FLIGHT 3914, DURING MAINTENANCE INSPECTION THE #2 EMERGENCY LIGHT BATTERY PACK PACK WAS FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE AFFECTED EMERGENCY LIGHT BATTERY PACK, OPERATIONALLY TESTED WITH NO DEFECTS AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED LAV SMOKE WARNING MESSAGE IN FLIGHT. THE AIRCRAFT RETURNED AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND REMOVED AND REPLACED THE #2 ENGINE. OPERATIONAL CHECKS WERE GOOD.
CREW REPORTED A BURNING SMELL AT SEAT 11D SIDEWALL. MX RAN ENGINES WITH CREW AND COULD NOT DUPLICATE FAULT. REMOVED AND REPLACED CABIN RECIRCULATION FAN AND MUFFLER WITH OPERATIONAL CHECK GOOD. A/C WAS APPROVED FOR RETURN TO SERVICE.

DURING C CHECK INSPECTION FOUND SHEAR CLIP AT FRAME 65 CORRODED AT LOWER BEAM AREA. MX REMOVED AND REPLACED SHEAR CLIP AT FRAME 65.
DURING MX INSPECTION, THE FLOOR EMERGENCY EXIT LIGHT STRIP AT SEATS 8A AND 9A WAS FOUND INOPERATIVE. MX REMOVED AND REPLACED THE EMERGENCY LIGHT AND OPS CHECKED GOOD. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING A MX INSPECTION, THE AFT OVERHEAD EMERGENCY EXIT LIGHT WAS INOPERATIVE. MX REMOVED AND REPLACED THE NR 3 EMERGENCY LIGHT CHARGER AND BATTERY, OPS CHECKED GOOD. ACFT WAS APPROVED FOR RETURN TO SERVICE.
BURNT SIDEWALL LIGHT SOCKET AT SEAT 6A. MAINTENANCE REPAIRED SIDEWALL LIGHT HARNESS AT SEAT 5A/6A. OPERATIONAL CHECKS WERE GOOD.
TAM- THE CREW REPORTED THAT THE MAIN CABIN DOOR WOULD NOT OPEN FROM THE OUT SIDE. THE AIRCRAFT WAS FERRIED TO IAH WHERE MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND THE AFT EXTERNAL HANDLE CAM TO HAVE A BROKEN BOLT AND BE OUT OF ALIGNMENT. MAINTENANCE REALIGNED THE CAM AND REMOVED AND REPLACED THE AFFECTED BOLT. OPERATIONAL CHECKS WERE PERFORMED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

PILOT REPORTED A BURNING ODOR ON FLIGHT DECK DURING DESCENT. CONTRACT MX PERFORMED GVI OF COCKPIT, NAV EQUIPMENT, COCKPIT LIGHT ASSY, AVIONICS FAN. OPS CHECK BOTH PACKS, PERFORMED ENGINE RUN, INSPECTED AVIONICS BAY EQUIPMENT COULD NOT DUPLICATE SMELL AND NO DEFECTS NOTED AT THIS TIME.
MCI - DURING 4A INSPECTION THE LEFT HAND OVERWING EMERGENCY LIGHT WAS FOUND TO BE INOPERATIVE. MAINTENANCE R & R THE AFFECTED LIGHT ASSEMBLY, OPERATIONALLY TESTED WITH NO DEFECTS NOTED, AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING ROUTINE MAINTENANCE, THE EMERGENCY EXIT SIGN ON THE CEILING AT ROW 18 WAS FOUND INOPERATIVE. MAINTENANCE INSPECTED AND RESET AND PERFORMED AN OPS CHECK. NO DEFECTS WERE NOTICED.
THE CREW REPORTED THE THIRD AISLE PATH EMERGENCY LIGHT WAS INOPERATIVE. MAINTENANCE INSPECTED AND REPLACED THE AISLE PATH LIGHT. OPS CHECKED GOOD.
THE CREW REPORTED THE OVERHEAD LIGHT AT 1A WAS SMOKING. MX R & R HARNESS, OPERATIONALLY TESTED WITH NO DEFECTS NOTED AND THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
THE CREW REPORTED A PACK 2 FAIL IN FLIGHT WITH A BURNING ODOR. MX INSPECTED AND FOUND SOME BOOTS OFF DUCTS ON NR 2 PACK. THE BOOTS WERE REPLACED ALONG WITH THE CLAMPS. OPS CHECKS WERE GOOD.
