THE AIRCRAFT WAS SINGLE-PACK ONLY WHEN IT LEFT DCA. DURING CLIMB AT FL230, THE FUNCTIONING LH PACK OVER-PRESSURED AND AUTOMATICALLY SHUTOFF. THE CREW INITIATED AN EMERGENCY DESCENT TO FL100. THEY CREW WAS ABLE TO RESET THE PACK ONCE REACHING THAT ALTITUDE AND IT BEGAN FUNCTIONING PROPERLY AGAIN. THE AIRCRAFT WAS ABLE TO CONTINUED TO CLE. THE AIRCRAFT LANDED AND TAXIED IN WITHOUT FURTHER COMPLICATIONS.

ON APPROACH TO DCA CREW RECEIVED FUEL IMBALANCE MESSAGES FOLLOWED QRH SAID NEEDED TO LAND WITH 20 DEGREES FLAPS, CREW DIVERTED TO IAD FOR THE LONGER RWY AND SAFETY. AN EMERGENCY WAS NOT DECLARED PAX WHERE NOTIFIED OF THE DIVERSION. DEFERRED FUEL QUANTITY INDICATING SYSTEM AND LEFT FUEL BOOST PUMP. SUPPLEMENTAL INFORMATION: MAINTENANCE REMOVED AND REPLACED THE FUEL SYSTEM COMPUTER AND SYSTEM OPERATIONS CHECKED GOOD.

ON DECENT THE CAPTAINS WINDSHIELD CRACKED IN FLIGHT. THE CREW DONNED THEIR OXYGEN MASKS AND DECLARED AN EMERGENCY. THE AIRCRAFT LANDED WITHOUT FURTHUR INCIDENTS. MAINTENANCE REPLACED THE WINDSHIELD AND IS BACK IN SERVICE.

CREW REPORTED A LIGHTNING STRIKE IN FLIGHT EN ROUTE FROM CAK TO PHL. AIRCRAFT LANDED IN PHL WITHOUT INCIDENT. MAINTENANCE INSPECTED AIRCRAFT AND REMOVED AND REPLACED RADOME, TAT PROBE, ATC ANTENNA AND STATIC WICKS. DAMAGE (BURN MARKS) WAS REPAIRED ON LEFT AND RIGHT NOSE LANDING GEAR DOORS, LEFT FUSELAGE, LEFT HORIZONTAL STABILIZER TIP FAIRING AND TAIL CONE. DURING SCHEDULED MAINTENANCE ON THE RIGHT AND LEFT WING FUEL TANK ACESS PANELS, MINIOR CORROSION WAS FOUND. PANELS INSPECTED AND REPAIRED ARE 650DB, 650AB, 650BB, 550CB, 550DB, 550AB, AND 550BB. THE ABOVE INSPECTIONS AND REPAIRS WERE ACCOMPLISHED USING THE AIRCRAFT MAINTENANCE MANUALS. AIRCRAFT WAS RETURNED TO SERVICE.
ON CLIMBOUT FROM GNV, CREW REPORTED THE LEFT ENGINE FAILED AT ABOUT 3,000 FEET WITH SEVERE VIBRATION AND LOSS OF THRUST DOWN TO BELOW IDLE. ENGINE WOULD NOT RESPOND TO THRUST LEVER INPUTS. CREW SHUT DOWN THE LEFT ENGINE PER QRH, RETURNED TO GNV AND LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED, REMOVED AND REPLACED THE LEFT ENGINE DUE TO COMPRESSOR DAMAGE AND PERFORMED CHECKS USING THE AIRCRAFT MAINTENANCE MANUALS. AIRCRAFT WAS RELEASED FOR SERVICE.

