AIRCRAFT RETURNED TO BLOCK DUE TO RIGHT HYDRAULIC SYSTEM PRESSURE LIGHT, QUANTITY LIGHT AND EICAS QUANTITY READING 0.03. IRREGULARITY OCCURRED ON RIGHT ENGINE START. FOUND LT WHEEL WELL BRAKE METERING VALVE FAILED. VALVE BODY END CAP SHEARED AWAY FROM COMPONENT. THREE OF FOUR ATTACHING FASTENERS FAILED. REPLACED VALVE, LEAK CKS GOOD.
ODOR IN THE COCKPIT SMELLED LIKE HOT INSULATION, AT THE SAME TIME THE LEFT RECIRC FAN INOP LIGHT CAME ON WITH EICAS MSG. TURNED OFF LEFT RECIRC FAN AND SMELL WENT AWAY. T/S AS PER AMM FOUND LEFT RECIRC INOP AND RED RUBBER SEAL BETWEEN FILTER AND FAN DAMAGED. PARTS NIS MUC FWD TO MCO AS PER MEL 21-25-01A. PLACARD INSTALLED. C/B R14 PULLED AND SECURED.
RT GENERATOR OFF LIGHT AND EICAS MESSAGE CAME ON AFTER T/O. STARTED APU AND GOT A GOOD ELEC FROM THE GENERATOR. REPLACED RT GEN PER M/M. OPS CHECK NORMAL.
WITH PWR ON ELECTRICAL ODOR COMING FROM COFFEEMAKERS IN FWD GALLEY. ODOR DISSIPATED WITH POWER OFF. FOUND HEAVY ACCUMULATION OF COFFEE RESIDUE. NO BURNT WIRES. REPLACED BOTH COFFEEMAKERS AS PRECAUTION. CK''D, NO SMELL OF BURNING.
SEAT MOUNTED PROXMITY ESCAPE PATH LIGHT INOP AT SEAT ROW 37FG. FOUND ILLUMINATOR CONNECTOR PIN PULLED LOOSE AT SEAT HARNESS. REMOVED AND REPLACED ILLUMINATOR ASSY. OPERATIONAL CHECK NOWOK.

A SKIN PUNCTURE WAS FOUND AFT OF THE NR 1 CARGO DOOR AT FS 618 BETWEEN STR 29R AND 30R. THE SKIN WAS CRACKED APPROX 1.5 AND HAD A 3.5 GOUGE. THE DAMAGE CLEANUP RESULTED IN A 1.75 BY 2.75 CUTOUT IN THE FUS SKIN ONLY WITH THE REMAINING GOUGE BLENDED WITHIN SRM LIMITS. NO DAMAGE TO INTERNAL STRUCTURE. DOUBLER FROM 0.100 CLAD 2024-T3, TRIPLER FROM 0.063 CLAD 2024-T3.
RT ENG COMPRESSOR STALL DURING REVERSE THRUST AT FAIRLY, HIGH RPM SLOWING THROUGH 70 KTS, NO ENGINE EXCEEDENCES. BORESCOPED NR 2 ENGINE, NO PROBLEMS DETECTED. RAN ENGINE, OPS CHECK GOOD.
DURING GROUND OPERATIONS, APU AUTO SHUTDOWN DUE WITH TAILPIPE FIRE. INSPECTION REVEALED GEARBOX OIL IN TAILPIPE CAUSING FIRE. REPLACED APU.
AFTER T/O, RT ENG EEC ALTN LIGHT CAME ON, EPR NORMAL NO EICAS MSG. REPLACED EEC SWITCH LIGHT PANEL, OPS CHECK OK.
LEFT ENGINE COMPRESSOR STALL IMMEDIATELY AFTER T/O. MADE SUCCESSFUL LANDING WITH LEFT ENGINE AT IDLE. REPLACED ENGINE.
WIRING FOR F/C SEAT 2-A AND OTHER SEATS ON CIRCUIT SHORTED AND BURNED THROUGH. PULLED ASSOCIATED CIRCUIT BREAKERS, P-36 AND P-37 IN E/E COMPARTMENT PER MTC CO-ORDINATOR. REPAIRED CHAFED WIRE. SYSTEM OPS CHECK GOOD.
FLIGHT OUT OF FRA - RT IDG DRIVE LIGHT CAME ON STEADY. APU STARTED AND RT IDG DISCONNECTED. APPROXIMATELY 1 HR LATER - APU OIL QTY - EICAS STATUS MSG. APPROXIMATELY 45 MINUTES LATER - APU AUTO SHUTDOWN. FLIGHT DIVERTED TO SNN. REPLACED RT IDG.
TWO CRACKS WERE FOUND IN THE NR 1 ENGINE PYLON UPPER SPAR WEB AT THE OUTBOARD DRAIN HOLE AT NAC STA 156 AND WL 149.21. THE CRACKS WERE STOP DRILLED AND REINFORCED WITH A FILLER AND DOUBLER PER DL ERA 273499-14, REV A.
TWO CRACK LOCATIONS HAVE BEEN REPORTED IN THE NR 2 ENGINE PYLON OUT'BD STRUT SKIN. THE CRACKS START FROM THE FWD TORQUE BOX ACCESS HOLE. A 1 INCH CRACK IS IN THE UPPER FWD CORNER AT NAC STA 158. SECOND CRACK IS IN THE UPPER AFT CORNER AT NAC STA 192 AND IS 2 INCHES LONG. BOTH CRACKS EXTEND THROUGH BOTH SHEETS OF 0.080 PLUS 0.080 2024-T3 BONDED SKIN ASSEMBLY. THE AFT CRACK TERMINATES IN AN EXISTING FASTENER HOLE. ACCOMPLISHED REPAIRS PER DL ERA 271951-14 REV, D.
PERFORMED ENGINE STABILITY RUN, FOUND LOW STALL MARGIN. ENGINE WAS REMOVED AND ROUTED TO SHOP FOR COMPLETION OF EO SI-690053. ETOPS CATEGORY 'F': ANY OTHER EVENT DETRIMENTAL TO ETOPS
DIFFERENCE BETWEEN CALCULATED AND TOTALIZER FUEL GREW TO 4,000 LBS. CALCULATED WAS GREATER, NO FUEL LEAK OBSERVED DURING FLT. FOUND NR 1 ENGINE FUEL PUMP HOUSING CRACKED. REPLACED PUMP, OPERATION NORMAL.
TWO CRACKS HAVE BEEN REPORTED IN THE NR 2 ENGINE PYLON O/B STRUT SKIN. THEY BOTH GO THROUGH BOTH SHEETS OF THE BONDED SKIN ASSEMBLY. REPAIRED PER DL ERA 271951-14.
DURING L/O CHECK, SMOKE WAS SEEN COMING FROM FORWARD CABIN AREA, GROUND POWER WAS IMMEDIATELY DISCONNCTED, FOUND THREE SHORTED WIRES INSIDE LEFT SEAT ATTACH TRACK AT SEAT LOCATIONS 1C AND 2C. SHORT CAUSED MINOR BURN DAMAGE TO AREA. REPAIRED SHORTED WIRING FOR SEAT FOOT REST MOTOR AND REPLACED ADJACENT DAMAGED CABLE ASSEMBLY.