IAH - FLIGHT 2603, THE CREW REPORTED THAT THE LEFT HAND BRAKES WERE INTERMITTENT DURING TAXI. THE AIRCRAFT RETURNED TO THE GATE WHERE MAINTENANCE REMOVED AND REPLACED THE BRAKE CONTROL UNIT, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
ORD FLT 5910 THE CREW REPORTED AISLE PATH LIGHTING WOULD NOT TEST. MAINTENANCE INSPECTED AND RECONNECTED AILSE PATH LIGHT STRIP CONNECTOR. OPERATIONAL CHECKS WERE GOOD.
CREW REPORTED SMOKE AND BURNING ODOR IN CABIN & COCKPIT WITH APU FAIL WHILE BOARDING. EMERGENCY EVACUATION AT THE GATE. MAINTENANCE INSPECTED AND FOUND APU INOP AND AUTO SHUT DOWN CAUSING SMOKE IN CABIN. MAINTENANCE REPLACED THE APU AND OPERATIONAL CHECKS WERE GOOD.
FLIGHT 5720 - IAD - ENGINE NR 2 FAIL TO START. SMOKE REPORTED BY RAMP. IAD MX REMOVED AND REPLACED ENGINE 2 STARTER AND PERFORMED OPERATIONAL CHECK SUCCESSFULLY. ACFT WAS APPROVED TO RETURN TO SERVICE.
FLT 5707 THE CREW REPORTED THE CAPTAINS WINDSHIELD ARCHING ON LANDING. MAINTENANCE REPLACED THE CAPTAINS WINDSHIELD AND OPERATIONAL CHECKS WERE GOOD.
TYS A/C WAS IN FLIGHT WHEN THE CAPTAINS WINDSHIELD HEAT BEGAN TO ARC. CREW DIVERTED TO TYS WHERE MAINTENANCE REMOVED AND REPLACED THE WINDSHIELD. WORK ACCOMPLISHED BY E3688 ON WO 2004739. WORK WAS IAW EMB 145 AMM 56-1-00-01. A/C RELEASED TO SERVICE.
DURING A SCHEDULED INSPECTION THE FIRST SECTION OF CABIN FLOOR EMERGENCY LIGHTING WAS FOUND TO BE INOPERATIVE. MAINTENANCE REMOVED AND REPLACED THE NR 2 EMERGENCY LIGHT INVERTER, PERFORMED A SYSTEM TEST AND CLEARED THE DISCREPANCY.
ORD FLT 5678 THE CREW REPORTED A BATT NR 2 OVERTEMP INDICATION IN FLIGHT. THE AIRCRAFT DIVERTED INTO CLE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE NR 2 BATTERY. OPERATIONAL CHECKS WERE GOOD.
ORD FLT 5685 THE CREW REPORTED A LIGHTING STRIKE ON APPROACH INTO ORD WITH POSSIBLE LIMITED ELEVATOR TRAVEL. THE AIRCRAFT LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED AND FOUND THE REINFORCING WALL INSIDE THE TAIL BOOM SEPARATED AND LODGED INTO THE REAR ELEVATOR QUADRANT BELL CRANK. THE TAIL BOOM WAS REPLACED AND THE LIGHTNING STRIKE INSPECTION WAS ACCOMPLISHED. OPERATIONAL CHECKS OF THE FLIGHT CONTROLS WERE GOOD.
CLE FLT NR 153, THE CREW REPORTED A BLEED NR 2 OVERTEMP MESSAGE WHEN THE ANTI-ICE WAS SELECTED ON. MAINTENANCE INSPECTED AND REPLACED THE NR 2 ENGINE FAN AIR VALVE. OPERATIONAL CHECKS WERE GOOD.
IAH - FLIGHT 555, THE CREW REPORTED A LANDING GEAR AIR GROUND FAIL MESSAGE. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT, AND REMOVED AND REPLACED THE CONNECTOR P2834, OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. (A)

MAINTENANCE INSPECTED THE AIRCRAFT, REMOVED AND REPLACED THE NOSE LANDING GEAR WOW PROXIMITY SWITCH. OPERATIONALLY TESTED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. (A)

LEFT FUSELAGE SKIN IS CRACKED AROUND RIVET AT FRAME 77 STR 6L. CUT OUT DAMAGE, INSTALLED FILLER AND DOUBLER. ALODINE PRIMED AND PAINTED IAW EC5330-01676. AIRCRAFT IS STILL IN MOD LINE MAINTENANCE.
CLE FLIGHT 2464 THE CREW REPORTED A VERY LOUD WIND NOISE UNDER NOSE OF AIRCRAFT FOR SEVERAL MINUTES AFTER TAKEOFF. THE CREW SUSPECTED THE NOSE GEAR DOORS DID NOT FULLY CLOSE. NO INDICATION WAS NOTED ON THE EICAS. MAINTENANCE INSPECTED AND REPLACED THE LEFT HAND NOSE GEAR DOOR ACTUATOR. OPERATIONAL CHECKS WERE GOOD.
