WAS IN CRUISE FROM MKG TO IND FOR ABOUT 5 MINUTES. BEGAN TO SMELL ODOR. GOT SLIGHTLY STRONGER WITHIN ABOUT 2 MINUTES. CHOSE TO DIVERT TO SBN. ON APPROACH/FINAL RADIOS CUT IN AND OUT.
DURING FLINAL APPROACH, ALL THREE BUSS TIE ANNUCIATOR LIGHTS ILLUMINATED INDICATING A FAILURE OF THE CENTER BUS. I VERIFIED THE STATUS OF THE ELECTRICAL SYSTEM AND CONFIRMED NO VOLTAGE ON THE CENTER BUS. I REFERRED TO THE QRH DUE TO THE LOSS OF THE CENTER BUSS, THE GEAR WOULD NOT EXTEND NORMALLY. DECLARED AN EMERGENCY AND SUCCESSFULLY PUMPED THE GEAR DOWN MANUALLY. I GOT 3 GREEN BUT OPTED TO DO A LOW APPROACH TO HAVE THE TOWER VERIFY THE GEAR WAS DOWN. ENTERED THE TRAFFIC PATTERN AND LANDED UNEVENTFULLY. TAXIED TO PARKING TO OFFLOAD. I DEBRIEFED WITH ARFF AT SDF, COMPANY MOC. PILOT DID COMPLETE NORMAL SYSTEM CHECKS ON THE FIRST LEG TO LRD WITHOUT ANY ISSUES. THOSE CHECKS INCLUDED CHECKING THE BUS TIES AND BUS SENSE. ALL CHECKS WERE SATISFACTORY.
AFTER DESCENDING AND LEVELING OFF LEFT ENGINE TORQUE NOT RISING PROPERLY AND INCORRECT ENGINE CONTROL / FEEL, PILOT NOTICED IMPROPER POWER OUT OF LEFT ENGINE AND DECLARED EMERGENCY DUE TO PARITIAL POWER ON LEFT ENGINE. PILOT WAS CLEARD TO LAND. LANDED SAFELY AND HOD NO OTHER ISSUES.
PILOTS REPORTED THAT WHILE TAXING OFF THE RUNWAY, WHEN RIGHT RUDDER PRESSURE WAS APPLIED, THE CAPTAIN'S RIGHT RUDDER PEDAL ASSEMBLY SHEARED OFF, FLOPS LOOSE ON THE ASSEMBLY. R & R THE CAPTAIN'S RT RUDDER PEDAL AND ARM ASSEMBLY IAW MM 27-20-03-201. OPS CHECKED AND RIGGING CHECKED GOOD.
PILOT REPORTED THAT DURING GEAR RETRACTION, LG HANDLE LIGHT REMAINED ON & LG HANDLE CB POPPED. WENT THROUGH CHECKLIST & MANUALLY EXTENDED GEAR, GOT THREE GREENS, LANDED UNEVENTFULLY. MAINTENANCE PERFORMED OPS CKS ON THE LANDING GEAR SYSTEM IAW MI 32.01, COULD NOT DUPLICATE ON THE GROUND AT THIS TIME, SYS OK FOR SERVICE.
DURING A SCHEDULED INSPECTION, FOUND DAMAGE ON THE BOTTOM SIDE OF THE WING TIP, APPROXIMENTLY 3 INCHES FROM WING TIP, DAMAGE DOES NOT APPEAR TO PENATRATE HONEYCOMB. REPLACED THE LT WING TIP ASSY IAW 57-30-00. MAJOR REPAIR.
DURING A SCHEDULED INSP FOUND A CRACK IN THE FUSELAGE FRAME DOUBLER AT BS 177.35 AND STRINGER 13L 1.2 INCHES FROM OTBD EDGE RADIATING INBD, CRACK IS IN THE FLANGE BEND RADIUS OF THE DOUBLER. REPAIRED IAW EO 13-1900-53-064. MAJOR REPAIR.
DURING A SCHEDULED INSPECTION, FOUND A CHAFE IN THE LT WING TO FUSELAGE BULKHEAD AT STRINGER 12L BETWEEN BS 258.25 - 273.25, AREA OF BULKHEAD THAT MEETS AT BELLY SKIN EDGE. REPAIRED IAW EO 13-1900-53-066. MAJOR REPAIR.
