IAH - FLIGHT 4451 THE CREW REPORTED THAT THE RUDDER TRIM KEEPS TRIMMING TO THE LEFT IN FLIGHT. THE AIRCRAFT LANDED AT IAH WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE YAW TRIM SWITCH, OPERATIONALLY TESTED WITH NO DEFECTS AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.

THE CREW REPORTED SMOKE IN LAV ABOVE SINK NEAR LIGHT FIXTURE. MAINTENANCE INSPECTED AND REPLACED LAV LIGHT HOLDER. OPERATIONAL CHECK WERE GOOD.
DURING THE "C" CHECK/"5A" CHECK INSPECTION, A CRACK WAS IDENTIFIED ON STRINGER 2L, LOCATED AT FRAME 75. THIS DISCREPANCY WAS CORRECTED BY REMOVING THE DAMAGED SEGMENT OF THE STRINGER, ADDING A NEW SEGMENT, AND DOUBLER. THE REPAIR WAS PERFORMED IN ACCORDANCE WITH THE EMB SRM 53-00-03.
DURING THE "C" CHECK/"5A" CHECK INSPECTION, A CRACK WAS IDENTIFIED ON THE CAPTAINS WINDSHIELD LATERAL POST. TO CORRECT THE DISCREPANCY, REPAIR PARTS WERE FABRICATED FOR THE LT COCKPIT UPPER LATERAL POST AND INSTALLED IN ACCORDANCE WITH ETD 2017-145-103501.
EICAS "LAV SMOKE" ON TAKEOFF AT 1000FT. VISIBLE SMOKE IN CABIN. AIR RETURN TO TYS. REMOVED AND REPLACED NR 2 ENGINE. RUNS AND LEAK CHECKED GOOD.
LAV SMOKE WARNING ON EICAS AT 500FT AND SMOKE IN CABIN. FOUND NR 2 ENG OIL OVER-SERVICED. REMOVED OIL AND PERFORMED ENGINE RUNS WITH PACKS IN OPERATION. NO SMOKE NOTED AT THIS TIME. AIRCRAFT RELEASED AND RETURNED BACK TO SERVICE.
CREW REPORTED LAV SMOKE ON EICAS. CABIN SMOKE DETECTED ON CLIMBOUT. PULLED CB'S E11, E12. MX REMOVED LAV MIRROR TO CHECK FOR BURNING INDICATIONS. NO BURNING INDICATIONS NOTED, RE-INSTALLED LAV MIRROR AND CONTINUED TO LOOK FOR ANY SIGNS OF BURNING NO INDICATIONS NOTED. A/C WAS APPROVED FOR RETURN TO SERVICE.
CREW REPORTED THE CAPTAIN PFD WENT BLANK WITH ELECTRICAL BURNING ODOR. MAINTENANCE R & R NR 1 PFD WITH OPS CHECK GOOD AND NO ELECTRICAL ODOR NOTED. ACFT WAS APPROVED FOR RETURN TO SERVICE.

DURING C-CHECK THE RT OMEGA BEAM WAS FOUND TO BE CORRODED AT FRAME 19. MAINTENANCE REMOVED THE CORROSION AND PERFORMED A DOUBLER REPAIR IAW EC5320-02057. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING C-CHECK THE RT AFT SKIN AT FRAME 75 STR 2-3R WAS FOUND TO BE CRACKED. MAINTENANCE PERFORMED A DOUBLER REPAIR IAW EC5330-03041. THE ACFT WAS APPROVED FOR RETURN TO SERVICE.
CREW REPORTED SMOKE IN COCKPIT ON RAMP AND SUSPECTED PACK NR 1 TO BE THE ORIGIN. RAN ACFT WITH CREW, WITH PACK NR 1 OFF AND NO EVIDENCE OF SMOKE. MAINTENANCE R & R NR 1 AIR CYCLE MACHINE WITH OPS CHECK GOOD. ACFT WAS APPROVED FOR RETURN TO SERVICE.
CREW REPORTED E1 FIRE DETECTOR FAIL ON EICAS, THREE TIMES DURING ANTI-ICE OPERATION IN CRUISE. MX INSPECTED E1 FIRE DETECTION SYS AND PERFORMED OPS CHECK SUCCESSFUL. ACFT WAS APPROVED FOR RETURN TO SERVICE.
