BTH-206L1-MM-1-5-31 ( TAIL BOOM) ,BTH-206L1-MM-1-27-60 PAR 5. REMOVAL AND INSPECTION OF TAIL BOOM STABILIZER REVEALED WEAR AND CORROSION EXCEEDING MANUFACTURES LIMITS. REMOVED AND REPLACED COMPONENT PER MANUFACTURES INSTRUCTIONS. NO FURTHER DEFECTS NOTED .
PILOT NOTED A +5 DEGREE ON POWER CHECK AND AT TIMES DID NOT FEEL THAT THE ENGINE WAS PRODUCING FULL POWER. UPON REMOVING THE ENGINE INLET COWLING A 1.7500" PIECE OF TUBING WITH A 90 DEGREE BEND WAS FOUND INSIDE THE INLET SCREEN AREA ON THE SCREEN FLOOR. THERE WAS DAMAGE TO THE IMPELLER ASSEMBLY AND SUPPORT HOUSING. THE TUBE SECTION WAS BROKEN OFF OF THE EJECTOR NOZZLE ASSEMBLY, PN 23084996 CONTAINED WITHIN THE EJECTOR TUBE ASSEMBLY.
ENGINE START WAS NORMAL BROUGHT THE AIRCRAFT TO A 3 FT HOVER ALL ENGINE INDICATIONS NORMAL, AT THIS TIME THE PILOT HEARD A SLIGHT BANG FROM THE REAR OF THE AIRCRAFT FOLLOWED BY A LOUD GRINDING WHINE AND A HIGH FREQUENCY VIBRATION IN AIRFRAME. AC BEGAN A SLIGHT YAW AND NOTED N2 DECREASING. THE PILOT SET THE AIRCRAFT DOWN ON THE HELIDECK AND SHUT THE ENGINE DOWN. HE ALSO NOTED OIL COMING OUT OF ENGINE DECK DRAIN, HE ALSO OPENED THE ENGINE COWLING AND OBSERVED AN OIL PUDDLE IN THE ENGINE PAN. NOTIFIED MAINTENANCE. THE MECHANIC SUBSEQUENTLY REMOVED AND REPLACED ENGINE. (SW03200405257)
DURING TAKE-OFF, THE TRANSMISSION CHIP LIGHT ILLUMINATED. RETURNED TO BRA7. PILOT PULLED, INSPECTED AND CLEANED CHIP PLUG. LEAK CHECKED AND GROUND RAN IAW OPS MANUAL. NO DEFECTS NOTED.
DURING REMOVAL OF COMPONENTS FOR AIRCRAFT MAJOR AIRFRAME INSPECTION. SEVERE CORROSION OF FITTING CAUSED BY MOISTURE TRAPPED IN THE FIRE SLEEVE PROTECTION. PART WAS SCRAPPED.
DURING DAILY INSPECTION FOUND TRAILING EDGE EXTRUSION HEAVILY CORRODED.
DISCOVERED DURING DAILY INSPECTION, M/R BLADE HAD CORROSION AND SKIN DEBOND AT BLADE TIP.
IN-FLIGHT AND GROUND RUN, FOUND INTERMITTENT HIGH PITCHED WHINE IN CABIN. INNER COUPLING FOUND TO HAVE UNACCEPTABLE WEAR PATTERN. OUTER COUPLING WAS SERVICEABLE; NEITHER COUPLING WAS WORN BEYOND DIMENSIONAL LIMITS.
DAILY INSPECTION FOUND BUSHINGS AND BEARINGS WORN BEYOND LIMITS. PARTS REQUIRING REPLACEMENT: 4 EACH 206-010-338-001 SPACERS, 4 EACH 206-010-337-001 BEARINGS, 2 EACH 206-010-338-003 BUSHINGS, 2 EACH NAS6605D64 BOLTS, 1 EACH 206-010-470-101 BEARING. NORMAL USE CAUSES PARTS TO WEAR OUT TOO SOON. WORN PARTS TT 690 HOURS.
