Part: STRINGER — CRACKED (LT WING) · airframe 9,591 hrs
DURING ROUTINE ANNUAL INSPECTION, THE FRONT STRINGER, LOWER WING SKIN STIFFENER, WAS FOUND CRACKED UNDER THE LT ENGINE NACELLE USING A FLASHLIGHT & MIRROR VIA THE FRONT WING SECTION ACCESS PANEL JUST INBD OF THE INBD MAIN FUEL TANK. IN ORDER TO FACILITATE A BETTER VIEW & ACCESS, THE LT FUEL TANKS WERE DRAINED & THE INBD TANK WAS DISCONNECTED AND REMOVED. AT THAT TIME, THE SECOND STRINGER WAS ALSO FOUND TO BE CRACKED. THE FRONT STRINGER CRACK WAS APPROXIMATELY 3" OTBD OF RIB AT WS 82.92 & THE SECOND STRINGER WAS CRACKED APPROXIMATELY 1.5" OTBD OF THAT SAME RIB. THERE APPEARED TO BE AN OLD CORROSION PIT OR HOLE APPROXIMATELY .1250" TO .1875" IN DIAMETER IN THE TOP OF THE FRONT STRINGER'S HAT SECTION & THE CRACK RADIATED OUT BOTH SIDES OF THAT HOLE. BOTH CRACKS APPEARED TO BE FULL-SPAN OF THE STRINGER CROSS SECTION WITH COMPLETE SEPARATION.

(ZB0R) DURING PREFLIGHT RUNUP, PILOT FOUND RT PROPELLER CONTROL TO BE FROZEN - WOULD NOT OPERATE. ACFT REPORTED TO MX. REPLACED RT PROPELLER CONTROL, PN 555-443. OPS CK GOOD

IN CRUISE, PILOT ATTEMPTED TO REDUCE POWER FOR MANEUVERS PRACTICE. LT ENGINE WAS UNRESPONSIVE. INSTRUCTOR IDENTIFIED LT ENGINE THROTTLE PROBLEM, ENGINE RPM WAS STABLE AT NEAR FULL POWER. RETURNED TO HOME BASE. PILOT FEATHERED AND SHUTDOWN LT ENGINE ON APPROACH. MAINTENANCE DISCOVERED LT ENGINE THROTTLE CONTROL ROD END AT FUEL CONTROL HAD SEPARATED.

DURING GEAR MAINTENANCE, FOUND NLG ACTUATOR LEAKING. INSPECTION REVEALED WORN SEALS. REPLACED SEALS BUT STILL LEAKED. FURTHER INSPECTION WITH BORESCOPE IN BODY OF ACTUATOR REVEALED LONGITUDINAL SCORING ON INSIDE WALL OF ACTUATOR BODY. REPLACED ACTUATOR TO CORRECT DISCREPANCY.
Part: RETAINER — LOOSE (RT MLG) · airframe 8,190 hrs
ON LANDING ROLL-OUT, RIGHT MLG COLLAPSED AFT AND CAME TO REST AGAINST AFT PORTION OF WHEEL WELL.RETAINING PIN HAD SLIPPED OUT OF TRUNNION, ALLOWING GEAR TO COLLAPSE. THIS GEAR WAS PREVIOUSLY REMOVED FOR SPAR REPAIR. THE RETAINER BOLT WAS EITHER LEFT OUT OR WAS NOT INSTALLED CORRECTLY. OVER A PERIOD OF TIME, THE RETAINER SLID OUT OF THE TRUNNION. RECOMMEND POST MAINTENANCE INSPECTIONS BE MORE COMPLETE. THIS INCIDENT, IF IT HAD HAPPENED 20 SECONDS EARLIER, WOULD HAVE CAUSED MAJOR STRUCTURAL DAMAGE AND POSSIBLY PERSONAL INJURY.
Part: CONTROL CABLE — FRAYED (AP ELEV TRIM)
AFT ELEVATOR TRIM/AUTOPILOT CABLE FRAYED WHERE IT GOES AROUND ELECTRIC TRIM AND AUTOPILOT SERVO PULLEYS.
Part: BRACKET ASSY — CRACKED (MLG)
REFER TO DOCKET NR (93-C3-61-AD). THIS PART IS THE MLG SWIVEL PIN ASSY. PART WAS DISASSEMBLED TO SEE IF IT COULD BE TIGHTENED UP. AFTER DISASSEMBLY AND CLEANING, CORROSION WAS NOTICED ON PIN SHANK AND RADIUS. SMALL CRACKS WERE FOUND WITH 10X MAGNIFYING GLASS. DYE-CHECK BOLT RT AND LT TO VERIFY THE CRACKS. ONE CRACK, .75 INCH AND THE OTHER CRACK, .50 INCH THE WAY AROUND (CRACKED). BOTH ASSYS WERE REPLACED. RECOMMEND TO DISASSEMBLE AND INSPECT EVERY 1,500 TO 2,000 HOURS.
