MAINTENANCE INSPECTION OF AIRCRAFT FOUND RT ENDPLATE UPPER COUNTERSUNK SCREWS SHEARED OFF, LOWER ANGLE ATTACHMENT PLATE BROKEN CLEAN. TRAILING EDGE CLOSURE HAS DAMAGE TO OUTBOARD RIB STRUCTURE ANGLE (REPAIRABLE POSSIBLY) DAMAGE LIMITED TO MISSING END PLATE, LOWER ATTACHMENT ANGLE AND TRIALING EDGE.
PILOT PERFORMED A SAFE PRECAUTIONARY LANDING FOR A BIRD STRIKE TO THE RIGHT HAND WINDSCREEN WITH PENETRATION. MECHANIC REMOVED WINDSCREEN PN 407-546-522 AND REPLACED WITH NEW WINDSCREEN, SAME PN. NO OTHER DEFECTS NOTED. ACFT RETURNED TO SERVICE.
PILOT PERFORMED A PRECAUTIONARY LANDING FOR A BURNING SMELL IN THE CABIN. DURING INSPECTION OF THE ACFT THE MECHANIC FOUND A RELAY WITH SIGNS OF HIGH HEAT. MECHANIC REPLACED RELAY PN 389FXAXC1-24D. FUNCTION CHECK OK WITH NO FURTHER SMELL
PILOT PERFORMED A PRECAUTIONARY LANDING FOR #2 HYDRAULICS FAILED IN FLIGHT. MECHANIC DISCOVERED THE HYDRAULIC PUMP BELT HAD SHEARED. DURING REMOVAL IT WAS DISCOVERED THE HYDRAULIC PUMP BEARING FAILED AND CAUSED THE BELT TO SHEAR. MECHANIC REMOVED AND REPLACED THE HYDRAULIC PUMP PULLEY AND BEARING ASSY AND REPLACED THE HYDRAULIC PUMP BELT. BIS.
PILOT PERFORMED A PRECAUTIONARY LANDING IN VACANT FIELD IN CARTHAGE, MS FOR IN STRAIGHT AND LEVEL FLIGHT AT 2500 MSL AT 120 KIAS, THERE WAS A HIGH FREQUENCY VIBRATION IN THE ANTI-TORQUE PEDALS. THE VIBRATION REMAINED THROUGH DESCENT TO LANDING.
PILOT REPORTED BIRD STRIKE THAT BROKE THE PILOTS LH SIDE WINDSCREEN. PRECAUTIONARY LANDING WITHOUT INCIDENT.
PILOT REPORTED A #2 ENGINE CHIP LIGHT ILLUMINATED IN FLIGHT. HE PERFORMED 2 CHIP BURNS PER THE RFM AND THE LIGHT CAME BACK ON. HE PERFORMED A PRECAUTIONARY LANDING WITH A PATIENT ON BOARD WITHOUT ISSUE. PATIENT WAS TRANSPORTED BY GROUND. MECHANIC INSPECTED CHIP PLUGS AND FOUND NO INDICATION OF A CHIP. MECHANIC TESTED THE CHIP DETECTORS PER THE SAFRAN ENGINE MANUAL WITH NO DEFECTS. PERFORMED A 15 MINUTE GROUND RUN WITH NO FURTHER DEFECTS.
PILOT REPORTED A TAIL ROTOR GEARBOX CHIP LIGHT IN FLIGHT AND PERFORMED A PRECAUTIONARY LANDING AT IOWA CITY, IA AIRPORT. MECHANIC INSPECTED TAIL ROTOR CHIP DETECTOR AND FOUND A CAT D CHIP. PERFORMED MAINTENANCE IAW AMM 64-40-00, 6-1. PERFORMED MOC WITH NO FURTER DEFECT NOTED.
PILOT REPORTED A MAIN XMSN CHIP LIGHT IN FLIGHT AND PERFORMED A PRECAUTIONARY LANDING IN MANHATTAN, KS WITHOUT INCIDENT. MECHANIC INSPECTED ALL 3 CHIP DETECTORS AND FOUND NOTHING VISIBLE ON THE CHIP DETECTORS. XMSN IS CURRENTLY IN PROCESS OF REPLACEMENT DUE TO THE FREQUENCY OF THE CHIP INDICATIONS.

CLEANED CHIP DETECTOR AND PERFORMED A SERVICEABILITY CHECK. DRAINED AND SERVICED WITH NEW OIL, REPLACED OIL FILTER. MOC OK WITH NO FURTHER DEFECTS.
