THIS RESULTED IN ITS LOWER TEETH DISENGAGING FROM THE AGB GEARS. THEREFORE, ALTHOUGH THE TOWERSHAFT WAS STILL SPINNING, NO TORQUE WAS BEING TRANSMITTED TO THE MGB. INVESTIGATED & FOUND LOOSE WINDOW HEAT CONTROL THERMOSTAT WIRING & "WINDOW HEAT 2L" MESSAGE ON EICAS. LOOSE WIRING MAY HAVE CAUSED ARCING & ODOR. THE LOOSE WIRE WAS REATTACHED TO COCKPIT WINDOW. "WINDOW HEAT 2L" MESSAGE WAS GONE, AND A/C RETURNED TO REVENUE SERVICE WITHOUT FURTHER ODOR OR WINDOW HEAT PROBLEM.

ROOT CAUSE OF THE FAILURE WAS THE 6TH STAGE BLADE DETACHMENT THIS IS AN INDUSTRY ISSUE WITH THE FRONT END DURABILITY BEING WORKED BY THE OEM.

THE FLANGE WAS TRIMMED OUT AS MUCH AS POSSIBLE. ACP HFEC NDT INSP OF AREA FOR CRACKS, NONE FOUND. THIS REPAIR APPROVED BY BOEING-FAA 8110-3 ON MOBILE AEROSPACE ENGINEERING AUTHORIZATION 47-5325-13-A1038.

MAJOR REPAIR ACCOMPLISHED TO FLOORBEAM WEB PER SRM 51-70-13-001 DOUBLER INSTALLED AFT SIDE OF CABIN FLOORBEAM BS 880 RBL 9 WL 199 HEIGHT OF BEAM APPROX 7 INCHES WIDE. *S/D* DAMAGE CAUSED AT CURRENT MAINTENANCE CHECK BY ACCIDENT WHEN PERFORMING WORK IN VICINITY. REPAIRED PER SRM 51-70-13-001.
ELECTRIC BURN SMELL IN COCKPIT. TURNED OFF LOWER RECIRCULATING FAN PER MEL. SMELL WENT AWAY. *S/D* REPLACED PILOTS SHOULDER HEATER (P/N: 2831-2). NO PROBLEMS AFTER, PROBABLE CAUSE WAS INTERNAL FAILURE.
ENGINE NUMBER TWO OIL QUANT ZERO, OIL PRESSURE DECREASING. SHUT DOWN NR 2WITH 73 PSI. WINDMILL OIL PRESS 15-16 PSI. NR 2 ENGINE IN FLIGHT SHUT DOWN DUE TO LOSS OF OIL PRESSURE AND OIL QTY. ENGINE WILL REQUIRE REMOVAL. OIL LOSS WAS CAUSED BY A CRACKED NUMBER 4 BEARING OIL TUBE. SUBSEQUENT BORESCOPE REVEALED 1 HPT T2 BLADE FAILED AT MID SPAN. RESULTANT VIBRATION CAUSED THE OIL LINE FRACTURE.

EMERGENCY EXIT LIGHTS INTERMITTENT ROWS 51 TO 59 LEFT HAND SIDE E-ZONE. *S/D* DEFECTIVE BATTERY PACK WAS REMOVED AND REPLACED.
ELECTRICAL SMELL AND SMOKE IN CABIN CONCENTRATED BETWEEN GALLEY 402 AND LAV 111-112. LEFT AND RIGHT UTILITY BUSES TRIPPED. SMOKE REMOVAL PROCEDURE, CLEARED CABIN. *S/D* UPPER NR 2 RECIRCULATION FAN BEARING FAILURE. REPLACED FAN.
UNWANTED FUEL TRANSFER. *S/D* THE SHAFTS TO BOTH JETTISON TRANSFER VALVES IN THE CENTER TANK WERE BROKEN. DUE TO PRESSURE IN THE MANIFOLD, THE VALVE BODY WAS FREE TO FLOP OPEN AND CAUSED THE FUEL IMBALANCE PROBLEM. REPLACED TRANSFER VALVES, OPERATION CHECKS NORMAL.
FLOOR EMERGENCY EXIT LIGHTS INOP IN RIGHT HAND FORWARD E-ZONE IN ADDITION TO CEILING LIGHT AND FLOOR LIGHTS IN D-ZONE. BATTERY PACKS LOW, REPLACED BATTERY PACKS.
AIRCRAFT EXPERIENCED INTERMITTENT -FLAPS PRIMARY+ AND -FLAPS DRIVE+ MESSAGES IN-FLIGHT SOON AFTER TAKE-OFF. FLIGHT CREW ELECTED TO DUMP FUEL AND RETURN TO FIELD. THE L1 LEADING EDGE FLAP DRIVE UNIT POSITION SWITCH ASSEMBLY WAS REPLACED WITH AN IMPROVED P/N SWITCH. THE OLD SWITCH HAS BEEN KNOWN TO CAUSE NUISANCE EICAS MESSAGES.
FLOOR EEL INOP IN E-ZONE RT AISLE AT ROWS 46 TO 52. REPLACED BROKEN JUMPER AT ROW 52.
