WITH NO DISCREPANCIES NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING PRE-FLIGHT INSPECTION, THE CREW REPORTED A SMOKE ODOR IN THE COCKPIT. MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND THE AIR DATA COMPUTER TO BE THE SOURCE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED COMPUTER, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

DURING SCHEDULED MAINTENANCE THE LT OVERWING EMERGENCY LIGHT WAS FOUND INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE LAMP IN THE AFFECTED LIGHT ASSEMBLY, OPERATIONALLY CHECKED THE LIGHT AND CLEARED THE DISCREPANCY.
DURING SCHEDULED MAINTENANCE THE FORWARD EMERGENCY LIGHTS IN THE CABIN WERE FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE NR 1 EMERGENCY LIGHT BATTERY PACK, OPERATIONALLY TESTED THE EMERGENCY LIGHTS AND CLEARED THE DISCREPANCY.
RT SIDE FORWARD OVERWING EMERGENCY LIGHT WAS FOUND INOPERATIVE. MAINTENANCE PLACED ON MEL 33-50-00/1, PLACARD NR 038565 FOR FURTHER TROUBLESHOOTING. MAINTENANCE RELAMPED THE EMERGENCY EXIT LIGHT, OPERATIONAL CHECKED GOOD.
TYS, DURING A SCHEDULED INSPECTION THE SHEAR CLIP AT FR61 BETWEEN ST20L AND 21L WAS FOUND CORRODED. MAINTENANCE REMOVED AND REPLACED THE CLIP IAW EMB-135/145 SRM 51-40-02.
TYS, DURING A SCHEDULED INSPECTION CORROSION WAS FOUND ON THE LEFT SUPPORT PULLY CMD STAB BRACKET BETWEEN FR60 AND 61, STR 21L AND 20L. MAINTENANCE REMOVED AND REPLACED THE PULLY SUPPORT BRACKET IAW EMB 135/145 SRM 51-40-02.

ALL WORK WAS ACCOMPLISHED AND WILL BE RELEASED UPON COMPLETION OF ALL WORK.

MAINT REMOVED THE OMEGA BEAM, FAB SPLICE DOUBLER, ALODINED, PRIMED AND INSTALL SPLICE DOUBLER AND INSTALL THE NEW OMEGA BEAM IAW EMB 135/145 SRM 51-40-02; 53-21-10; CPM 51-21-04-05 AND SRM 53-21-10. ALL WORK WAS ACCOMPLISHED AND WILL BE RELEASED UPON COMPLETION OF ALL WORK.

ALL WORK WAS ACCOMPLISHED AND WILL BE RELEASED UPON COMPLETION OF ALL WORK.

DURING PREFLIGHT THE CREW NOTED THAT THE NR 1 EMERGENCY LIGHT STRIP WAS INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE NR 1 EMERGENCY LIGHT STRIP, PERFORMED A OPERATIONAL CHECK, NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH FLIGHT 3650, THE CREW REPORTED THE AISLE PATH LIGHTING WAS INOPERATIVE AT THE MAIN CABIN DOOR. MAINTENANCE INSPECTED AND REPLACED THE AISLE PATH LIGHTING STRIP. OPERATIONAL CHECKS WERE GOOD.

IAH- THE CREW NOTED THAT THE FORWARD EXIT EMERGENCY LIGHT INTERMITTENT. MAINTENANCE SECURED A LOOSE CONNECTOR ON THE EMERGENCY EXIT LIGHT ASSY, PERFORMED A OPERATIONAL CHECK OF THE EMERGENCY LIGHTING SYSTEM, NO DEFECTS NOTED. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CLE FLIGHT 3175, THE CREW REPORTED THE FORWARD AISLE PATH EMERGENCY LIGHTS WERE INOPERATIVE. MAINTENANCE REPLACED THE EMERGENCY LIGHT STRIP. OPERATIONAL CHECKS WERE GOOD.
CLE FLIGHT 4040 THE CREW REPORTED THE CABIN EMERGENCY FLOOR LIGHT STRIP WAS INOPERATIVE. MAINTENANCE REPLACED THE LIGHT STRIP ASSEMBLY. OPERATIONAL CHECKS WERE GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.
ORF Flt#3483-Crew reported Air/Ground circiut breaker A29 popped. Maintenance found the # 2 Wieght on Wheels proximity sensor cracked.Maintenance removed and replaced the # 2 Weight on Wheels proximity sensor and performed a satisfactory system operational test. The aircraft was approved for return to service.
IAH- DURING C-CHECK INSPECTION IT WAS NOTED THAT THE OMEGA BEAM AT FRAME 20 WAS CORRODED. MAINTENANCE REMOVED THE CORRODED AREA, FABRICATED A FILLER, REINFORCEMENT AND INSTALLED IT ON THE OMEGA BEAM AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE, WHEN RELEASED FROM C-CHECK.
IAH- DURING C-CHECK IT WAS NOTED THAT THE GALLEY AREA HAS NUMEROUS AREAS OF CORROSION ON THE UPPER SURFACES OR THE FLOORBEAMS FROM FRAME 15 TO FRAME 23. MAINTENANCE REMOVED AND REPLACED THE RIGHT FLOOR SILL FROM FRAME 22 TO FRAME 23 AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE, WHEN RELEASED FROM C-CHECK.

