Maintenance disclosures
DURING TAKEOFF ROLL ON RWY 34 AT KHKS, AT APPROXIMATELY 80 KTS, WE RECEIVED THE WINDSHIELD OVERHEAT ANNUNCIATOR. TAKE OFF WAS ABORTED WITHOUT INCIDENT. AS POWER WAS RETARDED DURING THE ABORT, THE ANNUNCIATOR EXTINGUISHED AS ENGINES APPROACHED IDLE THRUST. ROTARY TEST OF WINDSHIELD HEAT DURING PREFLIGHT CHECKED OK.
PILOT HAD LOUD NOISE THEM VIBRATION, WAS ON TAKE-OFF OUT OF 15K FEET AT 285 KNOTS. RETURNED TO KRDU AND FOUND RIGHT ENGINE LOWER COWLING MISSING AND TOP COWLING HANGING ON AT TOP INBOARD ATTACH. FASTENERS WERE IN INLET AND TR AND COWLING SHEET METAL WAS TORN OUT AT ALL HOLES.
TAKEOFF FROM BUF TO UTICA, NY, PILOT REPORTED FLUX IN OIL PRESSURE LT ENGINE, FOLLOWED BY ZERO OIL PRESSURE. UNEVENTFUL DIVERT TO ROC. INVESTIGATION FOUND .75 INCH HOLE IN CRANKCASE JUST UNDER THE DATA PLATE. CYLINDER REMOVAL FOUND THE NR 3 ROD AND CAP SEPARATED FROM THE CRANK THROW. FOUND ONE BOLT SHEARED AND OTHER BOLT SHOWED SECONDARY DAMAGE. NO SIGNS OF OVERHEATING, OVERSTRESS, OVERBOOST, OR OVERSPEED, OR MISASSEMBLY TECHNIQUE. CRANK AND CASES DAMAGED BEYOND REPAIR.
