PILOT REPORTED A #2 ENGINE CHIP LIGHT ILLUMINATED IN FLIGHT. HE PERFORMED 2 CHIP BURNS PER THE RFM AND THE LIGHT CAME BACK ON. HE PERFORMED A PRECAUTIONARY LANDING WITH A PATIENT ON BOARD WITHOUT ISSUE. PATIENT WAS TRANSPORTED BY GROUND. MECHANIC INSPECTED CHIP PLUGS AND FOUND NO INDICATION OF A CHIP. MECHANIC TESTED THE CHIP DETECTORS PER THE SAFRAN ENGINE MANUAL WITH NO DEFECTS. PERFORMED A 15 MINUTE GROUND RUN WITH NO FURTHER DEFECTS.

PILOT REPORTED A #1 ENGINE FIRE LIGHT ILLUMINATED IN FLIGHT. PILOT PRESSED EMER OFF SWITCH 1 AND ENGINE FIRE BOTTLE #1 AND PERFORMED A RUN-ON LANDING. PATIENT WAS TRANSPORTED BY GROUND. MAINTENANCE DISCOVERED AN OIL LINE FROM MODULE 4 AT THE #7 BEARING RETURN OIL LINE HAD LOST TORQUE AND HAD SPRAYED OIL ON THE HOT ENGINE CAUSING THE SMOKE FROM THE COWLINGS. MECHANICS ARE CLEANING THE ENGINE INTERNAL AND EXTERNAL FOR THE HALON 1301 AND THE OIL SPRAY.

THE MECHANIC NOTICED THE CYCLIC LINK HAD COME IN CONTACT WITH THE INNER RING AND DURING HIS INVESTIGATION TO DETERMINE THE CAUSE OF THIS UNUSUAL CONDITION, HE FOUND SEVERAL PARTS TO BE EXCESSIVELY DAMAGED CAUSED BY A BROKEN BOLT 20-057-6-16D, TOTAL TIME 1688.10 OR WORN SLIDERS 222-310-469-103, TOTAL TIME 1688.10. THE SWASHPLATE WAS REMOVED FROM AIRCRAFT AND SENT TO OVERHAUL SHOP TO DETERMINE CAUSE OF FAILURE. COMPLIED WITH ALL CONDITIONAL INSPECTIONS AND OVERHAUL REQUIREMENTS. CONTACTED MFG CUSTOMER REP FOR ADDITIONAL INSTRUCTIONS THAT MFG MAY WANT TO PERFORM. LIST OF DAMAGED PARTS AS FOLLOWS -TOTAL TIME ON ALL 1688.10 - 1 EA. BOLT 20-057-6-16D CRACKED; 1 EA. BOLT 20-057-6-16D EX.