DURING SCHEDULED MX INSPECTION RIC MX FOUND AISLE 12BC OVERHEAD EMERGENCY LIGHT INOPERATIVE. MX REMOVED AND REPLACED 12BC EMERGENCY LIGHT BULB IAW EMB145 AMM 33-50-08 WITH OPERATIONAL CHECK SUCCESSFUL. ACFT WAS APPROVED FOR RETURN TO SERVICE.

TYS - A/C IS IN FOR A C-CHECK INSPECTION WHEN INSPECTION FOUND THE FUSELAGE DENTED AT FR 11 AND 12 AND STRINGER 17L. MAINTENANCE REPAIRED THE DENT IAW EC 5330-02451. PERFORMED A EDDY CURRENT INSPECTION AND NO CRACKS NOTED IAW XJT NDTM-ET-1. A/C WILL BE RELEASED AT THE END OF THE CHECK.
TYS - A/C IS IN C-CHECK WHEN INSPECTION FOUND THE NR 1 ENGINE FWD LWR YOKE WAS CRACKED. MAINTENANCE REMOVED AND REPLACED THE NR 1 FWD LWR YOKE FITTING IAW EC 5450-01028. A/C WILL BE RELEASED AT THE END OF THE CHECK.
FRAME 10 BELOW THE PILOTS DV WINDOW IS CRACKED. REMOVED AND REPLACED FRAME 10 IAW EMB145 SRM 51-40-02.
CLE FLT 5737 - THE CREW REPORTED A FLAP FAIL AT 9 DEGREES MESSAGE ON THE EICAS. THE AIRCRAFT LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE FLAP POWER DRIVE UNIT. OPERATIONAL CHECKS WERE GOOD.
FLIGHT 5685 - EWR - FLIGHT CREW REPORTED BURNING PLASTIC SMELL IN CABIN DURING CLIMBING. CREW PERFORMED AIR RETURN AND LANDED ACFT UNEVENTFULLY. IAD MX PERFORMED ENGINE RUNS AND RAN SYSTEMS AND FOUND NO DEFECTS, EVALUATION FLIGHT WAS PERFORMED AND NO DEFECTS FOUND. ACFT WAS APPROVED TO RETURN TO SERVICE.
ACFT 560 WAS IN FOR A 3C INSPECTION AND CORROSION WAS FOUND ON THE FRONT FRAME FORWARD FLANGE THAT HAD HOLES 9,24,24 AND 40 THAT WERE GREATER THAN THE R/R MANUAL LIMITS. EC 7220-01056 AND TVF5284 WAS ISSUED APPROVING THE REPAIR OF THE DAMAGE AND CONTINUED OPERATION OF THE ENG. MX REMOVED THE CORROSION TO A MINIMUM. CREATED A SMOOTH TRANSITION RADIUS FROM THE AFFECTED AREAS. INSPECT THE AREA WITH SPORT FLUORESCENT PENETRANT INSPECTION METHOD C SENSITIVITY LEVEL 3 IAW ROLLS ROYCE MANUAL. ALL WORK WAS ACCOMPLISHED IAW WITH EC7220-01057 AND TVF 5284 AND WILL BE RELEASED AT THE END OF THE C-CHECK. (S)
FLT 209 - SMF TO ONT - CREW REPORTED E1 FIRE DETECTION FAIL. MX PERSONNEL REMOVED AND REPLACED ENG FIRE DETECTION MODULE OPERATIONALLY CHECKED SATISFACTORY. A/C WAS RELEASED TO RETURN FOR SERVICE IN AIRWORTHY CONDITIONS.
BRO - FLIGHT 2572, AT CRUISE THE CREW REPORTED A BAG SMOKE INDICATION. BOTH FIRE BOTTLES WERE DISCHARGED, AND THE AIRCRAFT RETURNED TO BRO WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT WITH NO DAMAGE OR EVIDENCE OF SMOKE NOTED, MEL'D THE BAGGAGE COMPARTMENT, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. THE BAGGAGE FIRE BOTTLES WERE REMOVED AND REPLACED ON 09/11/06 THE MEL REMOVED, AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING PREFLIGHT INSPECTION FOUND LEFT AND RIGHTHAND OVERWING EMERGENCY LIGHTS TO NOT BE WORKING. REMOVED AND REPLACED NR 4 EMERGENCY BATTERY CHARGER IAW EMB145 AMM 33-50-04, OPS. CHECKED GOOD.