IAH - FLIGHT 2895, AFTER ROTATION THE CREW REPORTED A LANDING GEAR AIR GROUND FAIL MESSAGE ON EICAS. THE AIRCRAFT RETURNED TO IAH WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE LANDING GEAR ELECTRONICS UNIT (LGEU), OPERATIONALLY CHECKED WITH NO DEFECTS, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV - DURING C-CHECK THE APU FIREWALL WAS FOUND TO BE CHAFED AT THE 6 O'CLOCK POSITION. MAINTENANCE REMOVED THE DAMAGED AREA AND INSTALLED A DOUBLER REPAIR IAW EC5310-01359, REPAIR 1.2, FIG 2.1. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV - DURING C-CHECK THE AFT SERVICE DOOR FRAME WAS FOUND TO BE CORRODED BETWEEN FRAMES 14 AND 15R. MAINTENANCE REMOVED THE DAMAGED AREA AND INSTALLED A DOUBLER REPAIR IAW EC5310-01373. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CLE FLIGHT 2109 - THE CREW REPORTED AN ELECTRICAL SMELL/SMOKE DURING TAXI AFTER THE EICAS DISPLAY WENT BLANK THEN CAME BACK ON. MAINTENANCE REPLACED THE EICAS DISPLAY DU-870. OPERATIONAL CHECKS WERE GOOD.
STL FLIGHT 2162, CREW REPORTED THAT THE LANDING GEAR WOULD NOT RETRACT AFTER TAKE-OFF. RETURNED TO STL AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED SYSTEM, AND CYCLED LANDING GEAR SEVERAL TIMES. ALL OPERATIONAL CHECKS WERE GOOD. AIRCRAFT RETURNED TO SERVICE
CLE FLIGHT 3181, CREW REPORTED THAT THE LANDING GEAR DID NOT RETRACT AFTER TAKEOFF. MAINTENANCE INSPECTED SYSTEM AND REMOVED AND REPLACED THE LANDING GEAR ELECTRONIC UNIT. ALL OPERATIONAL CHECKS WERE GOOD. PERFORMED TEST FLIGHT, AND TEST FLIGHT WAS GOOD. AIRCRAFT RETURNED TO SERVICE.
SHV-DURING C CHECK THE APU FIREWALL WAS FOUND TO BE CRACKED AT THE LEFT UPPER SUPPORT. MAINTENANCE REMOVED THE DAMAGED AREA AND PERFORMED A DOUBLER REPAIR IAW EC 5330-01267. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV-DURING C CHECK THE LEFT OMEGA BEAM WAS FOUND TO BE CORRODED AT THE COMPOSITE FIBER CROSS BEAM AT FRAME 20. MAINTENANCE REMOVED THE DAMAGED AREA AND PERFORMED A DOUBLER REPAIR IAW EMB 145 SRM 53-21-10, PAGE 213, FIGURE 201, SHEET 3. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
CREW REPORT, LANDING GEAR FAILED TO RETRACT. MAINTENANCE REMOVED AND REPLACED LANDING GEAR LEVER. JACKED AIRCRAFT AND CYCLED GEAR SEVERAL TIMES. OPS CHECKED GOOD IAW EMB145 MM 32-32-02 AND 32-30-00.
CREW REPORTED, MELTING PLASTIC ODOR FOLLOWED BY BLEED NR 2 OVERHEAT WHILE IN ICING CONDITIONS. MAINTENANCE PERFORMED FIM TASK 36-11-00-810-803A, REMOVED AND REPLACED FAN AIR VALVE FILTER IAW EMB145 MM 36-11-03. PERFORMED OPS CHECK IAW EC3610-01062 STEPS 2.4 THROUGH 2.12. NO BURNING SMELL NOTED. AIRCRAFT RETURNED TO SERVICE.
THE CREW NOTED THAT THE LANDING GEAR WOULD NOT RAISE. THE PILOT BUMPED THE HANDLE AND THE GEAR CAME UP. IAH- MAINTENANCE REMOVED AND REPLACED THE LANDING GEAR CONTROL LEVER, PERFORMED A OPERATIONAL CHECK, NO DEFECTS NOTED. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
TYS FLIGHT 2466 WAS CLIMBING AFTER TAKEOFF WHEN THE AUTO-MANUAL PRESSURE CONTROLLER WAS GIVING FLUCTATIONS AND THE CONTROLLER WOULD'NT ADJUST. THE PILOT BROUGHT THE A/C BACK TO TYS. MAINTENANCE REMOVED AND REPLACED THE DIGITAL CONTROLLER IAW EMB 145 MM 21-31-01 AND OPS CHECK WAS GOOD IAW EMB 145 MM 21-31-00. A/C RELEASED TO SERVICE.