PILOT REPORTED A #1 ENGINE FIRE LIGHT ILLUMINATED IN FLIGHT. PILOT PRESSED EMER OFF SWITCH 1 AND ENGINE FIRE BOTTLE #1 AND PERFORMED A RUN-ON LANDING. PATIENT WAS TRANSPORTED BY GROUND. MAINTENANCE DISCOVERED AN OIL LINE FROM MODULE 4 AT THE #7 BEARING RETURN OIL LINE HAD LOST TORQUE AND HAD SPRAYED OIL ON THE HOT ENGINE CAUSING THE SMOKE FROM THE COWLINGS. MECHANICS ARE CLEANING THE ENGINE INTERNAL AND EXTERNAL FOR THE HALON 1301 AND THE OIL SPRAY.

PILOT REPORTED #1 ENGINE OIL TEMPERATURE EXCEEDANCE OF 129.5. PLACED THE #1 ENGINE TO IDLE AND EXECUTED A SUCCESSFUL PRECAUTIONARY LANDING WITH A PATIENT ON BOARD TO KROA AIRPORT. THE PATIENT WAS TRANSPORTED BY GROUND. THE MECHANIC REPLACED THE #1 ENGINE OIL COOLER BYPASS VALVE PN L792M2002102 AND UNSCHEDULED MAINTENANCE PER PRATT AND WHITNEY EMM CH. 05-50-00.
PILOT REPORTED A BIRD STRIKE THROUGH WINDSCREEN WITH EXTENSIVE DAMAGE. AIRCRAFT LANDED AT THE STORM LAKE IA AIRPORT NE OF HOME BASE. SHATTERED WINDSCREEN PN 407-546-522 REMOVED AND REPLACED WITH NEW, SAME PN WINDSCREEN.

MECHANIC INSPECTED MAIN TRANSMISSION CHIP DETECTORS AND FOUND SMALL CHIPS. MECHANIC FLUSHED THE TRANSMISSION AND RESERVICED. PERFORMED A 20 MINUTE GROUND RUN UNDER POWER. NO FURTHER DEFECTS NOTED. ACFT RELEASED FOR RETURN TO SERVICE.
DURING FINAL APPROACH TO THE HOSPITAL IN LOVELAND CO, THE CREW REPORTED THE SMELL OF BURNT RUBBER. AFTER LANDING, THE PILOT INVESTIGATED AND FOUND THE A/C COMPRESSOR BELT SHREEDED. HE ALSO NOTED THAT THE DAMAGED A/C BELT CAUSED SIGNIFICATE DAMAGE TO THE HYRDAULIC BELT THAT IS IN PLACE RIGHT NEXT TO IT. THE HYRDAULIC BELT HAD 50% OF THE BELT RIPPED APART FROM THE A/C COMPRESSOR BELT. MAINTENANCE ARRIVED AND REPLACED THE HYDRAULIC BELT AND MEL'S THE A/C FOR COMPRESSOR REPLACEMENT ONCE PARTS ARRIVED.
PILOT PERFORMED A PRECAUTIONARY LANDING SAFELY WITH A PATIENT ON BOARD. PATIENT TRANSFERRED BY GROUND. #1 ENGINE FIRE LIGHT ILLUMINATED IN FLIGHT. MECHANIC TROUBLESHOT THE FIRE DETECTOR SYSTEM ON THE ACFT AND SYSTEMS PASSED PER THE AMM CHAPTERS OF 26-10. ACFT REPOSITIONED TO BASE AND FURTHER TROUBLESHOOTING REVEALED THAT THE AFT PROBE FAILED 5 OF 8 TEST AND PRODUCED A FIRE INDICATION AT A LOWER TEMPERATURE AS PER THE AMM CHAPTER. REPLACED AFT FIRE PROBE.

MECHANIC PERFORMED VISUAL INSPECTIONS OF THE MAIN ROTOR SYSTEM, MAIN TRANSMISSION, BOTHE ENGINES AND THE TAIL ROTOR SYSTEM IAW EC135 AMM. NO DEFECTS NOTED. PILOT PERFORMED A SUBSEQUENT OPERATIONAL CHECK FLIGHT WITH NO DEFECTS NOTED. AIRCRAFT RELEASED FOR RETURN TO SERVICE.