CREW REPORTED AN ANTI-ICE DUCT WARNING MESSAGE, WITH THE RT FUSELAGE INDICATING RED, AFTER DEPARTURE FROM TYS EN ROUTE TO CLT. AIRCRAFT DIVERTED TO TRI AND LANDED WITHOUT INCIDENT. DURING SCHEDULED MAINTENANCE IT WAS FOUND THAT A PROTRUDING DENT 4 X .75 X .015 INCH DEEP BETWEEN FS 682 - FS 700 AND BETWEEN STRINGERS 23-24 R. REPAIRS WERE MADE USING THE MANUFACTURERS MANUALS AND AIRCRAFT RELEASED FOR SERVICE.
DURING SCHEDULED MAINTENANCE IT WAS FOUND THAT A CRACK AT FRAME 559 FRAME CAP ANGLE BEND RADIUS LBL 18.0. ALSO, A WEB CRACK AT 559.0 INBOARD OF RBL 18.0 WL 59.10. REPAIRS WERE MADE USING THE MANUFACTURERS MANUALS AND AIRCRAFT RELEASED FOR SERVICE.
CREW REPORTED ON TAKEOFF RECEIVED A RED ANTI ICE DUCT MESSAGE. AIRCRAFT RETURNED TO CLT AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AIRCRAFT AND REMOVED AND REPLACED A FUSELAGE LOOP USING THE AIRCRAFT MAINTENANCE MANUAL. CHECKS WERE MADE USING THE MAINTENANCE MANUAL AND THE AIRCRAFT RELEASED FOR SERVICE.
CREW RECEIVED NR 1 IDG DISCONNECT AND NR 1 GENERATOR OFF CAS MESSAGE ON TAKEOFF ROLL. THE CREW ABORTED THE TAKEOFF AND RETURNED TO GATE IN CLT. MAINTENANCE INSPECTED AND SERVICED THE NR 1 IDG AND LEAK CHECKED PER THE AIRCRAFT MAINTENANCE MANUAL. AIRCRAFT WAS RELEASED FOR SERVICE.
CREW REPORTED AN ABORTED TAKEOFF AT ABOUT 80 KNOTS AND RETURN TO GATE IN CLT FOR NOSE WHEEL STEERING PULLING RIGHT. MAINTENANCE INSPECTED AIRCRAFT AND REPLACED THE RT RVDT. OPERATION CHECKS ACCOMPLISHED WITH NO OTHER DEFECTS. AIRCRAFT WAS RELEASED FOR SERVICE.

CREW REPORTED AMBER CAS MESSAGE IDG 1 AT 60 KNOTS. CREW ELECTED TO ABORT TAKEOFF AND RETURNED TO GATE WITHOUT INCIDENT. MAINTENANCE INSPECTED AND SERVICED NR 1 IDG. CHECKS GOOD. NO FURTHER DEFECTS. AIRCRAFT RELEASED FOR SERVICE.
CREW REPORTED THE APU FLAMED OUT AFTER ENGINE SHUTDOWN. GROUND CREW STATED SMOKE WAS COMING FROM THE RIGHT SIDE OF THE AIRCRAFT. MAINTENANCE INSPECTED AND FOUND SMOKE COMING FROM THE APU INTAKE DOOR, FUEL LEAKING FROM DRAIN ON TO THE AFT EQUIPMENT BAY. MAINTENANCE PERFORMED A GENERAL VISUAL CHECK OF THE APU, PERFORMED A GENERAL VISUAL INSPECTION OF THE APU FUEL SYSTEM COMPONENTS AND PERFORMED A START/STOP SEQUENCE OF THE APU IN ACCORDANCE WITH THE AIRCRAFT MAINTENANCE MANUAL. OPERATIONAL CHECKS WERE PERFORMED. NO DEFECTS NOTED. AIRCRAFT WAS RELEASED FOR SERVICE.
CREW REPORTED ON TAXI-OUT AN APU AUTO SHUTDOWN. CREW ELECTED TO RETURN TO THE GATE WITHOUT INCIDENT. MAINTENANCE INSPECTED AND DEFERRED THE APU PER MEL 49-10-01-1. MAINTENANCE PROPERLY SERVICED THE APU OIL LEVEL IN ACCORDANCE WITH THE AIRCRAFT MAINTENANCE MANUAL. OPS CHECKS OF THE APU WERE GOOD, NO DEFECTS WERE NOTED. MEL WAS CLEARED. AIRCRAFT WAS RELEASED FOR SERVICE.
CREW REPORTED A BIRD STRIKE TO THE CAPTAIN SIDE WINDSHIELD ON SHORT FINAL. MAINTENANCE INSPECTED AND FOUND POSSIBLE DELAMINATION TO THE WINDOW. MAINTENANCE COMPLIED WITH INSPECTION OF LEFT HAND WINDSHIELD AND FOUND TO BE SURFACE DISCOLORATION ONLY IN ACCORDANCE WITH THE AIRCRAFT MAINTENANCE MANUAL 56-11-01-210-801. AIRCRAFT WAS RELEASED FOR SERVICE.

