Operated by GREAT LAKES AVIATION LTD (per NTSB report)
Failure of the left main landing gear actuator internal lock to engage in the locked position for undetermined reasons.
Operated by Great Lakes Aviation (per NTSB report)
A failure of the nose landing gear end cap due to fatigue. Contributing to the failure were the ineffective inspection and the unknown effect of grain direction on fatigue life.
Operated by Great Lakes Airlines (per NTSB report)
A failure of the nose landing gear end cap due to fatigue. Contributing to the failure were the ineffective inspection and the unknown effect of grain direction on fatigue life.
Operated by GREAT LAKES AVIATION LTD (per NTSB report)
A failure of the nose landing gear end cap due to fatigue. Contributing to the failure were the ineffective inspection and the unknown effect of grain direction on fatigue life.
Operated by Great Lakes Aviation (per NTSB report)
The fatigue failure of the nose landing gear (NLG) end cap, which resulted in insufficient hydraulic pressure to secure the left main landing gear into the down and locked position. Contributing to the fatigue failure was the NLG end cap’s non-optimum grain direction and the inadequate inspection procedure performed during overhaul.
Operated by Great Lakes Aviation, Ltd. (per NTSB report)
The pre-existent damage of the right-hand landing gear actuator attachment lug which led to fatigue cracking and eventual overload failure resulting in a collapsed gear upon landing.
Operated by Great Lakes Aviation Inc. (per NTSB report)
The pilot’s failure to establish proper rotation and liftoff speed during the takeoff roll.
Operated by Great Lakes Aviation (per NTSB report)
The flight crew’s unprofessional behavior and deviation from standard operating procedures which resulted in an in-flight emergency.
Operated by GREAT LAKES AVIATION LTD (per NTSB report)
arcing of the windshield deicing system. Factors contributing to the incident were the improper inspection of the windshield's exterior seal, deterioration of the windshield seal, and water penetration through the deteriorated seal.
Operated by GREAT LAKES AVIATION, LTD. (per NTSB report)
Debonding of the propeller erosion shield due to an improper overhaul and repair by other maintenance personnel.
Operated by GREAT LAKES AVIATION, LTD. (per NTSB report)
the failure of the pilots in the King Air A90 to effectively monitor the common traffic advisory frequency or to properly scan for traffic, resulting in their commencing a takeoff roll when the Beech 1900C (United Express flight 5925) was landing on an intersecting runway. Contributing to the cause of the accident was the Cherokee pilot's interrupted radio transmission, which led to the Beech 1900C pilot's misunderstanding of the transmission as an indication from the King Air that it would not take off until after flight 5925 had cleared the runway. Contributing to the severity of the accident and the loss of life were the lack of adequate aircraft rescue and firefighting services and the failure of the air stair door on the Beech 1900C to be opened. (NTSB Report AAR-97/04)
Operated by GREAT LAKES AVIATION LTD (per NTSB report)
The operator's inadequate progressive inspection program that did not require periodic analysis of airplane hydraulic system fluid; and the failure of the operator to comply with the airplane manufacturer's maintenance manual resulting in contamination of the airplane's hydraulic system that exceeded the maximum contamination level.
Operated by GREAT LAKES AVIATION, INC. (per NTSB report)
the pilot-in-command's failure to maintain clearance from obstacles during taxi operations. Related factors are the dark night light conditions and the wrong taxi route.
