CRACKS FOUND ON THE LT BRACKET AFT PORTION OF THE VERTICAL FIN BETWEEN NUT PLATES WHERE THE CAP MOUNTS ONTO.
AFTER REMOVING LOWER ENGINE FWD SKINS, A NEW CRACKED AREA WAS LOCATED AT STATION 3525 BULKHEAD LT AND RT SIDES WITH EXTRUDED ANGLES AT STATION 4010.
AFTER REMOVING LOWER ENGINE DECK SKINS, MULTIPLE CRACKS WERE FOUND AT STA 4010, WL 1020, BL 0 UNDERNEATH FORWARD ENGINE MOUNTS. THE VISIBLE CRACKS ON THE U CHANNEL "SPINE" EMANATE AROUND THE FASTENER HOLES FOR THE FORWARD ENGINE MOUNT. THE CRACKS ARE AROUND THE AFT 2 FASTENER MOUNT HOLES. VISIBLE CRACKS ARCH THROUGH THE EXTRUSION "L" ANGLE. "C" ANGLE WITHIN "SPINE" CHANNEL BROKE/ CRACKED OFF UPPER LEG WHERE AFT FASTENER HOLES ARE LOCATED. CRACKING WAS FOUND ON RT DECK SKIN AND FILLER DOUBLER IN SAME AREA WHERE ENGINE MOUNT WAS LOCATED. CHIPPING/ WEAR WAS FOUND TOWARDS THE FWD LT SIDE OF THE MOUNT IN THE "SPINE" AREA. THE FORWARD DECK SKIN HAS A LARGE SKIN TEAR THAT WAS UNDER THE A/C COMPRESSOR DOUBLER AND A PUNCTURE FROM A/C COMPRESSOR MOUNTING HARDWARE.
WHILE COMPLETING FURTHER INSPECTIONS IT WAS NOTED THAT AT STA 4010 WL 1020 A LT ANGLE WAS CRACKED INSIDE THE INSPECTION ACCESS HOLE.
WHILE PERFORMING FURTHER INSPECTIONS A CRACK DISCOVERED ON THE LT SIDE OF THE FUSELAGE SKIN, BY THE FOOT STEP AT STA 4710 WL 715 LOCATED APPROX 3.5 IN LONG.
WHILE PERFORMING AN AIRWORTHINESS CHECK, CRACKS WERE DISCOVERED ON THE LT SIDE OF THE FUSELAGE SKIN AT STA 4010 WL 1419 LOCATED 3 CRACKS 1/2 LONG. PER SERVICE ENGINEERING AND AN SPECIAL FLIGHT PERMIT, AIRCRAFT WAS REPOSITIONED TO MAINTENANCE FACILITY FOR SHEET METAL REPAIRS.
DURING A 135 FLIGHT WITH A PATIENT ON BOARD A CREW MEMBER TRIED TO CLOSE THE RT REAR SLIDING DOOR THE DOOR WOULD CLOSE BUT WOULD NOT LATCH SHUT, THE AIRCRAFT LANDED AT IFP WHERE THE DOOR WAS REPLACED, THE MECHANICS VERIFIED THAT THE DOOR CLOSED AND LATCHED. AFTER TEARING THE DOOR HANDLE APART THE MECHANIC FOUND THE THE SPRING THAT LATCHED THE DOOR HAD BROKEN AND REPLACED IT.
