NR 3 ENG FLAMEOUT. WHEN THE AGB COVER WAS REMOVED, THE TOWERSHAFT & TOWERSHAFT RETAINING RING FELL OUT. IT WAS CONCLUDED THAT THE TOWERSHAFT RETAINING RING WAS ONLY PARTIALLY ENGAGED IN THE TOWERSHAFT'S MATING GROOVE AT INSTALLATION. IN TIME, THIS ALLOWED THE TOWERSHAFT TO SLIP DOWNWARD. THIS RESULTED IN ITS LOWER TEETH DISENGAGING FROM THE AGB GEARS. THEREFORE, ALTHOUGH THE TOWERSHAFT WAS STILL SPINNING, NO TORQUE WAS BEING TRANSMITTED TO THE MGB. JICS HAVE BEEN REVISED TO ALERT MECHANICS & LEADS TO THIS POSSIBILITY.

ENGINE NUMBER TWO OIL QUANT ZERO, OIL PRESSURE DECREASING. SHUT DOWN NR 2WITH 73 PSI. WINDMILL OIL PRESS 15-16 PSI. NR 2 ENGINE IN FLIGHT SHUT DOWN DUE TO LOSS OF OIL PRESSURE AND OIL QTY. ENGINE WILL REQUIRE REMOVAL. OIL LOSS WAS CAUSED BY A CRACKED NUMBER 4 BEARING OIL TUBE. SUBSEQUENT BORESCOPE REVEALED 1 HPT T2 BLADE FAILED AT MID SPAN. RESULTANT VIBRATION CAUSED THE OIL LINE FRACTURE.