THE CREW REPORTED THE AUTO PILOT KICKED OFF WHEN GOING TO FLAPS 20, WENT BACK TO FLAPS 8 AND RE-ENGAGED AUTO PILOT, TRIED AGAIN WITH SAME RESULT. MAINTENANCE OPERATIONALLY CHECKED THE AUTO PILOT SYSTEM. OPS CHECKS GOOD. COULD NOT DUPLICATE ON THE GROUND. THE AIRCRAFT WAS RELEASED FOR SERVICE.
AFTER TAKEOFF THE CREW REPORTED WHEN SWITCHING BLEED SOURCES THE RT PACK SHUT DOWN WITH A HIGH PRESSURE MESSAGE. MAINTENANCE PERFORMED AN OPERATIONAL CHECK OF THE RT PACK. OPS CHECKS GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.
CREW REPORTED -O/B GROUND SPOILER FAULT- IN EN ROUTE. MAINTENANCE RESET AND OPERATIONALLY CHECKED SYSTEM. NO FAULTS FOUND. OPERATIONAL CHECKS WERE GOOD. AIRCRAFT WAS RETURNED FOR SERVICE.
AFTER TAKEOFF THE CREW REPORTED AN INTERMITTENT -STALL FAIL- WARNING MESSAGE. THE FLIGHT RETURNED TO DTW AND LANDED WITHOUT INCIDENT. MAINTENANCE FOUND THE OXYGEN SERVICING ACCESS DOOR OPEN. INSPECTION OF THE DOOR REVEALED NO DEFECTS. THE DOOR WAS PROPERLY SECURED. THE STALL PROTECTION SYSTEM OPERATIONALLY CHECKED GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING THE SERVICE CHECK, MAINTENANCE FOUND EVIDENCE OF BIRD STRIKE DAMAGE TO THE LEFT INBOARD FLAP. MAINTENANCE COMPLIED WITH A TEMPORARY REPAIR IAW BOMBARDIER REO 601R-57-52-517. THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING CLIMB THE RT ENGINE COWL ANTI-ICE CAUTION MESSAGE ILLUMINATED. THE CREW FOLLOWED THE QRH AND RETURNED TO PHL. MAINTENANCE REMOVED AND REPLACED THE RT COWL ANTI-ICE VALVE WITH A SERVICEABLE UNIT. THE SYSTEM LEAK AND OPERATIONALLY CHECK GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE FLAPS FAIL CAUTION MESSAGE ILLUMINATED WITH FLAPS AT 8 DEGREES. THE CREW FOLLOWED THE QRH AND LANDED WITHOUT INCIDENT. MAINTENANCE RESET AND OPERATIONALLY CHECKED THE FLAP SYSTEM. ALL CHECKS WERE GOOD. THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING AN FAA RAMP CHECK, THE INSPECTOR REPORTED EVIDENCE OF LIGHTNING DAMAGE ON THE LT WINGTIP. MAINTENANCE COMPLIED WITH EN ROUTE AND DETAILED LIGHTNING STRIKE INSPECTIONS. NO DAMAGE OR EVIDENCE OF A LIGHTNING STRIKE WAS FOUND. THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED A LIGHTNING STRIKE DURING DESCENT. MAINTENANCE COMPLIED WITH A DETAILED LIGHTNING STRIKE INSPECTION IAW CRJ200 AMM 05-51-05-220-801. THE AIRCRAFT WAS FERRIED TO DAY WHERE MAINTENANCE REPAIRED THE LIGHTNING DAMAGE AND RELEASED THE AIRCRAFT FOR SERVICE.
DURING CLIMB AFTER TAKEOFF, AT 1000 FEET, THE CABIN STARTED CLIMBING WITH THE AIRCRAFT AND A LOUD SQUEAL WAS HEARD COMING FROM THE FORWARD CABIN. THE CREW OF FLIGHT 2321 RETURNED TO DAY AND MADE AN UNEVENTFUL LANDING. PSA MAINTENANCE FOUND THE PASSENGER DOOR SEAL DAMAGED. THE SEAL WAS REMOVED AND REPLACED IAW CRJ200 AMM 52-11-10-000-803 & 400-803. THE PRESSURIZATION OPERATIONAL TEST WAS GOOD IAW CRJ 200 AMM 21-31-00-863-005. THE AIRCRAFT WAS THEN RELEASED FOR SERVICE.
AFTER SHUTDOWN IN DAY EVIDENCE OF A BIRD STRIKE ON THE RT WING INBOARD LEADING EDGE WAS REPORTED BY THE CREW OF FLIGHT 2461. PSA MAINTENANCE INSPECTED THE AREA AND FOUND THE DAMAGE TO BE OUT OF LIMITS FOR CONTINUED SERVICE. PSA MAINTENANCE THEN REMOVED & REPLACED THE DAMAGED RT WING INBOARD LEADING EDGE PANEL WITH A SERVICEABLE UNIT IAW CRJ200 AMM 57-41-01 AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED THAT THE LEFT HAND ENGINE FIRE LOOP-A WAS INOP. MAINTENANCE CONTROL DEFERRED THE LEFT HAND FIRE LOOP-A WITH THE CREW PER MEL 26-11-01 CODE-C. THE AIRCRAFT WAS THEN RELEASED FOR SERVICE.
DURING FLIGHT, THE CREW RECEIVED A FLAP DEGRADE STATUS MESSAGE AND DIVERTED TO BTV. MAINTENANCE WAS SENT TO BTV. MAINTENANCE THEN INSPECTED AND REPLACED THE LEFT HAND FLAP POSITION TRANSMITTER ASSEMBLY IN ACCORDANCE WITH DORNIER MAITNENANCE MANUAL 27-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
