INTERMITTENT FLASHING OF THE NR 2 TEE HANDLE AND NR 2 FIRE WARNING LIGHT WITH ASSOCIATED TONE DURING FLIGHT PATTERN AT NIGHT. NO SECONDARY INDICATIONS. LANDED AIRCRAFT WITH DISCREPANCY, SHUT AIRCRAFT DOWN, RELEASED AIRCRAFT TO MECHANIC. MAINTENANCE TESTED AND TROUBLESHOT SYSTEM IAW MM AND NO DISCREPANCIES WERE NOTED.
ON EXECUTING A MISSED APPROACH ON ILS, RECEIVED AN INTERMITTENT NR 2 FIRE LIGHT. ABORTED DEPARTURE AND LANDED AT THE BASE RAMP. AFTER SHUT DOWN NO EVIDENCE OF FIRE NOTED. MAINTENANCE PERFORMED TEST OF SYSTEM IAW MM AND CLEANED FIRE DETECTORS CONNECTOR PLUGS. ALL CHECKS FOUND OK. AIRCRAFT RAN UP AND POWER CHECK PERFORMED WITH NO FURTHER INCIDENTS.
TAIL ROTOR BLADE PRIMARY POLYSTRIP WORN THROUGH TO THE FIBERGLASS.
ROTOR BRAKE MOTOR DOES NOT SHUT OFF.
TAIL ROTOR BLADE NICKEL ABRASION STRIP IS UNBONDED ON PADDLE "A". BOTH PRIMARY RUBBER IS ERODED THROUGH AND THE TIP IS ERODED.
ROTOR BRAKE CAUTION LIGHT DOES NOT ILLUMINATE WHEN ROTOR BRAKE IS SWITCHED ON. RETURNED TO VENDOR FOR REPAIR.
TOP SKIN HAS A CRACK SPANWISE APPROXIMATELY 2 INCHES FROM TRAILING EDGE. THE ABRASION STRIP HAS A CRACK WITH SEPARATION. SENT TO COMPOSITE TECHNICS FOR INSPECTION AND REPAIR.
NR 2 NEEDLE STICKS AT 40 PERCENT WHEN DC POWER IS TURNED OFF WITH ENGINES SPOOLING DOWN. TURNED POWER BACK ON TO NR 2, NEEDLE GOES TO ZERO.
ACTUATOR'S NR 1 YAW NEEDLE STAYS RIGHT. INSPECTED BY ACCESSORY OVERHAUL SHOP AND FOUND POTENTIOMETERS ARE OUT OF CALIBRATION.
TAIL ROTOR PADDLE HAS A HOLE AT THE TIP END.
GENERATOR WANTED TO COME ON-LINE DURING START SEQUENCE. INTERMITTENT PROBLEM.
AUDIBLE NOISE HEARD FROM BOX AT DIFFERENT ENGINE LEVER POWER SETTINGS INTERMITTENTLY. CAN HEAR IT ON ALL COMM AND FM CHANNELS THROUGHOUT ALL HEADSETS.

SEPARATION ON THE TRAILING EDGE NEAR THE OUTBOARD END BEYOND THE MAX DEPTH ALLOWED. SENT TO COMPOSITE TECHNICS FOR INSPECTION AND REPAIR.
DEVIATION BAR ABOUT 20 DEGREES OFF FROM SELECTED BEARING.
BLADE IS CRACKED. (REMOVED TO COMPLY WITH 500 HOUR INSPECTION - FOUND SEVERAL CRACKS ON THE TITANIUM STRIP WITH SEPARATION. THE CRACK AT STATION 123" HAS MULTIPLE BRANCHES.
ON/OFF SWITCH VERY LOOSE - INDICATOR GLASS IS LOOSE. PERFORMED PRELIMINARY INSPECTION. TIGHTENED SET SCREWS ON ON/OFF KNOB. REPAIRED. REPLACED FADED AND CRACKED LENS. REPLACED BAD DISPLAY DS101. REPLACED HEADER P201 AND APPLIED HUMISEAL FOR CORROSION CONTROL. CHECKED 28V LIGHTS. BENCH CHECKGOOD.