PILOT REPORTED MGB CHIP FOR 1 SECOND DURING STRAIGHT AND LEVEL FLIGHT AT 3500 FT MSL, 88% Q 685 MGT. PRECAUTIONARY LANDING INTO TLH. PATIENT WHO WAS ON BOARD WAS PICKED UP BY COMPANY AIRCRAFT AND TRANSPORT WAS COMPLETED TO GAINESVILLE, FL. MECHANIC FOUND ONE HAIR LIKE FERROUS DEBRIS APPROXIMATELY .75 INCH LONG ON THE LOWER MGB CHIP PLUG. AS PER THE AMM THE CHIP PLUG WAS CLEANED AND REINSTALLED GROUND RAN FOR 15 MINS LIGHT ON SKIDS WITH NO FURTHER DEFECTS NOTED AT THIS TIME. AIRCRAFT RELEASED FOR SERVICE.
PILOT PERFORMED A PL WITH A PATIENT ON BOARD FOR AN MGT EXCEEDANCE INDICATION ILLUMINATED DURING FLIGHT. PATIENT WAS TRANSPORTED BY GROUND. INSTRUMENTS DOWNLOADED AND REVEALED A PEAK OF 737 DEG TEMP EXCEEDANCE. ENGINE INSPECTED IAW ENGINE INSPECTION TABLE 607, ITEM 6 AND NO DEFECTS NOTED.
DURING PILOT TRAINING THE NR 1 HYDRAULIC SYSTEM INDICATION ILLUMINATED DURING FLIGHT. THE LOW HYDRAULIC WARNING INDICATION ILLUMINATED AND APROXIMATELY 10 SECONDS LATER THE HYDRAULIC SERVO LIGHT ILLUMINATED. PILOT LOWERED THE LANDING GEAR AND LANDED WITHOUT INCIDENT. MAINTENANCE TROUBLESHOT THE HYDRAULIC SYSTEM AND DISCOVERED THE HYDRAULIC PUMP PRESSURE LINE TO BE CHAFFED AND LEAKING. HYDRAULIC PUMP WILL BE REPLACED DUE TO RUNNING DRY, AS WELL AS, THE PUMP PRESSURE AND RETURN LINES. DURING THE INSPECTION AS TO THE LOSS OF HYDRAULIC FLUID, IT WAS DISCOVERED THAT THE RUBBER INSIDE PORTION OF AN ADEL CLAMP HAD DETERIORATED TO THE POINT OF CHAFFING THE PRESSURE LINE AND WAS THE CAUSE OF THE SUBSEQUENT DAMAGE.
PILOT PERFORMED A PL WITH A PATIENT ON BOARD AND REPORTED ENGINE CHIP LIGHT ILLUMINATED DURING FLIGHT. PATIENT WAS TRANSPORTED BY GROUND.
PILOT PERFORMED A PL WITH A PATIENT ON BOARD AND REPORTED NR 2 ENGINE OIL TEMPERATURE ABOVE NORMAL LIMITS IN FLIGHT, PEAKED AT 134 DEGREES. MECHANIC INSPECTED ENGINE CHIP DETECTORS, DRAINED AND RESERVICED ENGINE OIL AND REPLACED NR 2 ENGINE OIL BYPASS VALVE. PERFORMED MOC AND OIL LEAK CHECK, NO DEFECTS NOTED.
PILOT PERFORMED A PL WITH A PATIENT ON BOARD AND REPORTED XMSN CHIP LIGHT ILLUMINATED DURING FLIGHT. PATIENT WAS TRANSPORTED BY GROUND. MECHANIC INSPECTED AL 3 CHIP DETECTORS OF THE ILO SYSTEM AND FOUND NO DEBRIS ON THE CHIP DETECTORS. PERFORMED 20 MINUTE GROUND RUN UNDER POWER WITH NO FURTHER INDICATIONS.
PILOT REPORTED ON GLIMB OUT AT 80 KNOTS HEARD A SERIES OF LOUD REPORT ON NR 1 ENGINE SIDE. MECHANIC INVESTIGATED BY INSPECTING XMSN AND DRIVETRAIN COMPONENTS AND NR 1 AND NR 2 ENGINES AND FOUND NO DEFECTS OR ABNORMALITIES. DOWNLOADED ENGINE DCU'S AND FORWARDED TO ENGINEERING FOR EVALUATION. NO DEFECTS NOTED. PERFORMED A HOVER CHECK AND FLIGHT AT 1200 FEET PER FOR POSSIBLE COMPRESSOR STALL. NO DEFECTS NOTED. ACFT RETURNED TO SERVICE.