DURING A SCHEDULED INSPECTION, FOUND A CRACK IN THE FUSELAGE FRAME AT BS 177.35 AND STRINGER 13L AT THE FRAME FLANGE BEND RADIUS 1.2 INCHES FROM THE OTBD EDGE RADIATING INBD. REPAIRED IAW EO 13-1900-53-065. MAJOR REPAIR.

DURING A SCHEDULED INSPECTION, MX FOUND THE CARGO DOOR FWD FRAME AT FS 456 CRACKED AT THE UPPER INBD GAS STRUT MOUNT ATTACHING NUT PLATE. THE CRACKS RADIATE UPWARD AND INBD FROM THE NUT PLATE. REPAIRED IAW EO 13-1900-53-074. MAJOR REPAIR.
DURING A SCHEDULED INSPECTION, MX FOUND THAT THE AIRSTAIR DOOR LOWER AFT FRAME CORNER AREA AT APPROXIMATELY FS 172.35 WAS CRACKED, 1.2" IN THE OTBD UPPER BEND RADIUS. REPAIRED IAW EO 13-1900-53-061. MAJOR REPAIR.

DURING A SCHEDULED INSPECTION, FOUND BELLY SKIN AND FASTENER DAMAGED AT FS 468.25 BETWEEN STRINGERS 13 - 14L. REPAIRED IAW EO 13-1900-53-058.
PILOT REPORTED THAT WHEN SELECTING THE GEAR DOWN, HAD NO LT MAIN INDICATION AND GEAR IN TRANSIT LIGHT REMAINED ILLUMINATED, LANDING GEAR RELAY CIRCUIT BREAKER WAS TRIPPED, RAN CHECKLIST, MANUALLY PUMPED GEAR DOWN UNTILL HANDLE WAS FIRM, PROCEDED TO LAND, LT GEAR FAILED AT 80KTS, FLAPS SELECTED TO 35. R & R NOSE GEAR ACTUATOR IAW D1900MM 32-30-14 AND STEPS 6 AND 20. LEAK AND OPS CHECKED GOOD.
PILOT REPORTED THAT WHEN THE LANDING GEAR HANDLE WAS SELECTED TO THE UP POSITION, THE RT MAIN DID NOT FULLY RETRACT, LANDING CIRCUIT BREAKER TRIPPED AND HAD TO PUMP GEAR DOWN BEFORE LANDING. MX REPAIRED WIRE G10A22, OPS CK GOOD.
LANDING GEAR FAILED TO EXTEND NORMALY. LANDING GEAR LIGHTS DID NOT ILLUMINATE. MX INSPECTED ALL SYS WIRING AND REPLACED THE LANDING CONTROL HANDLE, PERFORMED SEVERAL GEAR SWINGS, OPS CHECKED GOOD.
EMERGENCY INSTRUMENT LIGHT ON F/O'S SIDE INOPERATIVE. MX REPLACED THE BULB, OPS CK GOOD.
DURING A SCHEDULED ROUTINE INSPECTION, FOUND 4 CRACKS IN THE LT LWR FWD PRESSURE BULKHEAD AT THE INBD EDGE DOUBLER ROW OF FASTENERS, 1.1" IN LENGTH. REPAIRED IAW EO 12-1900-53-038. MAJOR REPAIR.
PILOT REPORTED THAT THE LANDING GEAR INTRANSIT LIGHT ILLUMINATED IN FLIGHT WITH TORQUE AT 3000LB AND AIRSPEED 210KNOTS SLOWED TO 170 AND PUT GEAR DOWN NORMAL APPROPRIATED CHECKK LIST RAN FOR UNSAFE GEAR EVEN WITH ALL INDICATIONS OF GEAR DOWN EXCEPT FOR HANDLE LIGHT. MAINTENANCE R & R LT DOWNLOCK SWITCH, OPS CHECKED GOOD.
PILOT REPORTED THAT THE LT AND RT LANDING GEAR INDICATION IS INTERMITTENT. MX REPLACED THE ANNUNCIATOR ASSY PN: 90-42291-2, GEAR SWING OPS CKS GOOD, NO DEFECTS NOTED.
PILOT REPORTED THAT THE AIR STAIR DOOR WAS INOP. MX REPLACED THE FUSE IN THE BATTERY BOX PNMDL5, IAW BMM 33-20-00, OPS CHECKED GOOD.
