FUEL ODOR WHEN CABIN HEAT TURNED ON.
TAIL CONE LT OUTER FRAME CRACKED.
DURING LANDING, ODOR OF SMOKE IN COCKPIT AND THEN CENTER PANEL LIGHTS WENT OUT. INSPECTION FOUND NO DAMAGED WIRES. PANEL LIGHTING DIMMERS WERE INSPECTED AND SHOWED NO DAMAGE. CENTER PANEL INSTRUMENTS WERE PULLED FROM THE PANEL AND INSPECTED. THE LIGHTING SYSTEM WAS OPS CHECKED WITH AN EPU FOR OVER AN HOUR WITHOUT ISSUE. NO EVIDENCE OF ANY COMPONENT THAT WOULD ACCOUNT FOR THE ODOR ASSOCIATED WITH THE PANEL LIGHTING WAS FOUND OTHER THAN THE SEARCH LIGHT WAS FOUND TO HAVE A BURNED OUT LAMP.
TRANSMISSION CHIP CAUTION ILLUMINATED 100' FROM BASE HELIPAD DURING APPROACH FOR LANDING. ACTIVATED CHIP PULSE SEVERAL TIMES AFTER LANDING AND DURING SHUTDOWN. THE TRANSMISSION CHIP CAUTION REMAINED ILLUMINATED, COMPLETED SHUTDOWN.
ON LANDING, ACFT PITCHED UP AND DOWN WITH NO PILOT CONTROL MOVEMENT AND CSAS PITCH LIGHT CAME ON. TROUBLESHOOTING AND INSP FOUND WIRE TERMINAL AT TB811 BROKEN. TB 811 PROVIDES AC GROUND FOR 3 AXIS P/R CSAS. WIRE BREAK WAS SUCH THAT INTERMITTENT CONTACT COULD OCCUR. UNCOMMANDED PITCH WAS DUPLICATED AND WOULD STOP WHEN WIRE CONTACT WAS MADE.
ON SECOND LEG OF PATIENT FLIGHT, THE NR 2 HYD WARNING LIGHT ILLUMINATED ALONG WITH PRESSURE GAUGE INDICATION FLUCTUATION BETWEEN 0 AND 40 BAR. INSPECTED DUAL HYD PACK, HYD PUMP, CEILING CONNECTOR PLUG, TRANSDUCER CONNECTOR PLUG. NO DISCREPANCIES NOTED. SOME MINOR HYD SEEPAGE WITHIN LIMITS NOTED. ACFT WAS RUN UP TO TRY AND DUPLICATE DISCREPANCY. COULD NOT DUPLICATE. TEST FLIGHT COMPLETED, COULD NOT DUPLICATE DISCREPANCY. ACFT RETURNED TO SERVICE.
5 MIN AFTER TAKEOFF, BASE CALLED AND SAID OUTERLINK WAS NOT TRACKING . THEN 15 MIN AFTER THE CALL FROM BASE, IT WAS NOTED, A SMELL OF SMOKE IN THE COCKPIT. STANDBY DG CIRCUIT BREAKER POPPED. PERFORMED PRECAUTIONARY LANDING. INSPECTION SHOWED THE STANDBY GYRO MELTED AT THE BACK. TIME SINCE REPAIR/OH UNKNOWN.
T/R GEARBOX CASTING CRACKED, DISCOVERED DURING ROUTINE ZONE INSPECTION. T/R GBX FITTING HAD BEEN REPLACED FOR A SIMILAR CRACK 832 HRS PRIOR.
FELT CYCLIC POP IN VERTICAL DIRECTION AT THE BOTTOM OF A SIMULATED SINGLE ENGINE LANDING FROM AN ABORTED MAXIMUM PERFORMANCE TAKEOFF PROFILE. AFTER LANDING, WAS ABLE TO REMOVE COPILOT CYCLIC FROM MOUNT WITH SLIGHTEST OF EFFORT.
DURING FLIGHT, YAW CSAS CAUTION LIGHT ILLUMINATED. DIVERTED LANDING FROM HELIPAD TO AIRPORT 4 MILES AWAY. COMPLETED APPROPRIATE EMERGENCY PROCEDURE IAW AFM.
