SMOKE COMING OUT OF BACK OF INSTRUMENT PANEL, COULDN'T DUPLICATE. THIS AIRCRAFT HAS BEEN RETURNED TO REVENUE SERVICE, REF #MEC-000964.
BURNING SMELL REPORTED BY THE MEDICAL CREW SITTING ON THE PATIENT COMPARTMENT (BACK COMPARTMENT) JUST BEFORE LANDING AT THE BASE, I74. THE MECHANIC WAS CALLED TO INVESTIGATE/TROUBLESHOOT AND FOUND SOME SCAT TUBING (HEATER HOSE) LOOSE. HE RESECURED THE TUBING, AND THE SMELL WENT AWAY. ALL SUBSEQUENT OPERATIONAL CHECKS (GROUND RUNS AND FLIGHT CHECKS) WERE GOOD.
CORROSION FOUND UNDER THE LT AND RT FWD CREW DOOR (MIDDLE) UPPER HINGE. THIS DAMAGE IS CURRENTLY BEING REVIEWED BY THE MANUFACTURER FOR AN APPROVED REPAIR PROCEDURE. THIS AIRCRAFT REMAINS OOS AT THE TIME OF THIS SUBMISSION.
MULTIPLE CRACKS FOUND ON THE REAR FRAME, STATION 5915 UNDER CARGO DOOR LATCH STOP.
THE PILOT WAS INBOUND TO A NEARBY AIRPORT. ON APPROACH, IT WAS NOTED THAT THERE WERE THREE GREEN DOWN & LOCKED GEAR INDICATIONS. AS THE PILOT INITIATED THE LANDING, THE NLG BEGAN TO FOLD AS HE WAS TOUCHING DOWN. THE NLG YOKE, RT PITOT TUBE, SEARCH LIGHT & LANDING LIGHT ASSEMBLIES ALL WERE DAMAGED AS THEY MADE CONTACT WITH THE GND. THE PILOT IMMEDIATELY PULLED IN TO A HOVER & RECYCLED THE GEAR. THE LANDING WAS COMPLETED WITHOUT FURTHER INCIDENT. THE MECHANIC DETERMINED THE CAUSE TO BE AN INTERMITTENT NOSE WHEEL CENTERING MICRO SWITCH. THIS SWITCH IS CURRENTLY BEING REPLACED & THE AIRFRAME DAMAGE IS BEING INSPECTED WITH GUIDANCE FROM THE MFG. THIS ACFT REMAINS OUT OF SERVICE.
ON FINAL APPROACH, PILOT NOTICED THE COLLECTIVE BECAME STIFF AND THE PEDALS HAD A RATCHET FEEL TO THEM. THE PILOT DECIDED TO ABORT THE APPROACH AND DIVERTED FOR AN EMERGENCY LANDING. THE APPROACH AND LANDING UNEVENTFUL. ON INSPECTION AND TROUBLESHOOTING, FOUND THE LT HYD FILTER DIRTY. THE FILTER REPLACED, AND THE ACFT RETURNED TO SERVICE.
DURING A 600 HOUR AIRFRAME INSPECTION, FOUND THE PILOT'S 25 DEGREE FRAME BROKEN. THIS FRAME WAS REPAIRED ABOVE THE AREA WHERE IT IS BROKEN NOW DURING THE LAST 500 HOUR AIRFRAME INSPECTION. AT AIRCRAFT TOTAL TIME: 3463.7 CYCLES: 18234 ON JUNE 6, 2005.

DURING PRE-FLIGHT INSPECTION FOR A MAINTENANCE INDUCTION CHECK FLIGHT, FOUND PLAY IN THE TAIL ROTOR OUTPUT FLANGE. GROUNDED AIRCRAFT. REMOVED TAIL ROTOR OUTPUT FLANGE NUT FROM MAIN GEARBOX, AND FOUND THE LOCKING TABS BROKEN OFF OF THE NUT.
ENGINE CHIP DETECTOR ILLUMINATED ON GROUND. REAR BEARING ELECTRICAL DETECTOR AND ACCESSORY GEARBOX AND REDUCTION GEARBOX WERE INSPECTED WITH METAL FOUND ON REAR BEARING AND REDUCTION GEARBOX DETECTORS. PARTICULE ORIGINATION UNKNOWN AT THIS TIME. (K)
