FADEC RESTART FAULT ILLUMINATED IN FLIGHT. NO OTHER ABNORMALITIES. DIVERTED TO NEARBY AIRPORT WHERE MAINTENANCE COULD BE PERFORMED. FOUND WFMV FAULT CODE. CLEARED FAULT CODE. PERFORMED NORMAL START UP AND RETURNED TO BASE.
MED CREW SMELLED FUEL IN AFT CABIN.
SUSPECTED A CRACK IN THE TAILBOOM ATTACH POINT FRAME ASSY. CONFIRMED PART WAS ACTUALLY CRACKED (AFTER DETAILED VISUAL INSPECTION) AT THE UPPER RH BOLT HOLE AREA AFTER PART REMOVAL . REPLACED FRAME WITH A NEW UNIT AT THAT POINT. P/N OFF SUSPECTED P/N 206-032-308-103, S/N UNK, P/N ON 407-030-027-107. S/N N/A
DURING PRE-TAKEOFF FOR A SECOND FLIGHT OF THE DAY. THE PILOT NOTICED THE TAIL ROTOR CHIP LIGHT ILLUMINATED. PILOT COMPLETED SHUTDOWN OF AIRCRAFT WITHOUT INCIDENT. ON INITIAL INSPECTION OF CHIP DETECTOR THE NORMAL FUZZ WITH SLUDGE APPEARED. AFTER FURTHER INSPECTION WITH REMOVAL OF BEVEL GEAR PLUG, THE MECHANIC DETECTED NOTICEABLE WEAR ON THE PINION DRIVE GEAR THAT WAS CAUSE FOR REJECTION. CLEANED THE CHIP DETECTOR 4 HOURS BEFORE DURING EVENT 4 INSPECTION AND NO METAL FOUND AT THAT TIME. THIS IS THE 4TH CHIP LIGHT OCCURRENCE IN THE LAST 150 HOURS.
DURING AN AIRWORTHINESS, PREFLIGHT INSPECTION, ONE CRACK ESTIMATIED TO BE .03125"-.0625" WAS FOUND ON THE CENTER TIP BLOCK SEAL OF THE TAIL ROTOR BLADE WHEN COMPLYING WITH INSTRUCTIONS LAID OUT IN THE MM A ADDITIONAL CRACK WAS FOUND ON THE TRAILING TIP BLOCK SEAL. THE SECOND CRACK HAD NOT SHOWN INDICATIONS THROUGH THE PAINT.
TAIL ROTOR CHIP LIGHT ILLUMINATED IN FLIGHT. PERFORMED PRECAUTIONARY LANDING AT NEARBY AIRFIELD. FOUND ONE SMALL SLIVER ON TAIL ROTOR CHIP PLUG. GEARBOX WAS RECENTLY REPLACED, DETERMINED TO MOST LIKELY BE BREAK IN WEAR.
THE OIL COOLER COWL WAS REMOVED TO FACILITATE REMOVAL OF THE TAIL ROTOR DRIVESHAFT COWL & DURING REINSTALLATION OF THE OIL COOLER COWL, FOLLOWING COMPLETION OF THE MX, THE INSTALLATION OF THE CORNER DZUS FASTENER WAS FOUND TO BE VERY DIFFICULT. FURTHER INSPECTION REVEALED THAT THE CORNER ANGLE OF SHEET METAL ON THE AIRFRAME, WITH THE DZUS FASTENER RECEPTACLE, WAS CRACKED & EASILY BROKEN OFF UPON HANDLING. THIS RESULTED IN AN APPROX 2 1/4" SECTION OF THE ANGLE THAT WAS CRACKED & BROKEN OFF.