PILOT REPORTED THAT THE CABIN DOOR ANNUNCIATOR WAS INOPERATIVE. MX FOUND CORROSION ON THE WIRE PINS, REPLACED A401 P1, P2, P3, & P4, SP11 12,13,114,15, & 16 IAW BMM 20-13-00, OPS CK GOOD.
PILOT REPORTED THAT DURING THERE POST FLIGHT INSP THEY FOUND BOTH INSTRUMENT EMERGENCY LIGHTS ON THE CAPTAIN'S SIDE ARE OUT. MAINTENANCE INSPECTED THE SYSTEM AND REPLACED THE EMERGENCY LIGHTS BULBS, IAW BMM 33-10-04 PARAGRAPH 1, OPS CHECKED GOOD.
RIGHT ENGINE TORQUE GAUGE WENT DOWN ON CLIMB OUT. ALL OTHER ENGINE INDICATIONS WERE NORMAL,NOTICED ON TAXI IN TORQUE GAUGE WAS BELOW ZERO. MAINTENANCE PERFORMED PARAMETER RUNUP OF LT & RT ENGINES. ALL CHECKS GOOD COULD NOT DUPLICATE DISCREPANCY.
MAJOR REPAIR - DURING A SCHEDULED ROUNTINE INSP, MX FOUND A CRACK IN THE FRAME AT FS 280.73 AT STRINGER 5R CUTOUT, .2" IN LENGTH. MAINTENANC PERFORMED ANOTHER INSPECTION ON THE AREA AND FOUND THE THE CRACK WAS A PEN MARK FROM ORIGINAL MANUFACTURE.
MAJOR REPAIR - DURING A SCHEDULED ROUNTINE INSP, MX FOUND 2 CRACKS AT THE FRAME FS 456.0 RADIATING FROM STRINGER 9AR CUTOUT, UPPER CRACK .6" IN LENGTH, LOWER CRACK .35" IN LENGTH. MAINTENANCE REPAIRED THE AFFECTED AREA AND PERFORMED ALL REQUIRED RII INSPECTIONS I.A.W. EO 11-1900-53-006. MATERIALS UTILIZED ARE INCLUDED IN THE ENGINEERING ORDER.

WHEN LANDING GEAR HANDLE WAS PLACED IN DOWN POSITION WE SAW 3 GREEN, HANDLE INTRANSIT LIGHTS, LG RELAY CB POPPED AND J HOOK CAME DOWN INT LOCKED POSITION OR LESS SIMULTANEOUSLY. MAINTENANCE TROUBLESHOT AND FOUND RIGHT LANDING GEAR ACTUATOR LAGGING UP AND DOWN, REPLACED RIGHT LANDING GEAR ACTUATOR IAW BMM 32-30-01, OPERATIONAL CHECKS GOOD, PN OFF 114-380041-15 SN OFF 05614 SN ON 081218029.
LANDING GEAR WOULD NOT COME DOWN PERFORMED CHECKLIST AND HAND PUMPED THE GEAR ALL 3 GREEN ANNUNCIATORS ILLUMINATED. MX REMOVED AND REPLACED THE LANDING GEAR CONTROL HANDLE IAW MM 31-10-00-201, SECTION 5 AND THE OPERATIONAL CHECK WAS GOOD IAW MM 32-30-00.
LANDING GEAR WOULD NOT RETRACT AFTER TAKEOFF. MX JACKED THE ACFT AND PERFORMED A GEAR RETRACTION AND EXTENSION TEST IAW MM 32-30-00. THE OPS CHECK WAS GOOD ON THE GROUND. SUBSEQUENT OPS CHECK FLIGHT WAS UNSATISFACTORY. MX REMOVED AND REPLACED THE MLG SQUAT SWITCH, DOWNLOCK SWITCH, AND HARNESS ASSY IAW MM 32-60-05. RII INSP COMPLIED WITH IAW. TASKCARD 4.3.8. FOUND TO BE SATISFACTORY. OPS CHECK FLIGHT WAS PERFORMED AND WAS SATISFACTORY.