PILOT REPORTED DURING CRUISE FLIGHT A SUDDEN LOUD BUZZING/GRINDING NOISE THAT DROWNED OUT THE CABIN NOISE. IT FOLLOWED POWER APPLICATION.

PILOT REPORTED A FUEL SMELL IN THE CABIN. MECHANIC TROUBLESHOT THE SMELL. REPLACED FUEL CAP AND REAPPLIED SEALANT TO FUEL CAP COMPONENTS. SMELL STILL PRESENT.
PILOT MADE A PL BACK TO HOSPITAL PAD WITH A PATIENT ON BOARD AND REPORTED AFTER 2 MINUTES INTO THE FLIGHT WITH CABIN HEAT APPLIED, CABIN BAGAN TO FILL WITH SMOKE ON THE CEILING. MECHANIC TROUBLESHOT AND FOUND EXCESS INLET BARRIER FILTER FLUID IN THE ENGINE INTAKE PLENUM FROM RECENT FILTER SERVICEING. CLEANED EXCESS FLUID AND PERFORMED ENGINE RUNUP WITH HEATER APPLIED AND NO FURTHER DEFECTS.
PILOT MADE A PL FOR AN UNUSUAL NOISE IN THE CABIN AND NO PATIENT ON BOARD. MECHANIC INSPECTED AIRCRAFT AND FROM DESCRIPTION OF THE NOISE SUSPECTED AIR CONDITIONER BLOWER. PERFORMED RUN UP OF AIRCRAFT AND COUL NOT DUPLICATE NOISE. ACFT RETURNED TO SERVICE.
PILOT MADE A PL INTO AIRPORT, FOR A #2 ENGINE FADEC FAIL WITH A PATIENT ON BOARD. #2 ENGINE WENT TO 80%. THE MECHANIC DOWNLOADED THE FADEC SYSTEM CODES AND IT WAS DETERMINED TO BE IN THE FUEL MANAGEMENT MODULE COMPONENTT. THE MECHANIC DISCOVERED THE LOCKING PORTION OF THE P5 CONNECTOR TO THE FMM WAS DAMAGED. REPLACED ENGINE HARNESS CABLE.
PILOT MADE A PL WITHOUT INCIDENT NEAR SIX MILE, SC, NO PATIENT ON BOARD WHILE REPOSITIONING THE ACFT. THE ENGINE OIL PRESSURE WAS INDICATING LOW AND FLUCTUATING. UPON MECHANICS INSPECTION, IT WAS DETERMINED THAT THE COMPRESSOR #1 BEARING OIL SUPPLY TUBE HAD DEVELOPED A LEAK WHICH IS INTERGRAL TO THE INTAKE HOUSING.
NR 2 ENG OIL TEMP INDICATION OUT OF LIMITS. PRECAUTIONARY LANDING WAS MADE AT AIRPORT.
WHILE IN CRUISE FLIGHT, THE PILOT LOOKED AT THE ENGINE OIL PRESSURE GAUGE AND SAW NO INSTRUMENT INDICATION. LANDED WITHOUT INCIDENT. MECHANIC TROUBLESHOT THE ENGINE OIL PRESSURE TRANSDUCER SYSTEM FOR THE INDICATION AND CLEANED THE CONNECTOR PLUG WITH NO OBVIOUS DEFECTS TO WIRING AND REINSTALLED. WITH SUBSEQUENT OPERATIONS CHECK, NO DEFECTS WERE NOTED AND THE ACFT WAS PLACED BACK INTO SERVICE.
DURING 3600 HOUR OVERHAUL FOUND BRAKE CALIPER MOUNTING BRACKET CRACKED, STARING AT LOWER CENTER CORNER EXTENDING AROUND BOLT HOLE ON BOTH SIDES. NO OBVIOUS SIGNS OF WHAT STRESSES CAUSED THE CRACK, NOR ANY INDICATION OF AN INDECENT.

ACFT LANDED DUE TO AN UNUSUAL VIBRATION. ON EVALUATION OF THE ACFT PILOT NOTICED THE LT SCENE LIGHT HOUSING BOLT WAS BROKEN. MAINTENANCE REMOVED THE LIGHT HOUSING, SECURED WIRING.