MAINTENANCE REMOVED AND REPLACED THE RIGHT FLOOR SILL FROM FRAME 19 TO FRAME 20 AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE, WHEN RELEASED FROM C-CHECK.

IAH- MAINTENANCE REMOVED THE DAMAGED AREA,AND, INSTALLED A DOUBLER WITH A FILLER,AND, THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE WHEN RELEASED FROM "C" CHECK.

IAH - DURING MAINTENANCE INSPECTION, THE AISLE PATH LIGHTING AT SEATS 8A THRU 12A WAS FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED EMERGENCY LIGHT BATTERY PACK, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SLW - DURING PRE-FLIGHT INSPECTION, THE CREW REPORTED THAT THE AISLE PATH LIGHTING AT ROW 11-12 WAS INOPERATIVE. THE AIRCRAFT WAS FERRIED TO IAH WHERE MAINTENANCE REMOVED AND REPLACED THE AFFECTED EMERGENCY LIGHT BATTERY PACK. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH- DURING 1A INSPECTION IT WAS NOTED THAT THE EMERGENCY FLOOR LIGHTING MIDDLE SECTION DID NOT ILLUMINATE WHEN SELECTED. IAH- MAINTENANCE REMOVED AND REPLACED THE EMERGENCY BATTERY, OPERATIONAL CHECKED. NO DEFECTS NOTED AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

AMA - FLIGHT 4022, THE CREW REPORTED A FUEL SMELL IN THE COCKPIT AND CABIN, THE AIRCRAFT RETURNED TO AMA WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE NR 4 THROUGH NR 13 FUEL NOZZLES ON THE NUMBER TWO ENGINE IN ACCORDANCE WITH R/R MM 73-11-10, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. THE PROBLEM REOCCURED ON 03/02/01 ON FLIGHT 3997 FROM LBB TO IAH. THE AIRCRAFT RETURNED TO LBB WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE NR 1, 2, 3, 14, 15 AND 16 FUEL NOZZLES ON THE NUMBER TWO ENGINE, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE WITH NO FURTHER INCIDENTS TO DATE.
EWR - FLIGHT 4148, THE CREW REPORTED THAT DURING THE IN-FLIGHT PORTION OF THE ICE TEST THE CABIN BEGAN TO FILL WITH SMOKE AND A STRONG ODOR. THE AIRCRAFT LANDED AT EWR WITHOUT INCIDENT WHERE MAINTENANCE PLACED THE NUMBER TWO RECIRCULATION FAN ON MEL 21-24-01 CONTROL NR 007600, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. THE AIRCRAFT LANDED AT IAH ON 03/03/01 WHERE MAINTENANCE REMOVED AND REPLACED THE NUMBER TWO RECIRCULATION FAN, REMOVED THE MEL, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
IAH - FLIGHT 4205, THE CREW REPORTED A STRONG ODOR IN THE COCKPIT AND CABIN. MAINTENANCE PERFORMED OPERATIONAL CHECKS ON THE AIR CONDITIONING SYSTEM WITH NO DEFECTS NOTED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CAE - FLIGHT 4205, THE CREW REPORTED SMOKE IN THE CABIN, THE AIRCRAFT DIVERTED TO CAE WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT, AND OPERATIONALLY CHECKED THE AIR CONDITIONING SYSTEM WITH NO DEFECTS NOTED. THE AIRCRAFT WAS THEN APPROVED FOR RETURN TO SERVICE.
BNA - DURING MAINTENANCE INSPECTION, THE TOP RT EXTERNAL OVERWING EMERGENCY EXIT LIGHT WAS FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED LIGHT ASSEMBLY, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CLE - FLT 3965 - THE CREW REPORTED A SECTION OF EMERGENCY LIGHTING INOPERATIVE NEAR THE EMERGENCY EXIT. MAINTENANCE REMOVED AND REPLACED THE NR 3 EMERGENCY LIGHT BATTERY. OPERATIONAL CHECKS WERE GOOD IAW 33-50-04 AND 33-50-00. THE AIRCRAFT WAS RELEASED FOR SERVICE. (M)
