PILOT REPORTED SEWAGE/ SULFUR SMELL. VOLTAGE ALSO DROPS TO 13 VOLTS ON START UP AND TAKES A WHILE TO RECOVER. REMOVED AND REPLACED BATTERY PERFORMED GROUND RUN AND 10 MIN MAINTENANCE FLIGHT. NO DEFECTS NOTED IN REF TO BELL 407 MM CH 24.
FUEL SMELL IN CABIN BY MED CREW, INSPECTED FOR FUEL LEAKS, NONE FOUND. REAPPLIED SEALANT TO PREVENT FUEL VENT FROM ENTERING CABIN. THIS AIRCRAFT HAS BEEN RETURNED TO REVENUE SERVICE.
PILOT CALLED ME THAT THEY LANDED AT BULLHEAD CITY MAINTENANCE AND HAD THE SMELL OF SMOKE IN THE CABIN. ARRIVED AT AIRPORT TALKED WITH PILOT STATED THAT THEY SMELLED IT IN FLIGHT TURNED OFF AIR CONDITIONING SYSTEM AND THE SMELL WENT AWAY. PUT AIRCRAFT IN THE HANGER RAN AC SYSTEM THE SMELL WAS NOTICEABLE FROM THE CEILING VENTS. COULD NOT PIN POINT WHERE IT WAS COMING FROM. REMOVED THE MEDICAL WALL DID NOT FIND ANY ISSUE DECIDED TO REPLACE AFT FAN MOTOR AS A PRECAUTION. ONCE FAN WAS REMOVED AND DISASSEMBLED FOUND THAT THE WINDINGS ON THE MOTOR STARTED COMING APART.
PILOT WAS STARTING AIRCRAFT DURING GROUND IDLE. TURNED AIR-CONDITIONING ON, CREW REPORTED SMOKE EMITTING FROM PILOT AND CO PILOTS VENTS. CREW SHUT DOWN AIRCRAFT AND ALERTED MAINTENANCE. MAINTENANCE WAS ABLE TO SEE THE RESIDUAL SMOKE, PULLED THE FAN MOTORS TO EXAMINE THE INTERNAL CONDITION OF THE EVAPORATOR. FOUND MELTED PLASTIC ON THE RESISTOR ON THE EVAPORATOR, AND THE RESISTOR ON THE OTHER EVAPORATOR LOOKED TO HAVE BURN MARKS ON IT.
ENROUTE TO PICK UP PATIENT, PILOT GOT AN ENGINE CHIP INDICATION AND LANDED AIRCRAFT IN NEAREST SAFE LAND SPOT POSSIBLE, IN A PASTURE. MECHANIC WAS DISPATCHED TO AIRCRAFT. MECHANIC FOUND METALLIC PARTICLES ON LOWER ENGINE CHIP PLUG EXCEEDING RR MM GUIDELINES. ENGINE IS BEING CHANGED.
DURING DECENT, A TRANSMISSION CHIP LIGHT ILLUMINATED ON THE CAUTION WARNING PANEL. DIVERTED TO THE CLOSEST SUITABLE LANDING AREA. AFTER LANDING AND SHUTDOWN,
MAIN ROTOR FEATHERING BEARING ELASTOMER SEPARATION BEYOND LIMITS, FOUND DURING AIRWORTHINESS CHECK. THIS ACFT REMAINS OUT OF SERVICE AT THIS TIME AWAITING A REPAIR TO THE MRH INSTALLING NEW FEATHERING BEARINGS.
THE ENG CHIP LIGHT ILLUMINATED IN CRUISE FLIGHT. THE PILOT IMMEDIATELY DIVERTED TO A NEARBY AIRPORT & LANDED WITHOUT ANY FURTHER ISSUES. THE MECHANIC ARRIVED & INSPECTED BOTH UPPER & LOWER ACCESSORY GEARBOX CHIP DETECTORS. THEY BOTH SHOWED EXCESSIVE AMOUNTS OF METAL & WERE DETERMINED TO BE WELL BEYOND THE ALLOWABLE LIMITS. THIS ACFT IS CURRENTLY OUT OF SERVICE, AT THE TIME OF THIS SUBMISSION, FOR AN ENG REPLACEMENT.
PILOT LEFT THE PAD TO PICK UP MEDICAL CREW. ON HIS WAY OVER, THE PEDAL STOP WARNING INDICATOR ON THE CAUTION WARNING PANEL ILLUMINATED, SO PILOT SLOWED HIS AIRSPEED BELOW 55 KNOTS & IMMEDIATELY RETURNED TO BASE. LESS THAN A MILE FROM THE BASE. RETURNING TO THE BASE WAS THE SAFEST & MOST PRACTICAL RESPONSE. PILOT THEN TRANSITIONED TO THE SPARE ACFT & WAS ABLE TO STILL PERFORM THAT REVENUE FLIGHT. DISCOVERED THAT THE SOLENOID THAT ENGAGES THE LIMITING PIN IN THE PEDAL STOP SYS WAS NO LONGER FUNCTIONING CORRECTLY. IN COMMUNICATING WITH THE REP, WE LEARNED THAT THE LIKELY CULPRIT WAS MOISTURE BUILD UP IN THE SOLENOID CAUSING IT TO MALFUNCTION. CHANGED THE SOLENOID. THE SYS IS ONCE AGAIN FUNCTIONING AS DESIRED.
EN ROUTE TO A PR EVENT, TAIL ROTOR CHIP LIGHT ILLUMINATE CAUSING AN UNSCHEDULED LANDING. REMOVED & EXAMINED THE MAGNETIC CHIP PLUG & FOUND A SMALL HAIR LIKE METAL SLIVER. THIS PARTICLE WAS REMOVED, CHIP DETECTOR CLEANED & THE PLUG REINSTALLED. GND RAN ACFT FOR 15 MINUTES WITH NO FURTHER LIGHT. THE ACFT WAS FOUND TO BE AIRWORTHY AT THAT TIME & WAS RETURNED TO SERVICE.
DURING MAINTENANCE FLIGHT HAD T/R CHIP LIGHT ILLUMINATE CAUSING A UNSCHEDULED LANDING. REMOVED & EXAMINED MAGNETIC CHIP PLUG. SMALL HAIR LIKE METAL SLIVER FOUND. CLEANED & RE-INSTALLED PLUG & STILL HAD CAUTION LIGHT. CHECKED WIRING & FOUND TO BE SERVICEABLE. FOLLOWED PROCEDURES FOR GEARBOX FLUSH IN MM CH 65 FOR INSPECTION & SERVICEABILITY CHECK. FOUND TO BE SERVICEABLE SO RE-SERVICED IAW MM CH 12. GND RAN ACFT WITH NO FURTHER LIGHT. ACFT WAS FOUND TO BE AIRWORTHY SO RETURNED IT TO SERVICE.
ON LANDING, ENGINE CHIP LIGHT ILLUMINATED. NUMEROUS SLIVERS AND CHIPS FOUND ON BOTH THE TOP AND BOTTOM GEARBOX CHIP DETECTORS. ACFT WAS REMOVED FROM SERVICE. ENGINE CHANGED.
