AIRCRAFT DEPARTED LRD AND AT 800' THE RH ENGINE LOST POWER, A RESTART ATTEMPT WAS UNSUCCESSFUL AND THE TOWER WAS INFORMED OF THE SITUATION AND THE AIRCRAFT RETURNED TO LRD FOR AN UNEVENTFUL LANDING. POST FLIGHT INSPECTION REVEALED THE RH ENGINE COMPRESSOR AND FAN HAD FOREIGN OBJECT DAMAGE CAUSING THE LOSS OF POWER.
AFTER DEPARTING LIT ICING CONDITIONS DEVELOPED AT FL270 THE ENGINE ANTI-ICE WAS ACTIVATED FOLLOWED BY THE AIRFRAME ANTI ICE SYSTEM, SHORTLY AFTER THAT THE NR 1 AND NR 2 ENGINES STARTING LOSING RPM. THE CREW SWITCHED TO AIR START AND AT FL255-250 BOTH ENGINES RESTARTED. THE AIRCRAFT WAS DIVERTED TO ATL.
RT GENERATOR TRIP OFF LINE IN FLIGHT, FOLLOWED BY LT GENERATOR. ACFT DIVERTED, LANDING NORMALLY. MX DISCOVERED RT GENERATOR OIL SOAKED AS A RESULT OF THE AXIS C SEAL LEAKING. REPLACED AXIS C SEAL AND RT STARTER GENERATOR. DETERMINED NO FAULT WITH LT GENERATOR SYS. ACFT ELECTRICAL GENERATION SYS GROUND CHECKED NORMAL.
UPON LANDING AT SHV, AIRCRAFT STARTED TO GENTLY VEER LEFT OF CENTERLINE. THE CREW RELEASED THE BRAKES AND TURNED ANTI-SKID OFF. AIRCRAFT CAME TO AN UNEVENTFUL STOP ON THE RUNWAY. INSPECTION REVEALED BOTH LH AND RH MAIN TIRES BLOWN. ANTI-SKID TESTED NORMALLY ON THE BEFORE LANDING CHECK LIST. NO DAMAGE TO AIRCRAFT OCCURED. MAINTENCE REMOVED AND REPLACED ALL FOUR MAIN TIRES AND WHEELS. DISCOVERED ANTI-SKID CONTROL BOX FAILED. REPLACED ANTI-SKID CONTROL BOX. ANTI-SKID FUNCTIONAL TEST GOOD.[CLOSED]
ENROUTE, AT FL280, BATTERY BUS ANNUNCIATOR LIGHT ILLUMINATED. RESET BUS IAW CHECKLIST AND REDUCED ELECTRICAL LOAD. PROBLEM CORRECTED FOR A FEW MINUTES FOLLOWED BY LOSS OF NR 1 GEN LIGHT. ATTEMPTED RESET OF NR 1 GEN UNSUCCESSFUL. NR 1 GEN WAS SHUT OFF AND ELECTRICAL LOAD FURTHER REDUCED. AFTER A FEW MINUTES NR 2 GEN LIGHT ILLUMINATED. CREW CONTACTED AIRPORT AND INFORMED OF MECHANICAL DIFFICULTIES. AIRCRAFT DIVERTED, NO EMERGENCY DECLARED. AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE DISPATCHED FOR REPAIR. FOUND NR 1 AND NR 2 GCU FAILED AND NR 2 GENERATOR FAULTY. REPLACED AND NR 1 AND NR 2 GCU, REPLACED NR 2 STARTER/GENERATOR. GROUND OPS GOOD. (K)
AIRCRAFT DEPARTED GSO, ON CLIMB-OUT CABIN FAILED TO PRESSURIZE. CREW NOTICED A/C VALVE C/B POPPED. AIRCRAFT RETURNED GSO. MORE INFORMATION TO BE ADDED WHEN AVAILABLE.