CLE FLIGHT 2469 THE CREW REPORTED UNCOMMANDED ACTIVATION OF THE EMERGENCY LIGHTS IN FLIGHT. THE LIGHTS WOULD COME ON FOR 3 SECONDS THEN RESET. MAINTENANCE INSPECTED AND REPLACED THE FLIGHT ATTENDANT CONTROL PANEL. OPERATIONAL CHECKS WERE GOOD.
CLE FLIGHT NR 2844, THE CREW REPORTED A NR 2 ENGINE FUEL IMPENDING BYPASS MESSAGE ON THE EICAS DURING TAKEOFF ROLL. TAKEOFF WAS ABORTED. MAINTENANCE INSPECTED AND REPLACED THE NR 2 ENGINE IMPENDING BYPASS SWITCH. THE NR 2 ENGINE FUEL FILTER WAS ALSO REPLACED. OPERATIONAL CHECKS WERE GOOD.
DURING ROUTINE MAINTENANCE, BOTH LEFT AND RIGHT EXTERNAL OVERWING EMERGENCY LIGHTS WOULD NOT ILLUMINATE. MAINTENANCE INSPECTED AND REPLACED THE FUSE IN THE NR 4 EMERGENCY BATTERY PACK. OPERATIONAL CHECKS WERE GOOD.
CLE FLIGHT NR 3247, THE CREW REPORTED A LANDING GEAR AIR/GROUND FAIL MESSAGE WHEN THE GEAR WAS SELECTED DOWN. MAINTENANCE INSPECTED AND REPLACED THE LANDING GEAR ELECTRONIC UNIT. OPERATIONAL CHECKS WERE GOOD.
TYS - DURING A 1A CHECK THE LT OVERWING EMERGENCY LIGHT WAS FOUND TO BE INOP. MAINTENANCE REMOVED AND REPLACED THE BULB AND OPS CHECK GOOD. A/C RELEASED AFTER FINAL-1A CHECK WAS C/W.
SHV- DURING C-CHECK THE APU FIREWALL WAS FOUND TO BE CRACKED AT THE UPPER APU MOUNT. MAINTENANCE REMOVED THE DAMAGED AREA AND PERFORMED A DOUBLER REPAIR IAW EMB 145 SRM 53-32-13. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV- DURING C-CHECK THE APU FIREWALL WAS FOUND TO BE CRACKED AT THE RIGHT UPPER APU MOUNT. MAINTENANCE REMOVED THE DAMAGED AREA AND PERFORMED A DOUBLER REPAIR IAW EMB 145 SRM 53-32-13, SECTION 5, FIGURE 204. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV- DURING C-CHECK THE LEFT FLOORBEAM BUTTSTRAP WAS FOUND TO BE DENTED BETWEEN FRAMES 60 AND 61. MAINTENANCE REMOVED AND REPLACED THE AFFECTED BUTTSTRAP IAW EMB 145 SRM 51-40-02. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV- DURING C-CHECK THE LEFT HAND FLOORBEAM WAS FOUND TO BE CORRODED AT FRAME 20. MAINTENANCE REMOVED THE DAMAGED AREA AND PERFORMED A DOUBLER REPAIR IAW EMB 145 SRM 53-21-10, FIGURE 201. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
SHV- DURING C-CHECK THE LEFT HAND ENGINE LOWER COWLING AFT EDGE STIFFENER WAS FOUND TO BE CHAFED. MAINTENANCE REMOVED AND REPLACED THE AFFECTED STIFFENER IAW EMB 145 SRM 51-40-02. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
DURING APPROACH THE CREW NOTED THAT THE TOO LOW GEAR WARNING ON SHORT FINAL DESPITE EICAS SHOWING DOWN WITH THREE (3) GREEN. ABORTED APPROACH, PERFORMED A GO-AROUND AND LANDED WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED GPWS COMPUTER IAW EMB-145 MM 34-44-01 AND THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE. PART NUMBER 965-0976-003-202-202.
THE CREW NOTED THAT THE EMERGENCY LIGHTS FAILED TO ILLUMINATE. MAINTENANCE REMOVED AND REPLACED THE INVERTER, PERFORMED A OPERATIONAL CHECK OF THE EMERGENCY LIGHTING SYSTEM, NO DEFECTS NOTED. THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
ON PREFLIGHT THE CREW NOTED THAT THE FORWARD EMERGENCY EXIT SIGN WAS INOP. MAINTENANCE CLEANED THE INVERTER CONNECTOR, PERFORMED A OPERATIONAL CHECK OF THE EMREGENCY LIGHTING SYSTEM, NO DEFECTS NOTED, AND, THE AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
