DURING FLIGHT TO HOSPITAL MEDICAL CREW NOTIFIED THE PILOT THAT THEY SMELLED FUMES FROM AN ELECTRICAL SOURCE IN THE CABIN BUT DID NOT SEE ANY SMOKE. AN OFF-SITE LANDING WAS PERFORMED, AND MAINTENANCE NOTIFIED OF THE DISCREPANCY. PILOT REPORTED NO ODOR OR FUMES DETECTED UP FRONT. ON CALL AMTS INSPECTED CABIN ELECTRICAL OUTLETS, MEDICAL EQUIPMENT POWER CORDS AND AIRFRAME CABIN WIRING WITH NO DEFECTS NOTED. STRONG ODOR EMANATING FROM CABIN AIR CONDITIONING OUTLETS LED TO DISCOVERING THE AFT BLOWER MOTOR TO BE SOURCE. PULLED CB, SECURED AND PLACARDED AIR CONDITIONING SYSTEM AS INOP. AIRCRAFT RETURNED TO BASE.
ABOUT 12 MILES OUT FROM BASE, NOTICED AN ELECTRICAL BURNING ODOR AND SMOKE SLOWLY CREEPING INTO THE FRONT CABIN FROM THE REAR. SHUT OFF ALL ELECTRICAL SYSTEMS AND OPENED THE FRONT 2 WINDOWS FOR VENTILATION. LANDED THE AIRCRAFT .
RIGHT OUTBOARD FLAP, P/N 35165050259, HAS A CRACK APPROXIMATELY .75 INCH LONG ON THE TRAILING EDGE, ON THE UNDERSIDE, 12 INCHES FROM INBOARD END. REPAIRED WITH A DOUBLER. (M)
DURING CRUISE FLIGHT, LT BUS TIE OPEN WARNING FOLLOWED BY LT DC GENERATOR FAIL WARNING. CAPTAIN LOST HEADSET AUDIO WHICH IS POWERED FROM LT GENERATOR BUS. THIS OCCURRENCE CAUSED LOSS OF COMMUNICATION BETWEEN COCKPIT CREW AT A TIME CREW COORDINATION IS CRITICAL. COMMUNICATION IS LOST DUE TO LOSS OF DC BUS POWER. SUBMITTER STATES BUS TIE PROTECT DEVICES TEND TO BE OVERLY SENSITIVE IN OPERATION.
DURING ROUTINE MAINTENANCE, A CRACK WAS FOUND 10 INCH FROM O/B EDGE 3.5 INCH FROM TRAILING EDGE. REPAIRED WITH PATCH 4 INCH BY 5 INCH. (M)
LEFT ENGINE ITT ROSE ABOVE LIMITS MOMENTARILY. WHEN THE ENGINE WAS SEPARATED FROM THE POWER SECTION, DISCOVERED THE CT VANE RING HAD A SECTION COMPLETELY BURNED THROUGH. THE INVESTIGATION IS LOOKING AT THE FUEL NOZZLES AS A POSSIBLE CAUSE. THE FUEL NOZZLES ARE CHANGED OUT AT 480.0 HOURS
DURING SCHEDULED MAINTENANCE, A .875 INCH CRACK WAS FOUND 15 INCHES FROM OUTBOARD EDGE AND 6 INCHES FROM TRAILING EDGE OF RIGHT INBOARD FLAP. STOPPED DRILLED AND INSTALLED DOUBLER.
DURING ROUTINE MAINTENANCE A CRACK 1 INCH WAS FOUND ON TRAILING EDGE SKIN APPROXIMATELY 1 FOOT INBOARD OF END CAP. ON TE FLAP (P/N 3516505259). STOP DRILLED AND INSTALLED DOUBLER. (M)
FLT 5630 - AFTER TAKEOFF LEFT TORQUE AND PROP RPM INDICATION FLUCTUATED. FOUND BETA CABLE LOOSE, TIGHTENED CABLE, RERIGGED LEFT ENGINE. OPS CHECK NORMAL. (M)
ON SCHEDULED MAINTENANCE, A SMALL CRACK INBD END CAP, WAS FOUND 4 INCHES UP FROM T/E ON TOP OF FLAP. REPAIRED WITH PATCH.