PILOT REPORTED AFTER DEPARTURE FROM STL GEAR INDICATION RED AFTER SELECTING GEAR UP, RETURN TO STL THREE GREEN AFTER SELECTING GEAR DOWN, WILL UPDATE AFTER MAINTENANCE. AFTER NUMEROUS ATTEMPTED FLIGHTS THE AIRCRAFT WAS RELOCATED VIA SPECIAL FLIGHT PERMIT TO ANOTHER MAINTENANCE FACILITY, R & R LT MLG UPLOCK ASSY AND ADJUSTED STRIKER PLATE, SYSTEM OPS CHECK GOOD, NO FURTHER PROBLEM ON SUBSEQUENT FLIGHTS.
PILOT REPORTED THAT EN ROUTE FROM ATL TO SRB GEAR DOOR LIGHTS REMAINED RED. AIRCRAFT MOVED FROM SRB TO SDF FOR MAINTENANCE ON A SPECIAL FLIGHT PERMIT. RT MAIN GEAR DOOR UPLOCK OUT OF SEQUNCE DUE TO MOISTURE. REPAIRED IAW DA20 M/M CHAP 32, RAN GEAR MULTIPLE TIMES, RETURN TO SERVICE.
PILOT REPORTED UNABLE TO CONTROL PRESSURIZATION AFTER DEPARTURE FROM KYIP, RETURN TO DEPARTURE AIRPORT FOR MAINTENANCE. MAINTENANCE REMOVED AND REPLACED CABIN PRESSURE CONTROLLER OPS CK GOOD AT THIS TIME.
PILOT REPORTED THAT THE AIRCRAFT LOST CABIN PRESSURE AND REQUESTED EMERGENCY DECENT, AFTER DECENT THE CREW NOTED THAT THE DUMP VALVE WAS IN DUMP POSTION, RESET SWITCH TO NORMAL POSTION AND CABIN BEGAN PRESSURIZE. NO MAINTENANCE REQUIRED.(CLOSED)
LANDING GEAR WOULD NOT RETRACT ON DEPARTURE FROM ATL, AIRCRAFT RETURNED TO ATL. WILL UPDATE AFTER MAINTENANCE. MAINTENANCE FOUND RELAY LOOSE. SECURED AND OPS CHECK GEAR SWING GOOD.
PILOT REPORTED AFTER REACHING FL330 -DOOR- UNSAFE ILLUMINATED, DIVERTED TO ADS FOR MAINTENANCE. MAINTENANCE ADJUSTED TOP FWD DOOR LATCH PER MAINTENANCE MANUAL AND OPS CHECK GOOD.
AIRCRAFT DEPARTED, PILOT REPORTED GEAR DOORS DID NOT FULLY CLOSE, DIVERTED FOR MAINTENANCE. MAINTENANCE PERFORMED SEVERAL GEAR SWINGS SYSTEM CHECKED GOOD.
PILOT REPORTED SMOKE COMING FROM CENTER PANEL AREA ABOVE ANNUCIATOR. REQUESTED DECENT TO NEAREST AIRPORT LANDED ATL WITH NO FURTHER PROBLEM. WILL UPDATE THIS SDR AFTER MAINTENANCE. MAINTENACE FOUND WIRE IN PILOTS GLARE SHEILD LT ASSY SHORTED AND BURNED. REPLACED FLOURESCENT TUBES AND CONNECTOR TO CO-PILOTS LIGHT ASSEMBLY.
ACFT DEPARTED ADS, GEAR DOOR RED LIGHTS WOULD NOT GO OUT, RETURN TO ADS. WILL UPDATE AFTER MAINTENANCE. ADJUSTED NOSE GEAR DOOR ACTUATOR ROD END OP'S CK GOOD. RETURNED AIRCRAFT TO SERVICE.
CARGO DOOR LIGHT ILLUMINATED, DESCENT TO 10,000 MSL BLEED AIR OFF DOOR LIGHT WENT OUT. RETURNED TO DEPARTURE AIRPORT. AIRCRAFT FERRY FLIGHT TO ADS FOR MAINTENANCE. FOUND AND STRAIGHTENED TWO LOCALIZED IMPACTS ON THE LOWER DOOR EDGE, REPAIRED IN ACCORDANCE WITH SRM 51-10-11, VISUALLY INSPECTED HINGE AND LOCKS, OPS CK GOOD.