SUSPECTED AUTOFEATHER - PILOTS REPORTED THE RT PROP AUTOFEATHERED IN FLIGHT WHILE REDUCING TORQUE FROM 2500 LBS TO 500 LBS. TOR4QUE LEVELS WERE EVEN THROUGHOUT PWR REDUCTION. MX ADJUSTED THE FLIGHT IDLE TORQUES ON THE RT AND LT ENG IAW MM 76-10-00. OPS CHECKS OF THE AUTOFEATHER SYS WERE PERFORMED IAW MM 61-21-00. ALL CHECKS WERE GOOD. RII INSPECTIONS C/W IAW TASKCARD 4.3.13.
MAJOR REPAIR - DURING ROUTINE SHEDULED MAINTENANCE, A CRACK WAS FOUND ON THE LT HEAVY ANGLE AT FS 288. CRACK IS LOCATED AT THE BEND RADIUS APPROXIMATELY 1.6 IN LENGTH. REPAIRED CRACK IN LT HEAVY DOUBLER AT FS 288 IAW EO 09-1900-53-022. RII INSP COMPLIED IAW EO 09-1900-53-022. MATERIALS UTILIZED ARE LISTED ON THE EO.
PILOTS REPORTED THAT THE MLG DID NOT RETRACT AFTER TAKEOFF AND IN TRANSIT LIGHTS DID NOT COME ON AND 3 GREEN STILL ANNUNCIATED. PILOTS CYCLED MLG HANDLE WITH THE SAME RESULT. PERFORMED A MANUAL MLG EXTENSION CHECK LIST AT THE DIRECTION OF MX CONTROL. MLG MOTOR COULD NOT BE HEARD. MX INSPECTED WIRE HARNESS FOR MLG FROM LT WING ENTRY INTO FUSELAGE AND TO FLOOR CIRCUIT BREAKER PANEL AND PRINTED CIRCUIT BOARDS. NO DISCREPANCIES WERE NOTED. MX RESECURED HARNESS AND ADJUSTED THE LT SQUAT SWITCH IAW MM 32-60-05. MX ALSO ADJUSTED LT UPLOCK SWITCH IAW MM 32-60-00. MX CYCLED MLG THROUGH EXTENSION AND RETRACTION IAW MM 32-30-00 OPERATIONAL CHECKS WERE GOOD.

AIR RETURN PILOTS REPORTED THAT THE LANDING GEAR WOULD NOT RETRACT AFTER TAKEOFF. MAINTENANCE JACKED AIRCRAFT ON RAMP AND PERFORMED 6 GEAR RETRACTION AND EXTENSION CYCLES WITH NO FAILURE. CHECKED SQUAT SWITCH CONNECTORS FOR SECURITY, FOUND OK. PERFORMED 1 MANUAL EXTENTION AND 2 MORE. REFERENCE MM 32-20-00 OPERATIONAL CHECKS WERE GOOD.

AIRSTAIR DOOR FRAME ANGLE CRACKED AT LOWER SNUBBER MOUNT NUTPLATES. MAINT INSTALLED INSERT AND REPAIR PIECE CUT FROM NEW ANGLE IAW. SRM 53-90-28 AND MFG NEW COVER PLATE. REPAIR AREA APPROXIMATELY 7.7500 INCH BY 8.7500 INCH. RII INSPECTION COMPLIED IAW TASKCARD 8.2.1.
BELLY SKIN CRACKED AT WING SPAR FS 290.5 AT BL 29.06 CRACK LENGTH APPROXIMATELY 1.250 INCHES. MAINTENANCE REPAIRED SKIN CRACK AT LT WING SPAR AREA FS 290 AND BL 29.06 IAW SRM 53-90. REPAIR DIMENSION 7 INCH BY 5.8 INCHES. 2024 T3, .040 INCH. RII INSPECTION COMPLIED IAW TASKCARD 8.2.1.
RIGHT WING AT BL 114.25 AND F.S. 330 FOUND A .094 CRACK ON AFT SPAR, REPAIR PENDING.
UPON APPROACH WHEN LANDING GEAR SELECTED DOWN, GEAR WOULD NOT EXTEND. FLIGHT CREW USED THE MANUAL GEAR DOWN HAND PUMP TO EXTEND GEAR SUCCESSFULLY AND LANDED. MAINTENANCE DISCOVERED HYDRAULIC POWER PACK CIRCUIT BREAKER WAS TRIPPED. RESET CIRCUIT BREAKER AND PERFORMED GEAR SWING AND COULD NOT DUPLICATE. AIRCRAFT RETURNED TO SERVICE.
