SHORT ED AND SHORT PD HOLES WERE DISCOVERED ON THE 7 DEGREE BULKHEAD ADJACENT TO THE RECESSED MEDICAL WALL INSTALLATION.
R/H SIDE FWD. TRANSMISSION DECK WEB (P/N: 350A21-4418) IS DAMAGED IN THE FORM OF A CHAFING. THE DAMAGE IN THE FWD IS APPROXIMATELY 0.3â IN LENGTH, 0.3â IN WIDTH AND 0.019â IN DEPTH AND DAMAGE AT AFT IS APPROXIMATELY 0.3â IN LENGTH, 0.25â IN WIDTH AND 0.010â IN DEPTH.
CHAFING DAMAGE WAS DISCOVERED ON THE RH SIDE OF THE TRANSMISSION DECK. THE DEEPEST DEPTH IS 0.015" WITH A LENGTH OF 2.1" AND WIDTH OF 0.1.
DAMAGE TO BOTH X1790 FRAME (P/N: 350A214099) AND SUPPORT (P/N: 350A214107) IN THE FORM OF A DENT AND CHAFE NEAR A LOWER FAIRING MOUNTING POINT. THE DAMAGE AREA WAS MEASURED TO BE 0.2â IN LENGTH, 0.2â IN WIDTH AND 0.03â IN DEPTH.
CRACK DAMAGE WAS NOTED ON THE RIGHT SIDE OF THE CABIN FLOOR NEAR THE DOOR POST WITHIN THE AREA OF THE DOOR STRUT FITTING.
HINGED RIGHT HAND CABIN DOOR HAS A 9 INCH VERTICAL CRACK IN THE DOOR.
TAIL BOOM 3035 RING FRAME CRACKED AT 11:00 POSITION AT SKIN LAP.
CORROSION AND DELAMINATION OF THE ALUMINUM GROUND PLANE OF THE CABIN ROOF (CANOPY)IN SEVERAL SPOTS
WHILE ON APPROACH TO HOSPITAL LANDING PAD, THE GENERATOR DISENGAGED. IT WAS RESET, AND THE SUBSEQUENT APPROACH AND LANDING WERE COMPLETED NORMALLY. THE CREW REPORTED AN ELECTRICAL ODOR AFTER LANDING. UPON INSPECTION, NO PROBLEMS WERE FOUND WITH ELECTRICAL SYSTEM OR GENERATOR, NOR COULD THE FAILURE BE DUPLICATED. WHILE CHECKING OUT ALL ELECTRICAL SYSTEMS, USED AT TIME OF INCIDENT, AIR CONDITIONING STEM WAS TURN ON AND SAME ODOR WAS OBSERVED FROM AFT BLOWER FAN. THE MECHANIC INSPECTED THE SYSTEM, AND DETERMINED THAT IT WAS OPERATING NORMALLY. THE AIRCRAFT WAS RETURNED TO SERVICE AT THIS TIME.
SMELL OF AN UNIDENTIFIED BURNING ODOR IN COCKPIT.
DURING CRUISE FLIGHT A CREW MEMBER REPORTED SEEING SMOKE IN THE CABIN FWD OF THE STRETCHER WITH A STRANGE SMELL IN THE CABIN. THE PILOT DID NOT WITNESS THIS SMOKE, BUT RETURNED TO BASE FOLLOWING A DISCUSSION WITH THE MEDICAL CREW. ONCE ON THE GND, THE PILOT COMMUNICATED VERBALLY WITH THE MECHANIC & IT WAS DETERMINED THE MOST LIKELY CAUSE TO BE FROM AN ENG WASH THE PREVIOUS DAY. THEY PERFORMED MULTIPLE GND RUNS FOLLOWING THIS EVENT & COULD NOT DUPLICATE THE ORIGINAL DISCREPANCY REPORTED BY THE MEDICAL CREW. THE ACFT WAS RETURNED TO SERVICE AT THIS TIME.
FLIGHT ABORTED DUE TO NOISE COMING FROM OIL COOLER FAN. DETERMINED WATER WAS PRESENT IN THE CONNECTOR.
ON A POST FLIGHT INSPECTION, FOUND THE UPPER LT HORIZONTAL STABILIZER ATTACH FITTING CRACKED. REMOVED THE HORIZONTAL STABILIZER & FOUND THAT THE FITTING HAD COMPLETELY CRACKED INTO TWO SEPARATE PIECES. THE CRACK APPEARED TO PROPAGATE FROM THE TWO BOLTS THAT ATTACH THE FITTING WITH ONE OF THE BOLTS HEADS SHEARED. THIS FITTING MUST BE REPLACED. THIS AIRCRAFT CURRENTLY REMAINS OUT OF SERVICE AT THE TIME OF THIS SUBMISSION AWAITING A REPAIR TEAM.
CRACK FOUND ON RT SIDE CANOPY FRAME ATTACHMENT WEB PANEL FWD ON BELLY OF ACFT DURING AN EC130 AAIP A0560 100HOUR, 12MONTH AIRFRAME INSPECTION
PILOT WAS GOING INTO A HOVER DETECTED SOMETHING BURNING, AT THE SAME TIME LOST ALL RADIO AND ICS COMMUNICATIONS. PILOT PUT THE ACFT BACK DOWN AND NOTICED HIS PILOT ICS CIRCUIT BREAKER POPPED. AFTER EXTENSIVE TROUBLESHOOTING, MX AOG ORDERED A 2 AMP CIRCUIT BREAKER (SOURCE OF BURNING SMELL) AND REPLACEMENT AUDIO PANEL (CAUSE OF CIRCUIT BREAKER FAILURE). PARTS WERE INSTALLED AND ACFT WAS RETURNED TO SERVICE AFTER A SUCCESSFUL OPS CHECK.
DURING AN INSPECTION, HORIZONTAL STAB WAS REQUIRED TO BE REMOVED. UPON REMOVAL, THE UPPER LT MOUNT FITTING WAS FOUND BROKEN COMPLETELY IN HALF. AN EDDY CURRENT INSPECTION OF THE AREA RESULTED IN THE DISCOVERY OF THE UPPER RT FITTING BEING CRACKED ALSO.
